Aortic Dissection and Other Acute Aortic Syndromes: Diagnostic Imaging Findings from Acute to Chronic Longitudinal Progression.

Acute aortic dissection is the prototype of acute aortic syndromes (AASs), which include intramural hematoma, limited intimal tear, penetrating atherosclerotic ulcer, traumatic or iatrogenic aortic dissection, and leaking or ruptured aortic aneurysm. The manifestation is usually sudden and catastrophic with acutely severe tearing chest or back pain. However, clinical symptoms do not allow distinction between AAS types and other acute pathologic conditions. Diagnostic imaging is essential to rapidly confirm and accurately diagnose the type, magnitude, and complications of AASs. CT fast acquisit

Surgical techniques. Ascending aorta.

Aneurysm and dissection are the most common diseases affecting the ascending aorta. Graft replacement of the ascending aorta is a straightforward cardiovascular procedure with excellent early and late results. When aneurysm or dissection extends into the aortic sinuses or arch, management becomes more complex and may entail replacement of the aortic root, aortic valve, or a portion of the aortic arch using hypothermic circulatory arrest. The optimal root prosthesis depends on several patient- and procedure-related variables. Valve-sparing procedures confer many long-term advantages and should

Patency of saphenous vein coronary artery bypass grafts from the vascular prosthesis of the ascending aorta.

BACKGROUND: Patients who have Stanford type A aortic dissection with impaired coronary arteries or who have aneurysms from the ascending aorta to the aortic arch with coronary artery disease need coronary artery bypass grafting (CABG) with tube graft replacement of the ascending aorta simultaneously. When vein grafts are used for CABG in these patients, the proximal anastomoses of vein grafts are attached to the prosthetic tube graft of the ascending aorta. However, the validity of proximal anastomoses of vein grafts to the prosthetic tube graft of the ascending aorta has not been confirmed.

[Descending Necrotizing Mediastinitis after Graft Replacement of the Thoracic Aorta:Report of a Case].

We report a case of a 64-year-old man with descending necrotizing mediastinitis. He had undergone ascending aortic replacement for Stanford type A acute dissection five years before, and thoracic endovascular aortic repair from the distal arch to the descending aorta for dilation of residual dissection three years before. The patient was admitted to our hospital complaining persistent fever, rubescent neck and chest. Computed tomography( CT) revealed a deep-seated mass extending between the neck and the right upper mediastinum and another mass around the ascending aorta graft. Computed tomogra

Acute type A aortic dissection and left main coronary artery obstruction detected by transesophageal echocardiography.

A 63-year-old man was admitted with severe chest pain. The electrocardiogram demonstrated ST-segment depression in the anterior and lateral leads suggesting acute anterior myocardial ischemia. Contrast-enhanced thoracic computed tomography performed due to severe back pain showed acute dissection of the descending aorta. Coronary angiography showed normal coronary arteries. Transesophageal echocardiography revealed a Stanford type A aortic dissection involving the left main coronary ostium and causing left main coronary occlusion. The dissected flap caused partial obstruction of the coronary o

[Giant pseudoaneurysm of the ascending aorta following the aortic valve replacement;report of a case].

56-year-old male, who had undergone re-aortic valve replacement (AVR) 33 years ago, received preoperative study for pancreatic surgery. Computed tomography (CT) revealed a giant pseudoaneurysm (7 cm diameter) of the ascending aorta. The ascending aorta was not dilated. A midline skin incision was performed, followed by full sternotomy. A tight pericardial adhesion was carefully dissected. Cardiopulmonary bypass was established by femoral arterial and bicaval venous cannulation. The pseudoaneurysm was incised under the retrograde cardioplegic protection. A communication between ascending aorta

[Surgery for acute aortic dissection extending to the aortic root using gelatine-resorcine-formol biological glue--a case report].

A 73-year-old woman was admitted with chest pain of sudden onset and hypotension. Enhanced CT showed the dilated ascending aorta with an intimal flap. Emergency surgery was performed under a diagnosis of type A acute aortic dissection and cardiac tamponade. Replacement of the ascending aorta was planned, but during the operation we found that the dissection had extended to the aortic root and that the laceration of the adventitia was located just distal to the aortic annulus. Since the laceration was closer to the aortic annulus than the coronary ostium, it was impossible to reconstruct the ao

Two Cases of Endovascular Repair with the Stent Graft for Retrograde Type A Acute Aortic Dissection with Complications.

Thoracic endovascular aortic repair (TEVAR) is used to treat retrograde type A acute aortic dissection (RTAAD). In case 1, a 52-year-old man, who was conservatively managed, reported worsening back pain. Emergency TEVAR was performed 7 days after onset. After deploying two GORE Conformable TAG (CTAG) in the descending aorta, his symptoms disappeared. In case 2, a 52-year-old man with progressive worsening resistant hypertension, renal dysfunction, and respiratory failure despite maximal medical therapy underwent TEVAR 8 days after onset. A CTAG was deployed from the left subclavian artery unde

Successful emergency replacement of the ascending aorta and aortic and mitral valves in the Marfan syndrome.

A 37-year-old man with the Marfan syndrome and chronic mitral regurgitation suffered an acute dissecting aneurysm of the ascending aorta with aortic regurgitation. Emergency replacement of the mitral valve, aortic valve, and ascending aorta was carried out, and the patient made an uneventful recovery. He is doing well 18 months after surgery. This is the first reported case of survival from such a procedure in the face of acute dissection with Marfan's syndrome. Indications for elective surgery are discussed and the uncertainties about the prognosis outlined. Continued reporting of the results

Penetrating ulcer of the thoracic aorta: what is it? How do we recognize it? How do we manage it?

BACKGROUND: Although classic type A and B aortic dissections have been well described, less is known about the natural history of penetrating atherosclerotic ulcers of the thoracic aorta. This study differentiates penetrating ulcer from aortic dissection, determines the clinical features and natural history of these ulcers, and establishes appropriate correlates regarding optimal treatment. METHODS: A retrospective review of patient records and imaging studies was conducted with 198 patients with initial diagnoses of aortic dissection (86 type A, 112 type B) at our institution from 1985 to 19

Valve-sparing operation for aortic root aneurysm late after Mustard procedure.

The aortic valve reimplantation technique leads to excellent clinical outcome in patients with aortic valve incompetence and aneurysms of the ascending aorta. This technique is now applied for aneurysms of ascending aorta, aortic dissection type A, and even dilatation of pulmonary autograft after the Ross operation. We report a case of aortic root dilatation late after a Mustard operation for transposition of the great arteries that was successfully managed by valve-sparing aortic root reimplantation.

New potential morphologic features of the aorta associated with the occurrence of acute type B aortic dissection.

The present study aimed to identify aortic morphological features associated with the occurrence of acute type B aortic dissection (aTBAD) and establish a risk prediction model. Patients in the control group were age- and sex-matched in a 1:1 ratio with the aTBAD group. Morphological parameters of the ascending aorta and aortic arch were measured on the GE Healthcare Advantage Workstation. The length and diameter of the ascending aorta and aortic arch in the aTBAD group increased significantly (all P < 0.001). The tortuosity and angulation of ascending aorta were significantly larger in the aT

Single aortic sinus repair with double annular sutures in acute type A aortic dissection: a single center case series of seven patients.

BACKGROUND: We evaluated a modified surgical technique designed to preserve and restore the aortic root geometry in patients with acute type A aortic dissection (ATAAD) when the intimal tear is limited to a single coronary sinus. METHODS: Between January 2015 and December 2021, 160 patients with ATAAD underwent emergency open replacement of the ascending aorta. Most patients (137, 85%) underwent supracoronary ascending aorta replacement. Thirteen patients underwent aorta root replacement with a valved conduit and three underwent valve-sparing aortic root replacement. The remaining seven patie

Aortic dilation in adults with repaired tetralogy of Fallot: a single-centre study.

BACKGROUND: Tetralogy of Fallot is the most prevalent cyanotic CHD. With the advent of advanced surgical methods, the majority of tetralogy of Fallot patients reach adulthood. However, many need re-intervention for the residual anomalies including residual right ventricular outflow obstruction, pulmonary regurgitation, residual ventricular septal defects, and progressive aortic dilatation. Aortic dilation could lead to aortic regurgitation or dissection requiring surgical correction. In the current study, we aimed to determine the prevalence and outcomes of aortic root dilatation in adults wit

[A case of acute A type aortic dissection with lower extremity ischemia--percutaneous fenestration of the aortic septum followed by ascending aortic graft replacement by open distal anastomosis and retrograde cerebral perfusion].

A 56-year-old woman with severe back pain and a cold, pulseless right extremity was admitted to our hospital. Angiogram revealed a type A aortic dissection extending from ascending aorta to the aortic bifurcation with no definite re-entry point. The false lumen gave origin to the right renal artery and the right external iliac artery was occluded. Therefore, a catheter was manipulated into the true lumen through a percutaneous right femoral artery approach, and was advanced into the false lumen through the right posterolateral wall of the dissecting aortic septum. Fenestration was then perform

Recurrent Cerebral Embolism Caused by Sluggish Retrograde Flow from the Residual False Lumen of the Aortic Arch after Type A Aortic Dissection.

A residual false lumen after type A aortic dissection repair may rarely serve as an embolic route to the cerebral circulation. A woman in her 60s underwent ascending aortic replacement for Stanford type A aortic dissection, leaving a residual patent dissection involving the supra-aortic branches and the descending aorta. Despite receiving anticoagulant and antiplatelet therapies, the patient developed recurrent multifocal cerebral infarctions. Transesophageal echocardiography (TEE) demonstrated an entry tear in the descending aorta communicating from the true lumen to the false lumen. Flow bec

Successful repair of iatrogenic acute aortic dissection with cerebral malperfusion.

We describe the successful treatment of a patient with iatrogenic acute aortic dissection including cerebral malperfusion as a complication of coronary artery surgery. After beginning cardiopulmonary bypass, a retrograde ascending aortic dissection associated with cerebral malperfusion was recognized. Systemic circulation was immediately arrested at 31 degrees C. After aortotomy, hypothermic selective antegrade cerebral perfusion was established. Replacement of the ascending aorta with coronary artery bypass grafting was performed without neurologic complications.
