Surgical decompression of the nerve for facial palsy in temporal bone trauma: Insights from a three-decade experience.

AIMS: To evaluate the surgical outcomes of facial nerve decompression for facial paralysis in temporal bone fractures. MATERIAL AND METHODS: A retrospective review was conducted on 11 patients who underwent surgery for facial paralysis due to temporal bone trauma in our ENT department between March 1994 and July 2024. Inclusion criteria were failure of corticosteroid therapy and persistence of severe facial paralysis. Surgical outcomes were assessed using the House-Brackmann (HB) classification at one year and by monitoring postoperative HB progression. Potential prognostic factors included d

Post-traumatic hydrocephalus after decompressive craniectomy: a multidimensional analysis of clinical, radiological, and surgical risk factors.

Decompressive craniectomy is a key treatment for refractory intracranial pressure after severe traumatic brain injury (TBI). Post-traumatic hydrocephalus (PTH) occurs in 7.6-36% of cases, and early diagnosis significantly improves rehabilitation outcomes. This retrospective study analyzed risk factors for shunt-dependent PTH in 126 TBI patients (93 men, 33 women, median age 53 years). Patients were divided into those requiring shunts and those who did not. Clinical and radiological characteristics, including volumetric measurements and surgical techniques, were assessed using SPSS® Statistics

Prior Thrombectomy Does Not Affect the Surgical Complication Rate of Decompressive Hemicraniectomy in Patients with Malignant Ischemic Stroke.

BACKGROUND: Even though mechanical recanalization techniques have dramatically improved acute stroke care since the pivotal trials of decompressive hemicraniectomy for malignant courses of ischemic stroke, decompressive hemicraniectomy remains a mainstay of malignant stroke treatment. However, it is still unclear whether prior thrombectomy, which in most cases is associated with application of antiplatelets and/or anticoagulants, affects the surgical complication rate of decompressive hemicraniectomy and whether conclusions derived from prior trials of decompressive hemicraniectomy are still v

Surgical timing and indications for decompressive craniectomy in malignant stroke: results from a single-center retrospective analysis.

PURPOSE: Acute ischemic stroke induces rapid neuronal death and time is a key factor in its treatment. Despite timely recanalization, malignant cerebral infarction can ensue, requiring decompressive surgery (DC). The ideal timing of surgery is still a matter of debate; in this study, we attempt to establish the ideal time to perform surgery in this population. METHODS: We conducted a retrospective study of patients undergoing DC for stroke at our department. The indication for DC was based on drop in level of consciousness and standard imaging parameters. Patients were stratified according to

Bow hunter's syndrome successfully treated with a posterior surgical decompression approach: A case report and review of literature.

BACKGROUND: Bow hunter's syndrome (BHS) is a rare but surgically treatable cause of vertebrobasilar insufficiency due to dynamic rotational occlusion of the vertebral artery. Typically, patients present with posterior circulation transient ischaemic symptoms such as presyncope, syncope, vertigo, diplopia, and horizontal nystagmus, but irreversible deficits, including medullary and cerebellar infarctions, have also been described. CASE SUMMARY: A 70-year-old patient presented an acute onset of vertigo and gait instability triggered by right head rotation. His medical history included previous

Virtual reality-based evaluation of neurovascular conflict for the surgical planning of microvascular decompression in trigeminal neuralgia patients.

OBJECTIVE: Trigeminal neuralgia (TN) is a lightning bolt of violent, electrifying, and stinging pain, often secondary to the neurovascular conflict (NVC). The vessels involved in NVC are mostly arteries and rarely veins. Evaluation of NVC in the deep infratentorial region is inseparably connected with cranial imaging. We retrospectively analyzed the potential influence of three-dimensional (3D) virtual reality (VR) reconstructions compared to conventional magnetic resonance imaging (MRI) scans on the evaluation of NVC for the surgical planning of microvascular decompression in patients with TN

The results of surgical decompression in the treatment of foot drop due to peroneal nerve entrapment after bariatric surgery.

BACKGROUND: The loss of the fat pad surrounding the fibular head after rapid and excessive weight loss after bariatric surgery can lead to foot drop symptoms due to peroneal nerve entrapment (PNE). Conservative and surgical approaches have been described for the treatment of this condition, but there is some controversy over the effectiveness of the treatment modalities. OBJECTIVE: We aimed to investigate the causes and frequency of foot drop due to PNE after bariatric surgery and to investigate the effects of peroneal nerve decompression (PND) as a surgical treatment for PNE. SETTING: Singl

Pain-Free Outcomes and Durability of Surgical Intervention for Trigeminal Neuralgia: A Comparison of Gamma Knife and Microvascular Decompression.

OBJECTIVE: Treatment options for trigeminal neuralgia include microvascular decompression (MVD) and Gamma Knife surgery (GKS). There is no consensus which option is more effective at providing immediate and long-lasting pain relief. This study evaluated the differences between these 2 options in terms of rates of complete pain relief and pain-free recurrence. METHODS: A systematic review was conducted of published studies of MVD and GKS for treatment of trigeminal neuralgia from 2004 to 2014. Studies were selected using a MEDLINE/PubMed search and from subsequent inspection of references from

Posterior Extensive Circumferential Decompressive Reconstructive Technique in Surgical Treatment of Upper Thoracic Spine Compressive Lesions.

BACKGROUND: Complexity in ventral surgical exposure and presence of scapula and large parascapular musculature during posterior surgery are the main obstacles in operative treatment of upper thoracic spine compressive lesions (UTSCLs), such as trauma, tuberculosis, and neoplasm. Recently, the advantages of combined ventrodorsal surgery could be successfully gained using a 1-stage posterior salvage technique with enough accessibility to all 3 spinal columns. This was a retrospective evaluation of 35 patients with UTSCLs treated by the posterior extensive circumferential decompressive reconstruc

L5 nerve root decompression after malunion of surgically managed vertically unstable pelvic ring injuries.

We describe the outcomes of late decompression of the L5 nerve root after malunion of surgically managed pelvis injuries. Four patients underwent decompression of the L5 nerve root. Surgery included hemilaminotomy with facetectomy at L5-S1 followed by decompression of the L5 nerve root laterally from the surrounding displaced sacral ala. L5-S1 fusion was not performed. Radiographs and Oswestry Disability Index (ODI) scores were obtained for each patient at latest follow-up. In all patients, adequate decompression required removal of bone to the anterior aspect of the sacral ala inferiorly to t

Internal decompressive craniectomy with craniotomy: a novel surgical therapy of giant frontal mucocele complicated by subdural empyema.

INTRODUCTION: Giant frontal mucocele (GFM) is an extremely rare cause of frontal lobe syndrome. Subdural empyema (SDE) is an uncommon complication of paranasal sinisutis, for which craniotomy and decompressive craniotomy are the most effective surgical procedures. CASE REPORT: A 54-year-old man was brought unconscious to the Emergency Room where recurrent generalized seizures occurred. Heroine abuse, HCV related hepatitis, prolonged antibiotic therapy for treatment of purulent rhinorrhea, along with recent personality changes, was reported. High white blood cell count, pansinusitis, GFM, SDE

In vitro and in vivo surgical and MRI evidence to clarify the effectiveness of the vascular tunnel technique in the course of decompressive craniectomy.

OBJECTIVE: To demonstrate the efficacy of a vascular protection technique during decompressive craniectomy (DC) which can reduce the risk of secondary venous infarction due to the blocking pressure for venous outflow through bridging veins. METHOD: The observation was carried out In vitro (cadaver) and in vivo (surgery and magnetic resonance imaging) in order to verify the durability of the vascular tunnel. RESULTS: in vivo observation proved the durability of vascular tunnel even 2 months later. The cadaveric experimental model showed that after the 12×13 cm size DC had been obtained, the o

Surgical or medical decompression as a first-line treatment of optic neuropathy in Graves' ophthalmopathy? A randomized controlled trial.

OBJECTIVE: Only a small percentage of Graves' ophthalmopathy (GO) patients develop optic neuropathy with impending loss of visual acuity. Therapy with methylprednisolone pulses is the treatment of first choice in severe and active GO patients. When the effect is insufficient, patients are usually treated with surgical decompression. We investigated whether surgery could become the first-line treatment, thus preventing treatment with steroids. DESIGN AND SUBJECTS: We performed a randomized trial in 15 patients with very active GO and optic neuropathy. Six patients were treated with surgical de
