Tumors of the duodenum are relatively rare, accounting for about 1-2% of all gastrointestinal neoplasms. Nevertheless diagnosis and therapy are important, as 50% of all duodenal neoplasms are malignant. In the period 1987-1992 we treated 14 patients with duodenal neoplasms (11 men, 3 women, age 30-81 years [median 60.4]). 5 patients had undergone previous treatment for malignant tumors (4 genito-urinary tract, 1 rectum). Gastrointestinal tuberculosis (TB) is a rare disease and only involves the duodenum in 2-2,5% of all cases. A 60-year-old female with no reported medical history, presented with constitutional syndrome with a 10 kg weight loss in three months, epigastric pain, bloating and vomiting. The case history of a 56-year-old woman with an acute abdomen is presented. Laboratory data, a plain abdominal X-ray and ultrasound examination provided no clues for a definite diagnosis. Surprisingly, a duodenal diverticulitis was found at operation. Some aspects and complications of duodenal diverticula are discussed.

Duodenal ulcer constitutes one component of the clinical-pathologic complex of peptic disease of the duodenum. Recent evidence suggests that this chronic process may be cured with antibiotic therapy. Radiologic evaluation provides a cost-effective approach to the selection of patients for such management. Emphasis upon meticulous technique is essential for maximum patient benefit.

The intestinal obstruction of duodenal atresia needs urgent differential diagnosis from malrotation with its risk of midgut volvulus and gangrene. There is an uncommon variant with a high incidence of familial recurrence, and a significant association with Down's syndrome, although the majority of the babies are otherwise normal. Oblique end-to-end anastomosis around the atresia is the preferred treatment and has a high success rate.

Fifteen cases of duodenal trauma that presented to Westmead Hospital between 1979 and July 1986 are reviewed. There were 12 blunt injuries, nine caused by motor vehicle accidents. Three patients sustained penetrating injuries, two due to stab wounds. Repair for blunt laceration or incised wound was by primary closure or serosal patch repair, most often with decompressive t-tube duodenostomy. Ulcerative colitis is a chronic inflammatory bowel disease usually confined to the colon and rectum. Although extraintestinal manifestations are common, ulcerative colitis-like lesions of the upper gastrointestinal tract, referred to as gastroduodenal ulcerative colitis, are rare and typically observed in patients with pancolitis or after total proctocolectomy. Strongyloides stercoralis can cause chronic infections that may escalate into life-threatening hyperinfection, especially in immunocompromised patients. Severe intestinal strongyloidiasis has been associated with bacteraemia and sepsis, classically due to translocation of enteric organisms across a disrupted intestinal mucosal barrier. We report a patient with lepromatous leprosy on steroids who developed Strongyloides hyperinfection presenting with ulcerative duodenitis and septic shock.
