Ileocolic neobladder in the woman with interstitial cystitis and a small contracted bladder.

Bladder substitution with an ileocolic segment after total cystectomy was performed in 5 women with a severely contracted bladder secondary to interstitial cystitis. The urethra was divided at the bladder neck in 2 patients and distal to it in 3. The latter 3 patients empty the bladder to completion and void naturally, while the former 2 have a large post-void residual volume and require intermittent self-catheterization. Mean patient age was 50 years and mean followup was 52 months. All patients have complete diurnal and nocturnal continence, and are free of all symptoms related to interstiti

Intravesical hyaluronidase causes chronic cystitis in a rat model: a potential model of bladder pain syndrome/interstitial cystitis.

OBJECTIVES: To determine whether a potential rat model of bladder pain syndrome could be developed through long-term intermittent intravesical hyaluronidase. METHODS: A total of 64 female Sprague-Dawley rats were divided into a control group, a low-dose hyaluronidase (1 mg/mL) group, a high-dose hyaluronidase (4 mg/mL) group and a hyaluronic acid-treated group. Hyaluronidase was given intravesically three times a week for 1 month. Hyaluronic acid (0.5 mL, 0.8 mg/mL) was introduced intravesically to hyaluronidase-treated rats' bladders. Histological changes, cystometry, nociceptive behaviors,

Role of patch cystoplasty in intractable interstitial cystitis/bladder pain syndrome.

PURPOSE: Management of intractable interstitial cystitis/bladder pain syndrome (IC/BPS) is very challenging with no standard treatment. Reconstructive surgery is one of the options. Here, we present our long-term experience of treating intractable IC/BPS with patch/clam ileocystoplasty. PATIENTS AND METHODS: From September 2012, 19 patients diagnosed with IC/BPS underwent patch cystoplasty. Two patients died, so data from 17 patients was analyzed. 15-20 cm segment of the ileum was isolated, leaving distal most 15 cm of ileum from the ileocecal junction intact. The urinary bladder was opened j

Establishing Evidence for the Painful Intercourse Self-Efficacy Scale-Interstitial Cystitis.

BACKGROUND: Although female sexual dysfunction (FSD) and low sexual self-efficacy are common in patients with interstitial cystitis/bladder pain syndrome (IC/BPS), existing measures of these constructs do not fully capture unique challenges faced by patients with IC/BPS, such as managing sex-related symptom exacerbations, experiencing rewarding sexual activity, and maintaining intimate relationships. To address the lack of tailored measurement of FSD and sexual self-efficacy in patients with IC/BPS, we aimed to adapt the Painful Intercourse Self-Efficacy Scale-Interstitial Cystitis (PISES-IC)

Experimental murine models of interstitial cystitis/bladder pain syndrome: A review.

Interstitial cystitis/bladder pain syndrome (IC/BPS) is a chronic enigmatic disease of the urinary bladder characterized by persistent bladder/pelvic pain in conjunction with lower urinary tract symptoms. IC/BPS is categorized as either Hunner-type IC (HIC) or BPS based on the presence/absence of the Hunner lesion, a reddish mucosal lesion in the bladder. HIC and BPS present with similar symptoms, however, the etiologies are completely different. Recent evidence suggests that HIC is an immune-mediated inflammatory disease of the urinary bladder. In contrast, BPS, other forms of HIC lacking Hun

Immune cell profiles of patients with interstitial cystitis/bladder pain syndrome.

Interstitial cystitis/bladder pain syndrome (IC/BPS) is a disorder characterized by bladder pain upon filling which severely affects quality of life. Clinical presentation can vary. Local inflammatory events typify the clinical presentation of IC/BPS patients with Hunner lesions (IC/BPS-HL). It has previously been proposed that B cells are more prevalent in HL, but understanding their exact role in this environment requires a more complete immunological profile of HL. We characterized immunological dysfunction specifically in HL using immunohistochemistry. We detected significantly more plasma

The bacterial microbiota of Hunner lesion interstitial cystitis/bladder pain syndrome.

OBJECTIVE: To undertake the first comprehensive evaluation of the urinary microbiota associated with Hunner lesion (HL) interstitial cystitis/bladder pain syndrome (IC/BPS). Despite no previous identification of a distinct IC/BPS microbial urotype, HL IC/BPS, an inflammatory subtype of IC/BPS, was hypothesized most likely to be associated with a specific bacterial species or microbial pattern. PARTICIPANTS AND METHODS: The bacterial microbiota of midstream urine specimens from HL IC/BPS and age- and gender-matched IC/BPS patients without HL (non-HL IC/BPS) were examined using the pan-bacteria

Painful Bladder Syndrome/Interstitial Cystitis and High Tone Pelvic Floor Dysfunction.

This article provides an overview of 2 conditions that defy straightforward characterization and treatment: interstitial cystitis/painful bladder syndrome often coexists with high tone pelvic floor dysfunction. These conditions are common in gynecologic patients who present with chronic pelvic pain and are often misdiagnosed due to their syndromic nature and amorphous definitions. Clinicians should maintain a high level of suspicion for these processes in patients with chronic pelvic pain or recurrent urinary tract infection symptoms. Optimal treatment uses a multimodal approach to alleviate s

Biomaterial-assisted drug delivery for interstitial cystitis/bladder pain syndrome treatment.

Interstitial cystitis/bladder pain syndrome (IC/BPS) is a chronic and painful bladder condition afflicting patients with increased urinary urgency and frequency as well as incontinence. Owing to the elusive pathogenesis of IC/BPS, obtaining effective therapeutic outcomes remains challenging. Current administrational routes such as intravesical-bladder injection improve the treatment efficacy and reduce systemic side effects. However, the bladder permeability barrier hinders drug penetration into the bladder wall to meet the desired therapeutic expectation. These issues can be addressed by enca

[Minimally invasive methods of treatment of interstitial cystitis/painful bladder syndrome].

Interstitial Cystitis (IC) or Painful Bladder Syndrome (PBS) is a heterogeneous disease which pathogenetic mode development is not fully studied. In the presented literature review IC/PBS is considered as urothelial dysfunction. The main issues discussed relate to IC/PBS diagnosis and minimally invasive methods of its treatment including intravesical instillations of therapeutic agents, botulinum toxin injections into bladder wall and hydrodistension. Efficacy of the above therapeutic methods is far from the desired what gives rise to search novel minimally invasive technologies and protocols

Prodrome and Non-prodrome Phenotypes of Bladder Pain Syndrome/Interstitial Cystitis.

OBJECTIVE: To test the hypothesis that risk factors for bladder pain syndrome/interstitial cystitis (BPS/IC) in women differ between those with and without the BPS/IC prodrome. MATERIALS AND METHODS: Incident cases of BPS/IC and healthy controls were recruited nationally. More than half the BPS/IC cases reported subsyndromal urinary symptoms for decades before onset of BPS/IC and were identified as having the prodrome. Risk factors for BPS/IC were examined separately for cases with and without the prodrome using a set of matched controls. RESULTS: Two risk factors distinguished 178 prodrome

Cystoscopic evaluation and clinical phenotyping in interstitial cystitis/bladder pain syndrome.

Herein, we aimed to review, report, and discuss the role of cystoscopy and clinical phenotyping in interstitial cystitis/bladder pain syndrome (IC/BPS). For this purpose; a comprehensive nonsystematic review of the relevant literature was conducted. We reviewed articles published in English and indexed in the PubMed, Embase, and Google Scholar databases. Original manuscripts, review articles, case series, and case reports were taken into consideration. Data regarding the indications for, technique, and possible findings of cystoscopy with hydrodistension (HD) and biopsy, as well as clinical im

Alexithymia and anesthetic bladder capacity in interstitial cystitis/bladder pain syndrome.

OBJECTIVE: In contrast to the inconsistent results of organic causes, it has been found that psychological risk factors are reliably related to functional somatic syndromes (FSSs), including interstitial cystitis/bladder pain syndrome (IC/BPS). Compared to patients with acute cystitis, a subgroup of IC/BPS patients with a history of childhood relational trauma reported intensified unregulated affective states (i.e., anxiety and depression) and trauma-related psychopathology (i.e., dissociation). Nevertheless, it remains unknown whether psychosocial risk factors can be separated from bladder-ce

Correlation Between Bladder Pain Syndrome/Interstitial Cystitis and Pelvic Inflammatory Disease.

Pelvic inflammatory disease (PID) has been investigated in Western countries and identified to be associated with chronic pelvic pain and inflammation. Bladder pain syndrome/interstitial cystitis (BPS/IC) is a complex syndrome that is significantly more prevalent in women than in men. Chronic pelvic pain is a main symptom of BPS/IC, and chronic inflammation is a major etiology of BPS/IC. This study aimed to investigate the correlation between BPS/IC and PID using a population-based dataset. We constructed a case-control study from the Taiwan National Health Insurance program. The case cohort co

Practical use of the new American Urological Association interstitial cystitis guidelines.

The American Urological Association recently developed guidelines to assist clinicians who evaluate and treat interstitial cystitis/bladder pain syndrome. Knowledge in this area continues to advance, and some of the guideline statements differ from what clinicians may have been previously taught. This review includes the 27 guideline statements, which address both evaluation and treatment. This review lists the guideline statements and, when applicable, comments on their practical implementation and the most recent research. Practical information includes the following: key questions that help

Potential targeting of siglecs, mast cell inhibitory receptors, in interstitial cystitis.

Mast cell increases and activation are detected in the chronic inflammatory bladder disease interstitial cystitis (IC), and their proinflammatory mediators are felt to contribute to regional pelvic pain and inflammatory pathophysiology. The immunoreceptor tyrosine-based inhibition motif-containing sialic acid-binding immunoglobulin-like lectins (Siglecs) expressed in mast cells could be evaluated as in vivo signaling regulators capable of inhibiting IC-related mast cell activation.

Symptom profile variability of interstitial cystitis/painful bladder syndrome by age.

UNLABELLED: Study Type--Diagnostic (symptom prevalence). Level of Evidence 2b. What's known on the subject? And what does the study add? Symptom variability is profound in the population of patients diagnosed with interstitial cystitis and painful bladder syndrome because these diagnoses represent a very heterogeneous patient population. Recently, younger patients are being diagnosed with these pain syndromes, without any specific investigation into the symptoms they experience. We identified that patients diagnosed with interstitial cystitis and painful bladder syndrome have variable clinical
