Dorsalis pedis flap with vascularized extensor tendons for dorsal hand reconstruction.

We describe three cases of combined loss of skin and tendons on the dorsum of the hand treated with the use of cutaneotendinous dorsalis pedis free flap. The functional and aesthetic results are excellent in the hand and aesthetically acceptable in the foot. This technique is the first choice when the treatment of these injuries requires three or four tendon grafts.

Plug the needle hub during subcision for a bloodless field.

BACKGROUND: Subcision is a simple such procedure with considerable efficacy especially in the management of the rolling type of atrophic post-acne scars. Since its introduction in 1995, the technique has undergone several modifications to enhance its results as well as to improve the ergonomics. However, 'gushing' of blood through the hub of the needle smears the surgeon's gloves and the treatment field, which is not desirable both aesthetically and hygienically. AIM: To describe a simple technique to avoid blood spillage to the treatment field while doing subcision. METHOD: While doing subc

Microsurgical Replantation of the Posterior Heel Skin and Subcutaneous Tissue.

A 45-year-old male presented with amputation of posterior heel soft tissue after an accident involving broken glass panes. The defect measured 4 × 6.5 cm, and the composite amputated tissue consisted of skin and subcutaneous layers. Intraoperative exploration of the amputated tissue revealed 2 perforator arteries (diameter ≤0.4 mm) in the central portion of the subcutaneous tissue and 4 superficial veins (diameter ≈0.6 mm) in the wound margin. These vessels were anastomosed end-to-end to the corresponding vessels from the wound bed using 11-0 nylon sutures. The replanted tissue survived comple

Repair of Auricular Defects.

Repairing defects of the auricle requires an appreciation of the underlying 3-dimensional framework, the flexible properties of the cartilages, and the healing contractile tendencies of the surrounding soft tissue. In the analysis of auricular defects and planning of their reconstruction, it is helpful to divide the auricle into subunits for which different techniques may offer better functional and aesthetic outcomes. This article reviews many of the reconstructive options for defects of the various auricular subunits.

Repair of bilateral clefts of lip, alveolus and palate. Part 2: Concomitant lip closure and columella lengthening after lip adhesion.

Lip repair and synchronous columella lengthening in bilateral clefts of the lip, alveolus and palate following lip adhesion according to the method outlined in Part 1 is described in this part of the paper. Together with lip and nose repair the gingivo-periosteoplasty can also be performed when the alveolar process is perfectly aligned and the greater and lesser segments abutt onto each other.

[Repairing the defects in the chest, back and axilla with a split-breast flap].

OBJECTIVE: To evaluate a method to repair the defects after the secondary tumor excision and radiation ulcer in the chest, back and axilla. METHODS: Eight patients, with the defects after the secondary tumor excision and the radiation ulcer in the chest, back and axilla, were undergoing the treatment. A "T" shape incision or up-side-down "T" shape incision was designed above the breast or along the inframammary fold below breast, just close to the defect. A split-breast flap was raised above the pectoralis major or deep fascia. The defect was then repaired with a rotating and advancing way.

Long-Term Outcomes of the Minimal Skin Incision Technique for Correcting Severe Microform and Minor-Form Cleft Lip With Philtrum Reconstruction Through the Intraoral Incision.

BACKGROUND: Because the traditional technique is known to cause visible scarring, it is challenging to yield optimal outcomes while treating a severe type of microform and minor-form cleft lip. The authors present a new refined technique with minimal skin incision and philtrum formation through an intraoral incision. METHODS: The surgical technique involves single Z-plasty or double or triple unilimb Z-plasty to restore an elevated cupid's bow peak and overlapping of an orbicularis oris muscle flap to create the philtrum through an intraoral incision. Cleft lip nasal deformity was corrected w

The use of tissue expanders for resurfacing of the penis for hypospadias cripples.

OBJECTIVE: To review our experience with tissue expanders for resurfacing the penis in selected cases of complex hypospadias repairs. MATERIALS AND METHODS: We reviewed the medical records of 6 hypospadias male patients who underwent tissue expansion for phallic skin resurfacing. The patients ranged in age from 14-29 years of age at the time of placement of the tissue expander. All 6 patients had 5-8 failed surgeries and were "hypospadias cripples." In each case, the injection port was placed in the suprapubic area, whereas the tissue expander was placed under the skin on the dorsum of the pe

A modified Ferguson hemorrhoidectomy for circumferential prolapsed hemorrhoids with skin tags.

PURPOSE: We describe a modification of the Ferguson hemorrhoidectomy for circumferential prolapsed hemorrhoids. This details the operative procedure and compares the results of other radical methods in patients with circumferential hemorrhoidal disease. METHODS: A total of 738 patients (mean age, 43 (range, 19-83) years) were treated with our modified Ferguson method under sedative analgesia between 1989 and 2004: 576 patients had Grade III and 162 patients had Grade IV hemorrhoids, and 131 patients also had a partial lateral internal sphincterotomy to correct anal hypertonia. RESULTS: Posto

Excision of benign pigmented skin tumours by deep shaving.

Benign pigmented skin tumours are frequently removed by shave excision. There is little information as to how often the method achieves a complete excision of the lesion and how often the full thickness of the dermis is removed. 43 benign pigmented skin tumours, on the trunk and proximal parts of the limbs, were removed by deep shaving and the shaved areas were then removed by elliptical excision. The shave excision achieved complete tumour removal in 72% of the lesions and did not divide the dermis to fat junction in 63%. Both objectives were achieved in 46%. Shave excision of these lesions m

Simple decompression of the ulnar nerve at the elbow via proximal and distal mini skin incisions.

The purpose of the present study was to describe a new minimally invasive surgical technique for decompression of the ulnar nerve at the elbow for treatment of cubital tunnel syndrome. Four patients underwent surgical treatment for cubital tunnel syndrome. Preoperative clinical states were classified by using the McGowan grading system and the postoperative states were recorded by using the Wilson and Krout grading system. Preoperative and last follow-up electromyographic results were also recorded. At the last follow-up, three patients were recorded as excellent and one patient was recorded a

Total lip reconstruction with tendinofasciocutaneous radial forearm flap.

INTRODUCTION: Squamous cell carcinoma is a common tumour of lower lip. Small defects created by surgical resection may be readily reconstructed by linear closure or with local flaps. However, large tumours resection often results with microstomia and oral incompetence, drooling, and speech incomprehension. The goal of this study is to describe our experience with composite free radial forearm-palmaris longus tendon flap for total or near total lower lip reconstruction. PATIENTS AND METHODS: This procedure was used in 5 patients with 80-100% lip defect resulting from Squamous cell carcinoma. P

[Neurovascular flaps for the reconstruction of fingertip injuries].

The fingertip is an extremely specialized end organ with a highly developed sense of touch. In this article, we present the different ways of reconstructing a traumatized fingertip. Additionally, we systematically explain the differential indications for the different techniques available. The whole spectrum of fingertip reconstruction is discussed, starting from local neurovascular flaps (palmar VY-Atasoy flap, Kutler flap, palmar Moberg flap, lateral Venkataswami and Subramanian flap, palmar Hueston flap), distant flaps (Littler flap, Foucher flap, dorsal metacarpal artery flap, cross-finger

Treatment of Cutaneous Pseudolymphoma: A Systematic Review.

Cutaneous pseudolymphoma (CPL) is a reactive polyclonal T- or B-cell lymphoproliferative process. CPL may appear as localized or disseminated skin lesions. While most cases of CPL are idiopathic, they may also occur as a response to, for example, contact dermatitis, arthropod reactions, and bacterial infections. CPL can be classified based on its clinical features, but all variants have similar histopathological patterns of either predominantly B-cell infiltrates, T-cell infiltrates, or mixed T/B-cell infiltrates. The prognosis of CPL is good, but the underlying disease process should be taken

The subcutaneous loop: a single suture technique for skin closure after superficial and subcutaneous surgery.

BACKGROUND: Removal of subcutaneous skin masses and certain deep skin injuries results in defects, which demand immediate closure. Although primary closure of these defects in 2 layers is an acceptable solution, this technique has a number of drawbacks, such as extended time consumption upon performance, demand for additional suture material and occasional tissue response or foreign body reaction to retained absorbable stitches leading to infection and its sequelae. OBJECTIVE: We describe an approach intended to furnish a satisfactory solution to the needs of tissue closure in such cases. The
