Blood supply to the first metatarsal head and vessels at risk with a chevron osteotomy.

BACKGROUND: Chevron osteotomy, a commonly performed procedure for the treatment of hallux valgus, results in osteonecrosis of the first metatarsal head in 0% to 20% of cases. The aim of this study was to map out the arrangement of the vascular supply to the first metatarsal head and its relationship to the limbs of the chevron osteotomy. METHODS: Ten cadaveric lower limbs were injected with an India ink-latex mixture, and the feet were dissected to assess the blood supply to the first metatarsal head. The dissection was carried out by tracing the branches of the dorsalis pedis and posterior t

Aortic dissection in a Jehovah's witness: Frozen Elephant Trunk and subsequent 4D flow MRI analysis, a case report.

Thoracic aortic surgery in Jehovah's Witness (JW) patients presents a challenge to the surgeon due to the patient's religious-based objection to the use of allogeneic blood products. Recently Frozen elephant trunk (FET) has been proposed as a more definitive treatment for patient presenting with thoracic aortic aneurysm and dissection (TAAAD) especially when the aortic arch is involved. We report a case of treating acute type A aortic dissection with the use of the Frozen Elephant Trunk technique in a 75-year old JW patient. The bloodless strategy included: minimize cardiopulmonary bypass and

A Timeline From Index Procedure To Complex Reinterventions For Thoracoabdominal Aortic Dissection.

The optimal management of acute type A aortic dissection (ATAAD) remains a controversial subject. While some surgeons opt for a hemiarch approach to minimize bypass and cross-clamping time, others prefer partial or total arch replacement to prevent the need for additional operations. The advent of hybrid approaches offers a variety of options to the aortic surgeon in treating ATAAD. Herein, we present a complex case of ATAAD requiring multistage reconstruction of the aortic arch and its branch vessels utilizing hybrid techniques. A 67-year-old man presented with chest pain and hypertension, le

A case of carotid artery dissection after surgical repair of aortic dissection successfully treated with endovascular therapy using intravascular ultrasound.

A 71-year-old woman developed the dissection from right brachiocephalic trunk (BCT) to internal carotid artery 6 days after the operation of ascending aortic dissection. Since no improvement of symptoms was observed despite conservative therapy, we performed endovascular therapy (EVT). Although a balloon-expandable stent was implanted in the BCT to seal the entry point of the dissection, the true lumen of the carotid artery was still compressed. Thus, we covered the dissected lesion completely with multiple stents, and her neurological findings improved. This case demonstrates that EVT using m

Endovascular treatment of distal cervical and intracranial dissections with the neuroform stent.

OBJECTIVE: Endovascular stent reconstruction is the primary intervention for cervical and intracranial dissections in symptomatic patients refractory to medical management. Advancement of traditional balloon-expanding stents into the distal internal carotid artery and vertebrobasilar artery can be technically challenging and potentially traumatic. METHODS: On retrospective review, nine patients at our institution with distal cervical and/or intracranial dissections were alternatively treated with the self-expanding, dedicated intracranial Neuroform stent. Three patients with dissecting aneury

[Treatment for Laterally Invasive Rectal Cancer with Robot-Assisted Surgery after Preoperative Treatment].

The patient is a 40-year-old male. He was referred to our department because, after a thorough examination, he was diagnosed with rectal cancer. Preoperative imaging showed a tumor in the rectum at the level of the seminal vesicles, and left lateral invasion was suspected. In addition, lymph node metastases in the left lateral area were suspected. We performed a robot-assisted low anterior resection plus bilateral lateral dissection plus covering ileostomy for this patient after neoadjuvant chemotherapy. The operation time was 495 minutes, and the blood loss was 50 g. The histopathological dia

An anatomical study of the full-length phrenic nerve and its blood supply: clinical implications for endoscopic dissection.

For surgeries aimed at the dissection of full-length phrenic nerve, a full appreciation of its trajectory, blood supply and correlation with adjacent anatomical structures is necessary, especially for endoscopic manipulations. A fresh cadaver study was conducted with the purpose of avoiding surgical complications and ensuring further efficacy and efficiency of endoscopic manipulations. Ten fresh adult cadavers were dissected. Special attention was paid to the topography of the origin, the trajectory of the phrenic nerve, and its anatomic communication with the surrounding vessels and organs. I

[Surgical treatment of ascending aortic dissecting aneurysm with Cabrol's operation in 3 patients].

Form Mar. 1993 to Dec. 1994, 3 patients with ascending aortic dissecting aneurysm underwent aneurysmal incision, artificial vessel interposition, aortic valvular replacement and coronary artery transposition as Cabrol's operation. All recovered health discussed. The procedures and techniques were modified on the Cabrol's. Anastomosis of coronary openings with 8 mm Gore-tex tube obtained good match, little tension, and avoided intimal tear around coronary ostia and blood leakage of the anastomosis. We have created a self blood infusion system to stop hemorrhage of vessel graft or anastomosis by

IVUS-Guided Rescue of Catheter-Induced Left Main Coronary Artery Dissection.

Catheter-induced left main coronary artery (LMCA) dissection is a rare but life-threatening complication of coronary angiography requiring rapid recognition and expert management. We report a 72-year-old hypertensive female who developed a National Heart, Lung, and Blood Institute (NHLBI) Type-F dissection of the LMCA extending into both the left anterior descending (LAD) and circumflex (LCx) arteries during guiding catheter engagement for left coronary angiography. The dissection resulted in abrupt LAD occlusion with TIMI 0 flow and acute ST-segment elevation. Intravascular ultrasound (IVUS)

Surgical treatment of synchronous gastric and esophageal carcinoma: case report and review of literature.

A 65-year-old man was admitted to our hospital because of advanced esophageal squamous cell carcinoma located on the left posterior wall of the lower thoracic esophagus and gastric adenocarcinoma in the antrum. Esophagectomy and distal gastrectomy with two-field lymph node dissection (mediastinum and abdomen) were performed via a left-sided abdominothoracic incision. The remnant gastric was pulled up successfully with the blood supply maintained by the left gastric vessel. He was discharged on the 13th postoperative day without any complications.

A Rare Anastomosis Between the Internal and the External Jugular Veins: A Case Report.

Usually, the external jugular vein (EJV) is located superficially over the sternocleidomastoid muscle and joins the subclavian vein or the venous angle. The internal jugular vein (IJV) lies deeply in close relation with the common carotid artery and vagus nerve, enveloped by the carotid sheath. Normally, there is no direct connection between those vessels. During a routine neck dissection, we found a rare anastomosis between IJV and EJV. The anastomosis was localized on the level of the cricoid cartilage. It was approximately 1 cm long, with the diameter of the lumen being 0.3 cm. There was no

Repair of thoracoabdominal aneurysm and dissection with branched and fenestrated technology.

Endovascular repair has transformed the management of complex aortic aneurysms and chronic aortic dissections, providing a minimally invasive alternative to open surgery. The introduction of fenestrated and branched endografts (FBEVAR) has significantly expanded the anatomical eligibility for endovascular repair, allowing for the treatment of thoracoabdominal and complex abdominal aneurysms with improved patient outcomes. This review provides a comprehensive analysis of recent advancements in FBEVAR, focusing on technical success, durability, reintervention rates, and the evolving strategies t

Hybrid noncovered open stents in repair of DeBakey type 1 acute aortic dissections.

OBJECTIVES: The indications for use, evidence base and experience with the novel noncovered open hybrid surgical stents for acute type A aortic dissection repair for concurrent stabilization of the 'downstream' aorta remains limited. We review the evidence base and the development of these stents. METHODS: Data were collected from Pubmed/Medline literature search to develop and review the evidence base for safety and efficacy of non-covered surgical stents. Existing guidelines for use and developments were reviewed. RESULTS: A single randomized control trial and 4 single-centre studies were

Current perspectives in acute type B aortic dissections: a literature review.

In this new millennial, endovascular strategies have revolutionized the treatment of acute type B aortic dissection (aTBAD). With reduced in-hospital mortality and good long-term outcomes TEVAR has become the gold standard for the treatment of complicated dissection and is gaining increasing support for its preventive applicability in some uncomplicated dissections. With this new paradigm came a shift of the treatment goal where just covering the entry tear is not enough and instead achieving long-term positive thoracoabdominal remodeling is needed. More extensive approaches with composite dev

Technical tips and clinical experience with the Nexus Endospan arch branch stent-graft.

Aortic arch pathologies are commonly treated by open surgery which is still the gold standard. Patients that are unfit for surgery can be effectively treated mini-invasively with novel endovascular endograft specifically designed for the arch. Endovascular technology has remarkably improved in recent years and is a valid alternative option for the treatment of aortic arch. Available endografts on the market include fenestrated and branched (single, double, or triple). Most endograft designed for the aortic arch are custom-made and production time could be limiting. The aim of this study is to

Prevention of distal stent graft-induced new entry after endovascular repair for type B aortic dissection: A retrospective cohort study.

OBJECTIVES: Distal stent graft-induced new entry (dSINE) can occur after thoracic endovascular aortic repair (TEVAR) for type B aortic dissection (TBAD). In this study we aimed to compare the effectiveness of restrictive bare stent (RBS), tapered stent graft (TSG), and non-TSG in TEVAR in preventing dSINE after a midterm follow-up. METHODS: This retrospective cohort study included patients with TBAD who underwent TEVAR (June 2010 to December 2018). The occurrence of dSINE during follow-up was examined. Predictors of dSINE were determined using Fine-Gray regression with death as the competing
