Antibiotic-Associated Drug Reaction With Eosinophilia and Systemic Symptoms (DRESS) in a Young Male: A Case Report of Recurrence Without Organ Involvement.

Drug reaction with eosinophilia and systemic symptoms (DRESS) is a rare but severe idiosyncratic hypersensitivity reaction, most commonly associated with antiepileptics and allopurinol. Although antibiotics are increasingly recognized as triggers, they remain underreported. The syndrome is characterized by fever, rash, eosinophilia, and variable organ involvement, with mortality. We report the case of a 26-year-old previously healthy male who presented with acute fever, diffuse morbilliform rash, facial edema, and eosinophilia following ceftriaxone exposure. His symptoms improved with corticos

Two cases with extensively drug-resistant Salmonella Typhi infection returning from Pakistan.

We report two cases of patients with extensively drug-resistant Salmonella Typhi infection who had recently traveled to Pakistan. These cases suggest that obtaining a detailed travel history and considering the epidemiology of drug-resistant S. Typhi in the suspected area of acquisition is crucial for managing patients with typhoid fever.

One Health approach to Rift Valley fever vaccine development.

Since its discovery in the 1930s, Rift Valley fever virus (RVFV) spread across the African continent and invaded the Arabian Peninsula and several islands off the coast of Southeast Africa. The virus causes recurrent outbreaks in these regions, and its continued spread is of global concern. Next-generation veterinary vaccines of improved efficacy and safety are being developed that can soon be used for the widespread vaccination of livestock. However, due to regulatory and economic challenges, vaccine manufacturers have been reluctant to develop a human vaccine. Recent innovations in veterinar

Enteric susceptibility profile: Finding hope amidst despair.

OBJECTIVE: To assess the overall antimicrobial susceptibility profile of enteric fever patients in a tertiary care setting. METHODS: The retrospective study was conducted at Pakistan Institute of Medical Sciences, Islamabad, Pakistan, and comprised data from January 2019 to April 2023 of patients aged >12 years who were admitted with febrile illness and had a blood culture positive for salmonella typhi. Demographic data as well the antimicrobial susceptibility profile parameters, like multidrug resistance, extensive drug resistance and extended spectrum beta lactamase status, were recorded. D

DRESS Syndrome That Resembles Graft-Versus-Host Disease after Chemotherapy in a Pediatric Patient: A Case Report.

INTRODUCTION: Drug reaction with eosinophilia and systemic symptoms (DRESS) syndrome is a potentially life-threatening, drug-induced adverse reaction characterized by skin eruptions, lymphadenopathy, fever, and a broad range of other bodily manifestations. The spectrum of histopathologic and clinical presentations is wide; therefore, DRESS syndrome can mimic other diseases. CASE PRESENTATION: We present a case of a 4-year-old male patient who started chemotherapy with vincristine, cytarabine, and etoposide. The first clinical signs were fever, hemodynamic in-stability, and maculopapular eryth

Drug-induced hypersensitivity syndrome with superficial granulomatous dermatitis--a novel finding.

Drug-induced hypersensitivity syndrome (DIHS) is a rare and potentially fatal reaction characterized by fever, rash, and internal organ involvement that typically occurs between 3 and 6 weeks after commencing the drug. We describe such a case in a 26-year-old woman, who developed fever, exfoliative erythroderma, facial edema, cervical lymphadenopathy, hepatitis, and leukocytosis 6 weeks after commencing carbamazepine for lower back pain. Her serum angiotensin-converting enzyme level was also raised to 144 U/L (8-52 U/L). Skin biopsies demonstrated an unusual superficial dermal perivascular inf

Fever, tachycardia, and hypertension with acute catatonic schizophrenia.

Fever, tachycardia, and hypertension developed concurrently with the administration of thiothixene during an acute episode of agitation in a case of catatonic schizophrenia. No cause for the fever or hyperkinetic state was found, and the syndrome resolved spontaneously one week after antipsychotic drug therapy was halted. This case appears to be an example of "acute lethal catatonia" or the neuroleptic "malignant" syndrome, both of which may be due to disturbances of dopamine function within the CNS. Such cases are rare, but may be dramatic in their presentation; however, antipsychotic drugs m

Loxoprofen-induced interstitial pneumonia: a case report.

BACKGROUND: Loxoprofen is a nonsteroidal anti-inflammatory drug used in the treatment of many diseases. However, there are no case reports about loxoprofen-induced pneumonia. We have encountered a rare case of loxoprofen-induced pneumonia. CASE PRESENTATION: We report the case of a 71-year-old Japanese woman who was initially treated with loxoprofen for fever. She was admitted to our hospital because of worsening of her symptoms, including fever and dyspnea. Her symptoms improved after treatment with ceftriaxone. Seven days after admission, she again developed high fever. She was again treate

Intranasal allergen challenge during corticosteroid treatment.

The effect of a corticosteroid drug on type I reactions in the nose was studied in eighteen hay fever patients. They were treated with a daily dosage of 800 microng beclomethasone dipropionate intranasally for I week, in a double-blind, placebo-controlled cross-over trial. Allergen challenge on the last day of each treatment period showed that steroid treatment diminished sneezing and rhinorrhoea, but the effect was not statistically significant (P = 0-10). Immediate nasal blockage was unaffected. There is a marked discrepancy between the minor protection offered by beclomethasone dipropionate

Antibiotic selection in the penicillin-allergic patient.

Clinicians should be familiar with which antibiotics are safe to use for different types of penicillin-allergic reactions. Clinically, it is convenient to divide patients with penicillin allergy into three categories: those with unknown or possible reactions to penicillin, those with a drug fever or rash, and those with hives or anaphylactic reactions. Beta-lactam antibiotics may be used safely for patients with unknown/possible penicillin allergy and drug fever or rash. Penicillins or beta-lactams should not be used for patients with hives or anaphylactic reactions. For all patients, clinicia…

Co-trimoxazole compared to chloramphenicol in the treatment of enteric fever.

Co-trimoxazole (trimethoprim-sulphamethoxazole combination) was effective in the control of the acute infection in 60 patients with proven enteric fever. The mean interval between initiating treatment and defervescence was similar to that observed with a comparable group of 38 patients treated with chloramphenicol. Both co-trimoxazole and chloramphenicol were well tolerated and no serious adverse reactions, attributed to either drug, were noted. Two patients treated with chloramphenicol relapsed. No relapses were noted in the co-trimoxazole treated group. Although the evidence available at pre

Dosage Considerations for Canakinumab in Children With Periodic Fever Syndromes.

Periodic fever syndromes are a group of rare diseases with a highly variable onset, yet limited treatment options are available for children at an early age. Canakinumab has been approved to treat patients with cryopyrin-associated periodic syndrome, a periodic fever syndrome, and systemic juvenile systemic arthritis, with age cutoffs of 4 years and 2 years, respectively. In 2016, the US Food and Drug Administration (FDA) approved canakinumab, without an age restriction, for the treatment of three conditions of periodic fever syndromes, including familial Mediterranean fever, hyperimmunoglobul

Fever with Rashes.

Fever with rashes is one of the commonest clinical problems a general practitioner or pediatrician has to face in day-to-day clinical practice. It can be a mild viral illness or a life-threatening illness like meningococcemia or Dengue hemorrhagic fever or it can be one with a lifelong consequence like Kawasaki disease. It is very important to arrive at a clinical diagnosis as early as possible with the minimum investigational facilities. The common causes associated with fever and rashes are infections, viral followed by other infections. There can be so many non-infectious causes also for fe

Typhoid Fever.

Typhoid fever is one of the most common febrile illnesses encountered by the physicians in Bangladesh. Diagnosis is not difficult but has lately become a challenge due to changed clinical pattern of the disease, lack of adequate facilities for blood, stool, urine culture, excessive reliance on nonspecific Widal test and non availability of any reliable rapid diagnostic tests. Further, the indiscriminate and injudicious use of antibiotics for treating fever in undiagnosed febrile illnesses early has created problems to the physicians to reach to a diagnosis later on. This has also led to the em

Clinical presentation, causes and outcome of febrile episodes in a prospective cohort of HIV-infected patients.

BACKGROUND: Fever was frequently caused by opportunistic conditions in HIV-patients in the early years of the epidemic. Little is known about diagnostic spectrum and outcome of febrile episodes in patients with good access to antiretroviral therapy. METHODS: We prospectively studied clinical presentation, diagnosis and outcome of febrile episodes in a contemporary cohort of HIV-patients with good access to antiretroviral therapy. Fever was defined as temperature 38.3 °C or higher, measured by a health care provider. RESULTS: We found 220 febrile episodes in 146 patients. In 25.9% of episodes

In vitro small molecule screening to inform novel candidates for use in fluconazole combination therapy in vivo against Coccidioides.

UNLABELLED: Identifying improved treatments for severe and refractory coccidioidomycosis (Valley fever) is needed. This endemic fungal disease is common in North and South America, and cases have increased substantially over the last 30 years. The current standard of care, oral daily fluconazole, often fails to completely eradicate Coccidioides infection; however, the high cost of identifying new compounds effective in treating Valley fever is a barrier to improving treatment. Therefore, repurposing existing pharmaceutical agents in combination with fluconazole therapy is an attractive option.

[A Study on the Clinical Course and Antimicrobial Susceptibility of Mycoplasma pneumoniae in a Community Hospital].

Recently, reports of macrolide-resistant strains of Mycoplasma pneumoniae have been increasing. We examined the antimicrobial susceptibility and clinical significance in patients with M. pneumoniae. Seventy patients in whom M. pneumoniae was detected from 2008 to 2012 were included in the study, and compared with patients between 2003 and 2006. There were no macrolide-resistant strains detected in the 38 strains from 2003 to 2006, but from 2008 to 2012, out of the 70 strains 46 (65.7%) were positive for the macrolide-resistant 23SrRNA gene mutant (A2063G), which is consistent with recent trend

[Use of aspirin for fever by Russian immigrant children].

This study describes the therapeutic drug approach in fever in Russian immigrant children. 974 recent immigrants with their 208 children were under medical treatment at 2 primary care clinics during 1991-1995. They came from 10 republics of the Commonwealth of Independence States (CIS, formerly the Soviet Union). In the CIS more than 3/4 of children aged 1-18 years had been given aspirin for fever. Immigrants brought with them stocks of drugs and their children continued to take them as they had in the CIS. This form of treatment is described in the official CIS pharmacology book without any m
