[Masquerade lacrimal duct stenosis: prevalence of lacrimal sac tumors].

BACKGROUND: Tumors of the lacrimal sac are a rare entity. Malignancies are often associated with a high mortality rate. Lacrimal sac tumors often present as lacrimal duct stenosis with dacryocystitis and are often only diagnosed after an intraoperative biopsy. The aim of this study was a retrospective analysis of all cases with lacrimal sac tumors, which presented with a lacrimal duct stenosis at this medical center in the past 10 years. METHODS: All patients who were treated by external dacryocystorhinostomy (DCR) at the University Medical Center Münster during the period January 2009-Decemb…

Primary Mucoepidermoid Carcinoma of the Lacrimal Sac -  a Case Report and Literature Review.

BACKGROUND: Lacrimal sac tumors are very rare and are often missed because patients present with features consistent with chronic dacryocystitis. Squamous cell carcinoma is the common-est lacrimal sac malignancy. Although primary mucoepidermoid carcinomas of the lacrimal sac are rare, they are locally aggressive. Furthermore, their proximity to vital structures and the skull base makes them potentially life-threatening. Multidisciplinary management is required, and wide excision followed by chemoradiation is the recommended treatment. CASE: Here, we report a 65-year-old male who presented wit…

Nonsurgical retrieval of retained lacrimal stenting material.

PURPOSE: The aim was to describe a novel technique to retrieve retained stenting material from the lacrimal system. METHODS: In this consec … icone on every occasion. In all children, there was complete resolution of the epiphora and chronic dacryocystitis without need for dacryocystorhinostomy. CONCLUSIONS: Retained stenting material can be responsible for persistent obstructive symptoms and chronic dacryocystitis. Patients examined for the clinical triad described should raise suspicion of the poss …

A simple and evolutional approach proven to recanalise the nasolacrimal duct obstruction.

AIM: To evaluate a new approach of recanalisation of nasolacrimal duct obstruction (RC-NLDO) in the treatment of the nasolacrimal duct obstruction (NLDO) and chronic dacryocystitis. METHODS: 583 patients with 641 eyes suffering from NLDO and chronic dacryocystitis were enrolled in this study. The RC-NLDO was performed in 506 eyes, with 135 eyes undergoing external dacryocystorhinostomy (EX-DCR) as controls. Patient follow-up for 54 months was evaluated by symptoms, dye disappearance test, lacrimal irrigation and digital subtraction dacryocystogram. The RC-NLDO was also performed in 12 rhesus …

Nasolacrimal duct obstruction following transantral-ethmoidal orbital decompression.

Of 200 patients undergoing bilateral transantral-ethmoidal orbital decompression for Graves' ophthalmopathy, nine suffered symptomatic postoperative obstruction of the lower nasolacrimal excretory system. Five of the nine patients had persistent symptoms of epiphora or repeated episodes of dacryocystitis that necessitated dacryocystorhinostomy. Dacryocystorhinostomy relieved symptoms and objective evidence of obstruction in these five patients. We believe that nasolacrimal duct obstruction occurred in these patients as a complication of nasoantral window construction.

Lacrimal surgery in children.

Of 142 children who underwent major lacrimal surgery (160 dacryocystorhinostomies, one congenital fistula excision, one dacryocystectomy, and one canaliculostomy), 49 had congenital nasolacrimal duct obstruction (34%). Lacrimal obstruction was also associated with canaliculitis, punctal agenesis, trauma, congenital fistula, dacryocystitis, craniofacial defects, and functional eyelid abnormalities. Although the timing and technique of the surgery varied according to the abnormalities, an overall functional success rate of 90% (144 of 160 procedures) was achieved.

Lacrimal drainage obstruction from lacrimal sac infiltration by lymphocytic neoplasia.

In two patients, a 52-year-old woman with chronic lymphocytic leukemia and a 71-year-old woman with malignant lymphoma, lymphocytic neoplasia infiltrated the lacrimal sac, obstructed the lacrimal drainage system, and resulted in acute or chronic dacryocystitis. The prognosis of lymphocytic neoplasia is sufficiently good that definitive treatment is usually warranted. Both patients benefitted from dacryocystorhinostomy with placement of silicone tubing in concert with management of the neoplasia and hemoglobin level.

The Elusive Diagnosis: A Case of Extranodal Natural Killer (NK)/T-Cell Lymphoma.

Extranodal natural killer (NK)/T-cell lymphoma is a rare malignant tumor, more prevalent in Asia and Latin America, and associated with Epstein-Barr virus infection. Its diagnosis is challenging due to its clinical presentation, which may mimic chronic sinusitis, dacryocystitis, or granulomatous diseases. This lymphoma follows a rapid and aggressive course, with a poor prognosis. We report the case of a 71-year-old man presenting with nasal obstruction, clear rhinorrhea, and facial heaviness. Initially, acute dacryocystitis and periorbital cellulitis were suspected and treated with antibiotics…

Dacryoliths causing intermittent epiphora associated with a patent lacrimal system.

We present two patients with dacryoliths and patent lacrimal drainage with intermittent tearing and without infection. Dacryoliths can be present in the lacrimal sac or lacrimal duct without acute or chronic inflammation. In these cases, we believe dacryolith formation was a causative factor of intermittent epiphora even with a patent drainage system, and we propose that dacryoliths and even fungal colonization formation may be the first event before dacryocystitis and should be considered as a cause of epiphora.

[Pathological changes found in the lacrimal sac during nasolacrimal duct surgery].

In the course of 489 external dacryocystorhinostomies one found: 19 pseudotumors (granulomas and inflammatory granulations), 6 true tumors (1 papilloma, 1 congenital cyst, 1 lymphoma), 6 cases of calcium deposits and stones and 1 case of epithelial metaplasia. The author points to a great number of pseudotumors which are the effect of a long-standing process of dacryocystitis treated conservatively without effect. She thinks that the surgical treatment should be undertaken as fast as possible.

Case Report: Ocular Sporotrichosis-A Seven-Case Series.

Ocular infections associated with sporotrichosis can present four clinical manifestations: granulomatous conjunctivitis, dacryocystitis, Parinaud oculoglandular syndrome, and bulbar conjunctivitis. The incidence of ocular sporotrichosis related to zoonotic transmission has significantly increased in endemic regions and is a frequently misdiagnosed cause of granulomatous conjuntivitis. Therefore, we present a series of seven cases of eye injury by Sporothrix strains, including clinical forms, therapeutic approaches, and laboratory procedures to alert health professionals who provide care to the…

Acute bacterial infections of the eye: their aetiology and treatment.

The aerobic microbial flora of 823 eyes with acute bacterial conjunctivitis, corneal ulceration, dacryocystitis, discharging sockets, blepharitis, and postoperative infections has been investigated. Staphylococci, Streptococcus pneumoniae, and nonfermenting Gram-negative bacilli including Pseudomonas aeruginosa were the main pathogens. Fermenting coliforms were also infrequently isolated. The least overall resistance of 30.9% was to chloramphenicol, but no one antibiotic was effective against all pathogens. Cloxacillin is the best drug for staphylococcal and polymyxin B for pseudomonas infecti…
