Alcohol use and cannabis craving in daily life: Sex differences and associations among young adults.

BACKGROUND: Alcohol and cannabis are commonly used together by young adults. With frequent pairings, use of one substance may become a conditioned cue for use of a second, commonly co-used substance. Although this has been examined for alcohol and cannabis in laboratory conditions and with remote monitoring, no research has examined whether pharmacologically induced cross-substance craving occurs in naturalistic conditions. METHODS: In a sample of 63 frequent cannabis-using young adults (54% female) who completed 2 weeks of ecological momentary assessment, we tested whether alcohol use was as

Craving induction through virtual reality cue-exposure for patients with alcohol dependence in rehabilitation treatment.

New therapeutic approaches for alcohol dependence (AD) include virtual reality (VR)-based treatments offering scalable options for cue exposure (CE), a well-established strategy in cognitive behavioral therapy. This study aimed to evaluate the feasibility and tolerability of a new VR-CE paradigm. On an explorative basis, factors influencing the induction of craving were examined. This single-arm monocentric clinical study included n = 21 patients with AD in inpatient rehabilitation treatment, that completed one VR-CE session including confrontation with alcohol-associated stimuli. Measurements

Investigating associations between appetite-regulating hormones, aggression and craving in males with cannabis use disorder.

BACKGROUND: Aggression and craving are common and important withdrawal symptoms in cannabis use disorder. The present study investigated the association between appetite-regulating hormones, aggression, and craving during cannabis withdrawal syndrome (CWS). METHODS: Fifty-six male subjects diagnosed with cannabis withdrawal and 45 healthy males were included in the study. The Substance Craving Scale, the Buss-Perry Aggression Questionnaire, and the State-Trait Anxiety Inventory were implemented at baseline. Blood samples were drawn to measure ghrelin, leptin, adiponectin, and resistin levels

Brain responses to drug cues predict craving changes in abstinent heroin users: A preliminary study.

BACKGROUND: Loss of control over drug intake occurring in drug addiction is believed to result from disruption of reward circuits, including reduced responsiveness to natural rewards (e.g., monetary, sex) and heightened responsiveness to drug reward. Yet few studies have assessed reward deficiency and related brain responses in abstinent heroin users with opioid use disorder, and less is known whether the brain responses can predict cue-induced craving changes following by prolonged abstinence. METHOD: 31 heroin users (age: 44.13±7.68 years, male: 18 (58%), duration of abstinence: 85.2 ± 52.5

Guanfacine effects on stress, drug craving and prefrontal activation in cocaine dependent individuals: preliminary findings.

Cocaine dependence is associated with increased stress and drug cue-induced craving and physiological arousal but decreased prefrontal activity to emotional and cognitive challenge. As these changes are associated with relapse risk, we investigated the effects of α2 receptor agonist guanfacine on these processes. Twenty-nine early abstinent treatment-seeking cocaine dependent individuals were randomly assigned to either daily placebo or guanfacine (up to 3 mg) for four weeks. In a laboratory experiment, all patients were exposed to three 10-min guided imagery conditions (stress/stress, drug cu

Aerobic exercise training reduces cannabis craving and use in non-treatment seeking cannabis-dependent adults.

BACKGROUND: Cannabis dependence is a significant public health problem. Because there are no approved medications for this condition, treatment must rely on behavioral approaches empirically complemented by such lifestyle change as exercise. AIMS: To examine the effects of moderate aerobic exercise on cannabis craving and use in cannabis dependent adults under normal living conditions. DESIGN: Participants attended 10 supervised 30-min treadmill exercise sessions standardized using heart rate (HR) monitoring (60-70% HR reserve) over 2 weeks. Exercise sessions were conducted by exercise physi

Written emotional expression during recovery from cocaine dependence: group and individual differences in craving intensity.

We conducted a prospective, single-blind, parallel group, controlled trial to evaluate effects of written emotional expression in patients receiving intensive treatment for cocaine dependence in a residential-unit setting. Randomization to the emotional expression treatment produced changes in blood pressure and mood during writing sessions, possibly because of its ability to stimulate active coping behavior. At an initial follow-up visit, patients that had received written emotional expression reported lower values for craving intensity and were less likely to self-report use of cocaine. Thes

Abnormal pain response in pain-sensitive opiate addicts after prolonged abstinence predicts increased drug craving.

RATIONALE: Craving is a primary feature of opiate addiction and is clinically significant because of its potential to trigger opiate use and relapse. Opiate use can also produce abnormal pain perception. We predicted that for opiate addicts (OAs), there may be an association between these two major features of addiction (drug craving and abnormal pain responses). OBJECTIVES: To examine pain responses in abstinent opiate addicts in comparison with healthy controls using a cold-pressor test (CPT) and investigate the correlations of cue-induced drug craving with pain responses. Stress-induced cocaine craving and hypothalamic-pituitary-adrenal responses are predictive of cocaine relapse outcomes.

BACKGROUND: Cocaine dependence is associated with high rates of relapse. Stress and drug cue exposure are known to increase cocaine craving and stress arousal, but the association between these responses and cocaine relapse has not been previously studied. OBJECTIVE: To examine whether stress-induced and drug cue-induced cocaine craving and hypothalamic-pituitary-adrenal axis responses evoked in the laboratory are associated with subsequent cocaine relapse. DESIGN: Prospective study design assessing cocaine relapse and drug use during a 90-day follow-up period after discharge from inpatient

A functional magnetic resonance imaging (FMRI) study of cue-induced smoking craving in virtual environments.

Smokers who are exposed to smoking-related cues show cardiovascular reactivity and smoking craving compared with their responses to neutral cues, and increased cue reactivity predicts decreased likelihood of successful cessation. Several brain imaging studies suggested four candidate brain regions that might differ in gray matter volumes and densities between smokers and nonsmokers. However, in these studies, smokers were only exposed to smoking-related objects. In our pilot study utilizing a virtual reality (VR) technique, virtual environments (VEs) were more immersive and evoked smoking crav

Substance-related intrusive memories in cocaine use disorder are different from but associated with craving.

BACKGROUND AND AIMS: Cocaine use disorder (CUD) is a significant global health concern, characterized by persistent craving despite severe consequences. Recent theories highlight maladaptive memory processes - such as intrusive, vivid recollections of past substance use that arise spontaneously in daily life - as key contributors to craving and relapse; however, empirical studies examining such substance-related intrusions in naturalistic contexts remain scarce. This study therefore used ecological momentary assessment (EMA) to investigate the frequency, phenomenology and emotional and behavio

Craving self-reports as outcome measures in drug addiction trials: A systematic review of ClinicalTrials.gov.

BACKGROUND AND AIMS: The subjective experience of drug craving is characterized by an overwhelming urge to consume substances. Due to strong validity and ease of use, self-report measures are widely employed to assess substance-related motivational dynamics. Multi-item questionnaires are increasingly favored for capturing the multidimensional nature of craving, providing valuable insights for clinical care and relapse prediction. This review aimed to summarize craving self-report measurement tools registered in clinical trials and examine their methodological parameters. METHODS: A search was

Comparative Study of Different Anti Craving Medication for Alcohol Dependence and Their Effect on Relapse Rate.

Background Alcohol use disorder is a chronic medical condition with a multifactorial aetiology. Food and drug administration approved treatment options for alcohol dependence syndrome includes: Naltrexone, Acamprosate, and Disulfiram and Topiramate as an off label drug. The lack of data specific to the Nepalese population pose significant challenges. Objective To fill that gap by assessing the effectiveness of Naltrexone, Disulfiram, Topiramate and Acamprosate in relapse prevention Method Hospital based cohort study conducted over a period of two months. Approach Bias and Tobacco Craving as Mechanisms of the Effect of Approach Bias Retraining on Smoking Cessation.

INTRODUCTION: Approach bias, the automatic tendency to advance toward, rather than move away from appetitive cues, has been associated with greater tobacco cravings, dependence, and likelihood of smoking relapse. Approach bias retraining (ABR) has emerged as one way to reduce approach bias and promote avoidance of smoking cues. Yet, additional research is needed to identify the mechanisms that may help explain the effect of ABR on smoking cessation. AIMS AND METHODS: The current study uses data collected as part of a randomized controlled trial to test two unique mechanisms of action ([1] app

Disparities in Tobacco use and cravings among sexual and gender minority adolescents in the United States.

BACKGROUND: Despite well-documented disparities in tobacco use between sexual and gender minority (SGM) populations and their non-SGM counterparts, limited research has focused on tobacco cravings within these groups. This study examines tobacco use and tobacco cravings among SGM adolescents who are past 30-day tobacco product users in the United States. METHODS: Data were derived from the 2023 National Youth Tobacco Survey of high school students. Multivariable logistic regression analyses were conducted to examine the association between various SGM identities and tobacco use and cravings.

Temporal Dynamics of Smoking Urges: Investigating Morning Cravings, Nicotine Dependence, and Smoking Behaviors.

BACKGROUND: Understanding the relationship between different dimensions of nicotine cravings and smoking behavior is crucial for addressing nicotine dependence. This study examined how morning cravings and past-week cravings relate to nicotine intake, as measured by salivary cotinine, and the mediating role of cigarettes per day (CPD) and time to first cigarette (TTFC). METHODS: Data were obtained as part of the Pennsylvania Adult Smoking Study (PASS), a community-based study of adult smokers. Participants (N = 353) completed structured questionnaires assessing smoking behaviors, cravings, an

The relationship between nicotine craving and state anxiety in daily and non-daily nicotine users.

Anxiety as a broad construct has been linked to nicotine craving, frequency of use, and relapse rates. However, there is limited research on the relationship between state anxiety and nicotine craving. This study explored the association between state anxiety and pre-cue versus post-cue craving among adults who use nicotine and tobacco products (NTPs) at different frequencies. Participants with either non-daily (n = 34) or daily (n = 80) NTP use completed a laboratory assessment including a virtual reality (VR) NTP cue-exposure paradigm and self-report assessments of state anxiety and nicotine

Effects of cognitive reappraisal on gaming craving and inhibitory control in adolescents with internet gaming disorder.

OBJECTIVE: According to the dual-system theory of addiction, increased gaming craving and diminished inhibitory control constitute the core mechanisms underlying internet gaming disorder (IGD). However, developing effective interventions targeting these mechanisms remains challenging. The present study aimed to investigate the effects of cognitive reappraisal strategies on gaming craving and inhibitory control in adolescents with IGD. METHOD: Experiment 1 recruited 60 adolescents with IGD (Mage ± SE = 15.58 ± 0.12, all male and of Asian descent), employing a cue-reactivity paradigm to examine

Physical sensations of craving in binge drinking and severe alcohol use disorder: A phenomenological approach.

BACKGROUND: Craving is a key concept in addictive disorders. However, despite seminal results identifying the pivotal role of its physical component, craving remains conceptualized as being centrally underpinned by psychological and cognitive processes. To explore the phenomenological dimension of physical craving in addiction, we measured self-reported body sensations of craving among individuals with subclinical (binge drinking, BD) or clinical (severe alcohol use disorder, SAUD) alcohol use disorders. METHODS: We used a body mapping technique, allowing for the free report of the bodily cou

Shared orbitofrontal dynamics to a drug-themed movie track craving and recovery in heroin addiction.

Movies captivate groups of individuals (the audience), especially if they contain themes of common motivational interest to the group. In drug addiction, a key mechanism is maladaptive motivational salience attribution whereby drug cues outcompete other reinforcers within the same environment or context. We predicted that while watching a drug-themed movie, where cues for drugs and other stimuli share a continuous narrative context, functional MRI responses in individuals with heroin use disorder (iHUD) will preferentially synchronize during drug scenes. Thirty inpatient iHUD (24 male) and 25
