How do I interpret transfusion transmissible infectious disease testing in a low-risk donor population?

BACKGROUND: Blood donors represent a unique population. Pre-donation screening questions, donor self-deferral, and temporary deferral and re-testing of repeat reactive donors result in lower prevalence of infectious disease compared to the general population. Prevalence directly affects the clinical performance of assay systems apart from the analytical sensitivity and specificity of the assay. STUDY DESIGN AND METHODS: It is important that blood operators understand what situations frequently elicit false reactive and false nonreactive cases, and how to mitigate the effect of these. RESULTS

"No-donor" liver transplantation.

Liver transplantation is hindered by organ shortage. The potential way to relieve this issue is to expand the donor pool via extending the donor criteria and make full use of all available grafts. The concept of "no-donor" liver transplantation allows grafts to be recovered from other liver recipients. This review summarizes the current clinical practice of "no-donor" liver transplantation, focusing on the experiences of Chinese transplant teams. Domino liver transplantation was introduced by Furtado in 1995 and implemented later in 2013 in China, and novel donor indications including some ess

Pre-operative assessment of living liver donors' liver anatomy and volumes.

Decades of experience supports LDLT as a favorable strategy to reduce waitlist mortality. The multiple regenerative pathways of hepatocytes and other hepatic cells justify the rationale behind it. Nonetheless, living liver donation is still underused and its broader implementation is challenging, mostly due to variability in practices leading to concerns related to donor safety. A non-systematic literature search was conducted for peer-reviewed original articles related to pre-operative evaluation of living liver donor candidates. Eligible studies were synthesized upon consensus for discussion

Domino transplantation for pediatric liver recipients: Obstacles, challenges, and successes.

BACKGROUND: Domino liver transplantation aims to address the need to increase the liver donor supply. In a domino liver transplant, the domino recipient receives the explanted liver from the recipient of a traditional liver transplant. The domino donor typically requires liver transplant to correct a metabolic disorder; the explanted liver thus has a single gene defect but otherwise normal structure and function. METHODS: In this review, we detail the history of domino liver transplantation, appropriate domino donor indications, the technical advances to the surgical approach, current outcome

Venous sample is superior to repeated skin-prick testing in blood donor haemoglobin second-line screening.

BACKGROUND: Blood donor haemoglobin concentration (Hb) is commonly measured from a skin-prick sample. However, the skin-prick sample is prone to preanalytical error and variation, which may lead to false deferrals due to low Hb. STUDY DESIGN AND METHODS: We assessed the efficacy of two second-line screening models for the evaluation of blood donors failing the initial skin-prick test. In the venous model (n = 305), Hb was measured from a venous sample at the donation site. In the skin-prick model (n = 331), two additional skin-prick samples were measured. All on-site Hb measurements were perf

Donor-derived Viral Infections in Liver Transplantation.

Donor-derived infections are defined as any infection present in the donor that is transmitted to 1 or more recipients. Donor-derived infections can be categorized into 2 groups: "expected" and "unexpected" infections. Expected transmissions occur when the donor is known to have an infection, such as positive serology for cytomegalovirus, Epstein Barr virus, or hepatitis B core antibody, at the time of donation. Unexpected transmissions occur when a donor has no known infection before donation, but 1 or more transplant recipients develop an infection derived from the common donor. Unexpected i

Transfusion-transmitted CMV infection - current knowledge and future perspectives.

Transmission of human cytomegalovirus (CMV) via transfusion (TT-CMV) may still occur and remains a challenge in the treatment of immunocompromised CMV-seronegative patients, e.g. after stem cell transplantation, and for low birthweight infants. Measures to reduce the risk of TT-CMV have been evaluated in clinical studies, including leucocyte depletion of cellular blood products and/or the selection of CMV-IgG-negative donations. Studies in large blood donor cohorts indicate that donations from newly CMV-IgG-positive donors should bear the highest risk for transmitting CMV infections because th

Kidney transplantation from deceased donors with high terminal serum creatinine.

The increasing number of possible recipients for kidney transplantation and relatively unchanged number of organ donors has led to consideration of alternative strategies and expansion of deceased donor criteria in order to expand donor pool. Previously, kidneys from expanded criteria donors (ECD) were strongly underestimated because of the conventional opinion suggesting these kidneys to have a higher rate of preservation injury, delayed graft function, rejection and nonfunction. Reducing the difference between graft outcome in patients transplanted from ECD and standard criteria donor (SCD)

Risk of transfusion-transmitted infections among returned high-risk deferred donors: A cohort study.

BACKGROUND AND OBJECTIVES: Blood donor selection is crucial for ensuring safe blood supply. A well-designed donor selection system helps in maintaining donor engagement by minimizing unnecessary exclusion while safeguarding transfusion safety. We aimed to assess the risk of transfusion-transmitted infections (TTIs) among high-risk deferred donors to evaluate deferral criteria efficacy. MATERIALS AND METHODS: This study included data from volunteers who were temporarily deferred because of bloodletting, high-risk sexual contact, tattooing, endoscopy and needle sticks, as well as from eligible

New strategies for evaluating the quality of kidney grafts from elderly donors.

The increased demand for kidney transplantation and organ shortage resulted in the increased use of kidneys from suboptimal donors. Therefore, identification of kidneys that can be accepted without significantly compromising the outcome of allograft or recipient has become critical. A robust assessment of organ quality is of particular importance especially in kidneys from elderly donors in whom morphological and functional changes associated with aging and diseases are obvious. A number of predictive tools have been developed to help with evaluating the suitability of a deceased-donor kidney

Second Transplantation for Relapsed Hematologic Malignancies After First Allogeneic Hematopoietic Stem Cell Transplantation From an HLA-Matched Related Donor.

BACKGROUND: Relapse after allogeneic hematopoietic stem cell transplantation (allo-HCT) from human leukocyte antigen (HLA)-matched related donors (MRDs) remains a major clinical challenge. While second allo-HCT may be curative, the optimal donor selection strategy for such patients remains unclear. OBJECTIVES: This study aimed to evaluate the impact of donor change in patients who relapse following allo-HCT from an MRD, and to clarify its effect on outcomes after second allo-HCT. STUDY DESIGN: Using data from the Japanese national transplant registry, we retrospectively analyzed 922 individu

The efficacy of novel centrifugation-free sperm selection (Io-Lix) on sperm parameters and ICSI reproductive outcomes.

RESEARCH QUESTION: What is the effect of a novel non-centrifugation method (Io-Lix) of sperm selection on sperm parameters and intracytoplasmic sperm injection (ICSI) reproductive outcomes? DESIGN: This pilot study elevated the capacity of the Io-Lix sperm selection protocol to improve sperm parameters (concentration, motility and sperm DNA fragmentation) of the neat ejaculate. Once established, the reproductive outcomes of Io-Lix selected spermatozoa were used for autologous and donor oocyte ICSI programmes and their efficacy compared with those using conventional swim-up. RESULTS: Io-Lix s

Lung transplantation: donor and recipient critical care aspects.

PURPOSE OF REVIEW: The purpose of this paper is to highlight new developments in donor and recipient lung transplant issues for the critical care physician. RECENT FINDINGS: A shortage of suitable lung donors has led to the use of extended donors and the development of novel techniques such as live-donor lung transplantation and the use of non-heart-beating donors. The increased experience and success with lung transplantation has also resulted in the extension of this therapy to patients previously considered unsuitable for transplantation. Postoperative outcomes can be affected by many of t

The relationship between donor-recipient lymphocytotoxicity and the transfusion response using HLA-matched platelet concentrates.

Matching donor-recipient pairs for HLA antigens provides a logical starting point for selecting platelet donors for thrombocytopenic recipients who have demonstrated refractoriness to pooled random donor platelet transfusions. However, not all recipients achieve a compatible transfusion response with platelets selected by HLA type, indicating that additional selection methods are required. Because of studies indicating that a positive lymphocyte cross-match assay between recipient serum (antibody) and donor lymphocytes predict acute renal allograft rejection, it was hoped that this assay might

Era of metabolic dysfunction-associated steatotic liver disease and impact on the liver donor pool.

Metabolic dysfunction-associated steatotic liver disease (MASLD) has emerged as a silent epidemic having substantial clinical implications, with liver transplantation being one of the areas most impacted. The increasing prevalence of metabolic fatty liver disease has reduced the quality of available donor organs. While noninvasive methods are increasingly applied to evaluate liver steatosis in deceased donors, liver biopsy remains the gold standard. Many aspects of liver biopsies are not yet fully standardized. Macrovesicular hepatic steatosis is associated with decreased allograft quality and
