OBJECTIVE: The current article describes the case of a 23-year-old female with purging-type bulimia nervosa who was evaluated by her primary care physician for dizziness and lightheadedness.
METHODS: After laboratory studies were performed by her primary care physician, the patient was admitted to the hospital because of severe anemia. The patient had been taking nonsteroidal antiinflammatory drugs (NSAIDS) at prescribed doses for shin splints that were secondary to jogging and developed gastric erosion.
RESULTS: Endoscopic examination showed that she had diffuse gastritis with linear, strea…

BACKGROUND: Although dizziness is a common symptom in both primary care and referral practices, the relative frequency of various causes has not been well delineated.
METHODS: A MEDLINE search identified 12 articles containing original data on the etiology of dizziness in consecutive patients. Study sites included primary care offices (n = 2), emergency room (n = 4), and referral clinics (n = 6). Each study's strength of design was graded using nine quality criteria.
RESULTS: Dizziness was attributed to a peripheral vestibulopathy in 44% of patients, a central vestibulopathy in 11%, psychiat…

The peripheral and central vestibular systems exhibit an age-related structural deterioration which may be responsible for vestibular reflex deficits and dizziness in the elderly. However, it seems likely that the central nervous system is capable of compensating for a certain degree of decline in function, since not all elderly people are impaired to the extent that the clinical signs of vestibular dysfunction are apparent. Dizziness and other vestibular disorders may develop only when the degree of deterioration of the vestibular system exceeds the ability of the nervous system to compensate…

Dizziness is a common symptom that often remains unexplained despite extensive medical evaluation. Psychiatric disorders are usually considered only after all medical causes of dizziness have been ruled out. Sixty-five patients referred to an otolaryngology practice received a structured psychiatric interview, an otologic evaluation, and a dizziness questionnaire modified to assess psychiatric symptoms. They were divided into four diagnostic groups: psychiatric diagnosis only, otologic diagnosis only, both diagnoses, or neither diagnosis. Eleven questionnaire items were significantly associate…

UNLABELLED: OBJECTIVE. To determine the one-year outcome for patients with a chief complaint of dizziness that had persisted at least two weeks.
DESIGN: Prospective cohort study.
SETTING: Federal teaching hospital.
PATIENTS: 100 dizzy patients and 25 control subjects.
MEASUREMENTS: The primary outcome was dizziness status (improved or not improved); the secondary outcomes were morbidity and health care utilization.
RESULTS: The dizziness resolved for 18 patients, whereas the status improved for 37, stayed the same for 32, and worsened for 11, with two patients lost to follow-up. Thus, 55%…

We have developed an expert system to assist in the diagnostic work-up of otoneurological cases. Our otoneurological expert system ONE takes advantage of both patient history and clinical measurement data in order to supply all possible information about the patient's symptoms and other findings. This paper presents ONE after its initial stage of development, which included tests with numerous patients.

SETTING: Four outpatient clinics at a military teaching hospital.
PATIENTS: 100 dizzy patients and 25 control patients.
MEASUREMENTS AND MAIN RESULTS: Structured psychiatric interviews were conducted using the Diagnostic Interview Schedule, and functional status was assessed with the Sickness Impact Profile and the 20-item MOS (Medical Outcomes Study) Short-Form. Psychiatric disord…

The clinical significance of horizontal head-shaking nystagmus (HSN) was evaluated in 85 patients who complained of dizziness and vertigo. This was done by comparison of the horizontal head-shaking test with routine rotatory and caloric vestibular testing. We found that HSN evoked by horizontal head-shaking is a highly sensitive way to detect unilateral vestibular hypofunction. Except in patients with additional central vestibular imbalance or in patients with Meniere's disease, the direction of horizontal HSN is highly significant in indicating the side of the lesion, with the fast phase beat…

Pain and dizziness are common experiences throughout the lifespan. However, nearly a quarter of those with acute pain or dizziness experience persistence, which is associated with disability, social isolation, psychological distress, decreased independence, and poorer quality of life. Thus, persistent pain or dizziness impacts peoples' lives in similarly negative ways. Conceptual models of pain and dizziness also have many similarities. Many of these models are more expansive than explaining mere symptoms; rather they describe pain or dizziness as holistic experiences that are influenced by bi…

INTRODUCTION: Vertigo and dizziness are a common complaint affecting 15% to over 20% of adults yearly and can reach up to 36% at the age of 70 and 50% at 90 years of age. Stroke is one of the main concerns in the emergency department, although previous studies demonstrated a very low rate of cerebrovascular events in these patients.
AIM: To explore the prevalence, demographic and clinical characteristics of the patients who were hospitalized with isolated vertigo and dizziness and were diagnosed with cerebrovascular event.
METHODS: A retrospective cohort study that included all patients ≥40 …

BACKGROUND: Coronavirus Disease 2019 (COVID-19) is an infectious disease that affects not only the respiratory system but also the central and peripheral nervous systems. Dizziness, vertigo, and balance problems are among the most frequently reported neurological complaints following COVID-19 infection.
AIMS: This study aimed to determine the prevalence of dizziness/vertigo in patients who had COVID-19 and to examine the associated risk factors.
METHODS: This cross-sectional study evaluated 296 patients aged 20-66 years who had recovered from COVID-19. Demographic information and clinical as…

Vertigo and dizziness are among the most common chief complaints in the neurology and emergency departments. Benign, self-limiting peripheral causes such as benign positional paroxysmal vertigo or vestibular neuropathy, Ménière's disease are the majority, but dangerous underlying conditions such as cerebrovascular or cardiovascular diseases are still overlooked. In this paper, the anatomy of the vestibular network from peripheral to central and the classification based on "triggers and timing" rather than the analysis of patient's word (rotational versus dizzy) are presented. Based on these, I…

Vertigo is common in childhood and adolescence. Although children and adults share common causes of vertigo, epidemiology changes with aging. For instance, ischemic stroke is less frequent in childhood, whereas audiovestibular disorders, such as vestibular neuritis and the migraine equivalent, are the leading causes of vertigo. However, even if severe causes of vertigo are rare, clinicians must not miss them. In this review, we discuss the neurological causes of central vertigo in children. The diagnostic approaches reviewed here are focused on the search for signs of severity, such as an abru

Background and Objectives: Individuals report persistent symptoms after becoming infected by SARS-CoV-2 (COVID-19) that last for >4 weeks (long-COVID syndrome). Dizziness and hearing loss have been reported among long-COVID symptoms. However, little is known about the potential predictors of dizziness and hearing loss in individuals with long COVID. This study aimed to explore the presence and correlates of dizziness and hearing loss in a sample of people with long-COVID syndrome. Materials and Methods: Individuals aged 18 years and older who were infected with COVID-19 at least 8 weeks prior …

OBJECTIVE: To establish a functional connection between neck physical evaluations, dizziness discomfort and image findings among subjects diagnosed with proprioceptive cervical dizziness.
METHODS: After exclusion of peripheral vestibular disorders, 20 subjects with proprioceptive cervical dizziness hypothesis were selected. A Visual Analogue Scale (VAS) was used to quantify pain and vertigo. The active neck Range of Motion (ROM) and the Muscle Strength (MS) of the neck region were examined. The manipulation of vertebral bodies by the Maitland method and imaging scan were performed.
