Lens-induced glaucoma in the elderly.

Lens-induced glaucoma comprises a number of different glaucomatous processes occurring in the elderly that share in common the role of the crystalline lens in the mechanism of increase in intraocular pressure. We will review the anatomic predisposing factors, their physiology, signs and symptoms, and therapeutic approach. We will consider two studies and discuss the visual results and risk factors associated with these pathologic conditions.

[Marfan's syndrome].

A family with Marfan's syndrome is reported showing as outstanding features, demonstration of the hereditary character of the entity through the genealogical tree. The classical clinical characteristics of the disease are: long face, arachnodactyly, decreased subcutaneous tissue, muscular hypoplasia, somatometric values above normal percentiles for Mexicans, weight and perimeters of arm and leg showing low mass and height, upper and lower segments above percentil 50 and ocular disorders like bilateral ectopia lentis, spherical and small lens, iridodonesis and myopia.

[The Marfan syndrome with a small body (author's transl)].

The authors report a case of a Marfan syndrome with a small body. This original observation comes from an union witch is equally original, the one of a man who has a Marfan syndrome and measures 1,91 m with a dwarfish woman who measures 1,36 m.

Homocystinuria in two South African Negro siblings.

This, to our knowledge, is the first report of homocystinuria in the South African Negro. Two siblings are presented who developed normally until the age of 5 years but then showed mental retardation. Both were tall and thin, had genu valgum and arachnodactyly. The elder sib also had kyphoscoliosis, pes cavus and bilateral ectopia lentis. The diagnosis was confirmed by the presence of homocystine in the urine, elevated plasma homocystine and methionine and diminished plasma cystine levels.

Renal autotransplantation for aortic dissection in Marfan's syndrome.

Marfan's syndrome is an autosomal dominant condition with a prevalence of 4 to 6:100,000, and classically comprises skeletal changes, ectopia lentis and cystic medial necrosis of the largest arteries. The latter leads to aneurysm formation, most often commencing at the aortic root, and is responsible for an average age at death of 32 years. We describe successful preservation of threatened renal function by renal autotransplantation, without concurrent aortic replacement, following dissection of the descending aorta in a patient with Marfan's syndrome.

Histology of the lens in the Weill-Marchesani syndrome.

The Weill-Marchesani syndrome is a rare systemic connective tissue disease characterised by small stature, brachydactyly, ectopia lentis, and spherophakia. Three siblings with typical manifestations of this syndrome were reported. The ophthalmological findings in all these cases were spherophakia, severe myopia, a shallow anterior chamber, and narrow angle glaucoma. Two cases underwent laser iridotomy and drug treatment. In the third case the lens was removed from the eye because of injury, and this lens was examined by light and electron microscopy.

Angiographic findings in homocystinuria.

Homocystinuria (HC) is an inborn error of amino acid metabolism characterized by ectopia lentis, mental retardation, and skeletal abnormalities. Vascular disorders may also occur in HC, although they are less common. Arteriographic studies of two sisters with HC are described. The younger woman's renal arteries showed wall irregularities and aneurysms, narrowing of the celiac and superior mesenteric arteries, and some aneurysmatic changes. In the older patient, irregular right carotid and splenic arteries were seen and a splenic aneurysm was present.

Ocular abnormalities in the myopathic hamster (UM-X7.1 strain).

Eyes from cardiomyopathic hamsters (UM-X7.1 strain) were examined histologically for evidence of ocular defects. Changes observed included microphthalmia, scleral ectasia, scleral rupture, keratoconus, retinal detachment, retinal dysplasia, retinal fragmentation, retinal thinning, fibrosis of iris and ciliary body, ectopia lentis, and cataract formation. Lesions characteristic of cardiomyopathic hamsters were observed in the myocardium and skeletal muscle. This strain may be a suitable animal model to study the pathogenesis of ocular changes seen in certain congenital connective tissue disorde…

FBN1-related connective tissue disorders: unraveling cardiovascular, skeletal, and ocular complications through TGF-β signaling dysregulation and genotypic correlations.

Fibrillin-1 is an extracellular matrix glycoprotein essential for microfibril integrity, mediating cell-matrix interactions, providing structural support to tissues, and serving as a scaffold for elastogenesis. Pathogenic variants in the fibrillin 1 gene (FBN1) give rise to a spectrum of autosomal dominant connective tissue disorders collectively termed type-1 fibrillinopathies, which include Marfan syndrome, geleophysic dysplasia 2, acromicric dysplasia, Weill-Marchesani syndrome 2, marfanoid-progeroid-lipodystrophy syndrome, stiff skin syndrome, MASS syndrome, and isolated ectopia lentis 1. …

Adamts10 inactivation in mice leads to persistence of ocular microfibrils subsequent to reduced fibrillin-2 cleavage.

Mutations in the secreted metalloproteinase ADAMTS10 cause recessive Weill-Marchesani syndrome (WMS), comprising ectopia lentis, short stature, brachydactyly, thick skin and cardiac valve anomalies. Dominant WMS caused by FBN1 mutations is clinically similar and affects fibrillin-1 microfibrils, which are a major component of the ocular zonule. ADAMTS10 was previously shown to enhance fibrillin-1 assembly in vitro. Here, Adamts10 null mice were analyzed to determine the impact of ADAMTS10 deficiency on fibrillin microfibrils in vivo. An intragenic lacZ reporter identified widespread Adamts10 e…

Differential Diagnosis between Marfan Syndrome and Loeys-Dietz Syndrome Type 4: A Novel Chromosomal Deletion Covering TGFB2.

Marfan syndrome (MFS) and Loeys-Dietz syndrome type 4 (LDS4) are two hereditary connective tissue disorders. MFS displays ectopia lentis as a distinguishing, characterising feature, and thoracic aortic ectasia, aneurysm, dissection, and systemic features as manifestations overlapping with LDS4. LDS4 is characterised by the presence of hypertelorism, cleft palate and/or bifid uvula, with possible ectasia or aneurysms in other arteries. The variable age of onset of clinical manifestations makes clinical diagnosis more difficult. In this study, we report the case of a patient with Marfan syndrome…
