Echocardiography has proven a useful and sensitive means to determine noninvasively the cardiac dimensions. This report describes the echocardiographic detection of progressive dilatation of the aortic root over an 18-month period, which led to death in a 33-year-old man with Marfan's syndrome; however, at no time was the aortic dilatation evident on chest x-ray films. This patient illustrates the sensitivity of the echocardiogram in detecting and assessing the severity of disease of the aortic root not readily assessed by other noninvasive techniques and the value of serial measurements of ao

Sinus of Valsalva aneurysms (SVAs) are rare congenital entities arising from eccentric aortic root dilatation that can protrude and rupture into adjacent cardiac chambers. Treatment entails aneurysmal sac excision and aortic defect closure. We present a young patient with a ruptured SVA fistulizing into the right ventricle and acute decompensated heart failure. He also had moderate aortic root enlargement and a dysmorphic aortic valve with 3 highly asymmetrical leaflets. This pathologic condition was successfully repaired with a novel combination of valve-sparing root replacement, aortic valve

Choledochal cysts are an uncommon anomaly of the biliary system manifested by cystic dilatation of the extra or intrahepatic biliary tree or both. It is most frequently found in Orientals and in females. Endoscopic retrograde cholangiopancreatography is a valuable imaging technique in the diagnosis of choledochal cysts in adults. Additionally, in selected cases, a choledochocele may be effectively managed by endoscopic sphincterotomy. We present clinical and endoscopic findings of six adult patients with choledochal cysts. Clinical symptoms were characterized by abdominal pain, jaundice and ch

Six small-breed, middle-age dogs with a history of chronic intermittent vomiting had benign pyloric lesions causing gastric outlet obstruction. Marked similarities were found in clinical signs, pathologic changes, and treatment results. The condition was classified as a syndrome and was named chronic hypertrophic pyloric gastropathy. The appearance of the obstructive lesions at surgery were various forms of mucosal hypertrophy. Microscopically, the syndrome was characterized by mucosal foveolar and glandular hyperplasia, cystic glandular dilatation, superficial mucosal ulcerations, and various

A 7-year-old girl with a 3-year history of intermittent bouts of abdominal pain and jaundice underwent endoscopic retrograde cholangio-pancreatography, which revealed an anomalous junction of the pancreatico-biliary ductal system (bile duct dilatation being virtually non-existent). These abdominal symptoms being considered to be attributable to the anomaly, resection of choledochus followed by hepatico-duodenostomy was performed. Now, one and a half years after operation, the patient is entirely asymptomatic. The legitimacy of "common channel syndrome" as an independent pathologic entity was p

BACKGROUND/AIMS: To evaluate the clinical, pathologic, and imaging findings of solid pseudopapillary neoplasm (SPN) of the pancreas in order to differentiate benign from malignant pancreatic neoplasms.
METHODOLOGY: We retrospectively identified 23 patients with 24 SPNs confirmed by pathology at National Taiwan University Hospital between January, 2001 and March, 2012. All computed tomography and magnetic resonance imaging were reviewed by two radiologists in consensus. Each tumor was analyzed for location of tumor, tumor margin, proportion of solid component, capsule morphology, growth patter

OBJECTIVE: Patients with fusiform dilatation of the carotid artery (FDCA) following pediatric craniopharyngioma resection typically have a benign clinical course. We reviewed the neurosurgical literature for FDCA outcomes after resection of these tumors.
METHODS: Using PubMed, Web of Science, and Cochrane databases, we identified surgical series or case reports reporting incidences of FDCA following craniopharyngioma resection. Inclusion criteria included FDCA outcomes reported specifically after craniopharyngioma resection, with at least 6 months of follow-up data.
RESULTS: Our literature s

OBJECTIVE: To investigate the prognostic value of left atrial volume index (LAVI) in patients with moderate to severe aortic regurgitation (AR) and bicuspid aortic valve (BAV).
METHODS: 554 individuals (45 (IQR 33-57) years, 80% male) with BAV and moderate or severe AR were selected from an international, multicentre registry. The association between LAVI and the combined endpoint of all-cause mortality or aortic valve surgery was investigated with Cox proportional hazard regression analyses.
RESULTS: Dilated LAVI was observed in 181 (32.7%) patients. The mean indexed aortic annulus, sinus o

One of the roles of a pediatric cardiologist who suspects or diagnoses a genetically determined connective tissue disease (e.g., Marfan, Ehlers-Danlos, and Loeys-Dietz syndromes) is to assess whether the aortic root is dilated. The aortic root diameter is affected by the patient's age, sex, and body surface area. Therefore, the aortic root diameter needs to be determined and expressed as a z-score. Calculation of the z-score is time-consuming and problematic if used infrequently. This study aimed to introduce a simple screening method for identifying aortic root dilation in children. The study

Background Choledochal cysts (CCs), congenital cystic dilatation of the biliary tract, are more commonly identified in females and have been associated with a myriad of other developmental abnormalities. Case Report: We present a male infant who was diagnosed with type I CC prenatally. He subsequently underwent cyst and gallbladder resection with hepaticoduodenostomy reconstruction at the age of 6 months. Pathologic examination confirmed type I CC with co-existing septate gallbladder and ectopic pancreas (Heinrich type 1). Conclusions: Although the clinical significance is unclear, this second

Gastroesophageal intussusception is a rare but life-threatening condition that requires immediate diagnosis and urgent surgical intervention. We describe the clinical, radiographic, ultrasonographic, and gross pathologic examinations of a 50-day-old German Shepherd dog with gastroesophageal intussusception associated with esophageal dilatation. The dog was brought to the clinic 10 days after weaning with a history of regurgitation, persistent vomiting, hematemesis, and dyspnea. On admission, the dog was lethargic with signs of shock and died just before surgery. Gastroesophageal intussusceptio

Circulatory vessels are subject to diverse pathologic processes that depend on the properties of the vessels. Vascular problems such as dilatation, dissection, and rupture can occur as a result of many disorders. Conditions that affect vessels include inherited disorders such as Marfan syndrome and the Ehlers-Danlos syndromes and poorly understood congenital disorders such as arteriovenous malformations and capillary fragility. Depending on which condition patients have, they must be given proper counseling about activity and options for management. For some patients, strenuous exercise, pregn
