The economic burden of gout: A systematic review.

OBJECTIVE: Gout is a painful and disabling joint disease that constitutes the most common inflammatory arthritis in the US. To clarify the economic impact of gout, we systematically reviewed the literature on the direct and indirect costs associated with this disease. METHODS: We conducted a literature search of MEDLINE, EMBASE, International Pharmaceutical Abstracts, NHS Economic Evaluation, and CINAHL databases to identify studies of gout and economics. We systematically reviewed published studies that met our inclusion criteria and extracted and summarized all relevant economic parameters.…

Ebola, jobs and economic activity in Liberia.

BACKGROUND: The 2014 Ebola virus disease (EVD) outbreak in the neighbouring West African countries of Guinea, Liberia and Sierra Leone represents the most significant setback to the region's development in over a decade. This study provides evidence on the extent to which economic activity declined and jobs disappeared in Liberia during the outbreak. METHODS: To estimate how the level of activity and number of jobs in a given set of firms changed during the outbreak, we use a unique panel data set of registered firms surveyed by the business-development non-profit organisation, Building Marke

[Epistaxis and anticoagulation - a medical and economic challenge?].

BACKGROUND: According to recent statistics more and more elderly patients are hospitalized due to epistaxis and need anticoagulation. Aim of the present study was to investigate if it was more complex to treat anticoagulated patients than non-anticoagulated patients, if these patients needed a longer hospital stay and to characterize the study population in terms of the various applied therapeutic methods. METHODS: Retrospective study, collecting clinical data of all in-patients treated between 1.1.2007 to 1.8.2011 at an ENT-Department of an University Clinic due to epistaxis. RESULTS: Of 65

Mortality and morbidity risks and economic behavior.

There are theoretical reasons to expect that high risk of mortality or morbidity during young adulthood decreases investment in human capital. However, investigation of this hypothesis is complicated by a variety of empirical challenges, including difficulties in inferring causation due to omitted variables and reverse causation. For example, to compare two groups with substantially different mortality rates, one typically has to use samples from different countries or periods, making it difficult to control for other relevant variables. Reverse causation is important because human capital inv

Economic considerations of the treatment of ankylosing spondylitis.

Ankylosing spondylitis (AS) is associated with both significant direct and indirect costs, which vary by country, and have generally increased dramatically since the introduction of anti-tumor necrosis factor therapy. The cost-effectiveness of biologic agents is controversial, although cost-effectiveness studies need to consider the potential impact of anti-tumor necrosis factor treatments on work ability. Alternatives to reduce costs associated with biologics have been examined, including on-demand dosing and lower dose alternatives. Other treatment measures, such as total hip arthroplasty an

Economic impact of cystic echinococcosis in peru.

BACKGROUND: Cystic echinococcosis (CE) constitutes an important public health problem in Peru. However, no studies have attempted to estimate the monetary and non-monetary impact of CE in Peruvian society. METHODS: We used official and published sources of epidemiological and economic information to estimate direct and indirect costs associated with livestock production losses and human disease in addition to surgical CE-associated disability adjusted life years (DALYs) lost. FINDINGS: The total estimated cost of human CE in Peru was U.S.$2,420,348 (95% CI:1,118,384-4,812,722) per year. Tota

Economic impact of comorbidities in spine surgery.

OBJECT: Comorbidities in patients undergoing spine surgery may reasonably be factors that increase health care costs. To verify this hypothesis, the authors conducted the following study. METHODS: Major comorbidities and age-adjusted Charlson Comorbidity Index scores were retrospectively analyzed for 816 patients who underwent spine surgery at the authors' institutions between 2005 and 2008, and treatment costs (hospital charges) were assessed with the help of statistical software. The sample was collected by a nonmedical staff (hired at the beginning of 2006). Patients underwent one of the t

The potential economic value of a hookworm vaccine.

Hookworm infection is a significant problem worldwide. As development of hookworm vaccine proceeds, it is essential for vaccine developers and manufacturers, policy makers, and other public health officials to understand the potential costs and benefits of such a vaccine. We developed a decision analytic model to evaluate the cost-effectiveness of introducing a hookworm vaccine into two populations in Brazil: school-age children and non-pregnant women of reproductive age. Results suggest that a vaccine would provide not only cost savings, but potential health benefits to both populations. In f

The use of economic evaluation in hand surgery.

This article explains the concepts of an economic evaluation relevant to evidence-based hand surgery. Cost-effectiveness analysis in hand surgery is increasingly important as health care resources become scarce in most jurisdictions. Hand surgeons need to incorporate the "manager of health care system" competency in their daily practice. Hand surgery literature may claim that "a novel hand technique" is more cost-effective than a prevailing one; it is important that hand surgeons and other users of clinical research appraise such innovation claims before adopting them in their practices. Economic burden of a gluten-free diet.

BACKGROUND: Coeliac disease is a common, autoimmune disorder, for which the only treatment is lifelong adherence to a gluten-free diet. This study evaluates the economic burden of adhering to a gluten-free diet. METHODS: A market basket of products identified by name brand, weight or package size for both regular wheat-based products and gluten-free counterparts was developed. The differences in price between purchase venues, both type of store (general grocery store, an upscale grocery store and a health food store and four internet-based grocery sites) and region was also analysed. The economic burden of gout on an employed population.

OBJECTIVE: To examine the economic burden of illness of gout in an employed population, quantifying the impact on employers annual health benefit costs for medical and prescription claims, sick leave, short- and long-term disability, and workers' compensation. METHODS: Adjudicated claims data from 300000 employees from 2001 through 2004 were utilized. T-tests were used to compare demographic data and medical costs and services by Agency for Healthcare Research and Quality (AHRQ) diagnostic categories. Two-part models were used to determine average annual health benefit costs overall and medic

Economic and humanistic burden of fibromyalgia in the USA.

Fibromyalgia is a complex, chronic condition involving persistent and widespread pain of unknown origin. In the USA the syndrome affects approximately 2% of the population, with prevalence increasing with age, and has a greater predominance in females. The results of this review have found that there is a considerable economic burden associated with this disease, and in particular, the effect of the syndrome on economic losses associated with work activities is at least as important as direct medical costs. The humanistic burden is more difficult to quantify due to the numerous symptoms associ

Economic and policy implications of improving longevity.

With all the rhetoric surrounding the impending "entitlement crisis" produced by the "graying of America," there has been surprisingly little serious analysis of the social and economic implications of increased longevity and the doubling of the number of elderly people that will occur in this country over the next 30 years. This article identifies five critical areas in which the effect of demographic change will be significant. First, patterns of work life and labor-force participation will almost inevitably change. Second, government expenditures now financed largely by payroll and federal

Economic analysis of the treatment of posterior epistaxis.

BACKGROUND: The aim of this study was to compare the economic impact of controlling posterior epistaxis by transnasal endoscopic sphenopalatine artery ligation (TESPAL) and endovascular embolization. METHODS: We conducted a retrospective chart review of patients undergoing treatment of posterior epistaxis by either TESPAL or embolization. International Classification of Diseases 9 code 784.7 (epistaxis) was the initial screen followed by common procedural terminology codes for TESPAL and angiography with embolization. Epidemiology and economic impact of atrial fibrillation.

The clinical and public health importance of detecting and treating atrial fibrillation (AF), a predominantly chaotic and fast cardiac dysrhythmia that disrupts cardiac output and increases the probability of intracardiac and systemic thrombosis, is increasingly being recognized. This article describes the epidemiology and economic health burden of AF and reviews the evidence to suggest that an epidemic of AF will persist for the foreseeable future.

Pharmacotherapy of essential thrombocythaemia: economic considerations.

The clinical course of essential thrombocythaemia (ET) is mainly outlined by a predisposition to both thromboembolic, and more rarely, haemorrhagic complications. The individual clinical course is, however, variable, ranging from an event-free course to life-threatening thromboembolic episodes. In order to treat ET patients economically, it is necessary, above all, to consider if cytoreductive therapy is really indicated. Risk stratification according to clinical criteria such as age, previous ET-related events and platelet count may help to define patients at risk. In low-risk ET patients, a

[Economic evaluation of H. pylori eradication].

A review of economic evaluation studies of Helicobacter pylori eradication was performed. As a result of the high quality and quantity of data emerging, the eradication therapy for patients with gastric or duodenal ulcer was considered to be less costly and more effective than conventional therapy in a Japanese clinical setting. Furthermore, population-based Helicobacter pylori screening was seemed to have the potential to produce important health benefits at a reasonable cost, especially in Japan. Controlled studies are needed to quantify the impact of Helicobacter pylori eradication on the r
