Is gender dysphoria dysphoric? Elevated depression and anxiety in gender dysphoric and nondysphoric homosexual and bisexual men in an HIV sample. HNRC Group.

Few if any studies before the AIDS epidemic suggested that male homosexuals may on average have higher levels of depression than male hetero … le, FGI). For comparison purposes only, we also evaluated the subjects for the DSM-III diagnosis of Ego-Dystonic Homosexuality. Highly gender nonconforming men (high FGI scores) were more likely to have current symptoms of anxiety and depression by self-report, and to have had a lifetime history of depression by clinical interview. This association wa …

Formal Thought Disorder and Self-Disorder: An Empirical Study.

Background: Formal thought disorder was constitutively linked to the original concept of schizophrenia and has since been one of central features supporting its diagnosis. Bleuler considered formal thought disorder as a fundamental symptom of schizophrenia among other fundamental symptoms, including ego disorders. The contemporary concept of self-disorder represents a more developed, nuanced, and systematic approach to disturbances of self-experience than the Bleulerian concept of ego disorders. As fundamental symptoms, on Bleuler's account, are persistently present in every case, an associati…

The function of suffering as portrayed in the Scarlet Letter and reflected in clinical work.

Suffering is commonly seen as an unconscious effort to alleviate painful feelings of guilt. However, suffering also aims at averting loss of ego functions and hence loss of mental stability. This second function of suffering is discussed in the light of Freud's observations of characters wrecked by success and Weiss's ideas about mutual love as a threat to mental stability. Hawthorne's portrayal of Arthur Dimmesdale in The Scarlet Letter (1850), biographical material about the author, material from his diaries, and material from a psychotherapy case and an analysis illustrate the function of s…

On trauma, perversion, and "multiple personality".

The role of perverse sexuality as an organizing influence in "multiple personality" is explored in this paper. Following a brief review of psychoanalytic thinking on sexual trauma and perversion, the author discusses his own views on dissociation and "multiple personality." A clinical case is then presented in which transsexualism, homosexuality, and sadomasochistic heterosexual practices were manifested during altered ego states. Analysis of the transference revealed the centrality of sadomasochism in this patient. It is hypothesized that various perverse structures may be formed within these…

Castration complex. Evidence from men operated for hypospadias.

Thirty-three adult hypospadiacs and 36 matched controls were investigated by means of Rorschach's test, in a double-blind design. From psychoanalytic theory predictions were made about the effects of the influences of the genital malformation and its treatment on the psychological adjustment. The hypospadiacs differed significantly from the controls as predicted. They exhibited more neurotic constriction with less ego-strength and less utilization of resources. Their anxiety and hostility levels were judged higher and they showed less self-esteem and activity as well as less capacity for inter…

Transvertism: a disorder of the sense of self.

Transvestism is not simply a sexual disorder, but is best understood as primarily a disorder of the sense of self. A descriptive background of transvestism is provided through a review of developmental history, clinical course, presonality structure, and family history. The predominant transvestic fantasies and their modes of enactment are described. The disorder of the sence of self that gives rise to transvestic behavior is identified as a split in the ego into incompatible male and female gender identities. The split is attributed to an early identification with the mother as a defense agai…

A Different Conversation: Psychological Research and the Problem of Self in Autism.

Observations about peculiarities in the autistic population concerning type and frequency of references to subjective states, and lack of perspective taking, have been on the whole referred to as the paradox of the autistic self, i.e. a co-presence of ego-centeredness and weak self-referentiality (Lombardo & Baron Cohen 2010). Prevalent approaches in autism ascribe these peculiarities to high order disfunctions caused by neurological factors, such as defective self-encoding processes. Two narratives told by an adult man with Asperger during counselling are examined with Conversation Analysis; …

Future-oriented mental time travel in individuals with disordered gambling.

This study investigated the ability of individuals with disordered gambling to imagine future events. Problem gamblers (n=35) and control participants (n=35) were asked to imagine positive and negative future events for three temporal distances (one week, one year, 5-10years). Then, a variety of phenomenological aspects of their future thoughts (e.g., sensory and contextual details, autonoetic consciousness) were rated. Compared to control subjects, problem gamblers generated fewer positive and negative events across all temporal distances, an impairment that was correlated to verbal fluency s

The experimental analysis of the interruptive, interfering, and identity-distorting effects of chronic pain.

Pain is an unpleasant sensory and emotional experience urging the individual to take action to restore the integrity of the body. The transition from a common episode of acute pain to a state of intermittent or chronic pain has been a constant preoccupation of researchers and clinicians alike. In this review, we approach chronic pain from a modern learning perspective that incorporates cognitive, affective, behavioral and motivational aspects. We view pain as a biologically hard-wired signal of bodily harm that competes with other demands in the person's environment. The basic tenet is that pa

Psychodynamic psychotherapy for social phobia: a treatment manual based on supportive-expressive therapy.

Social phobia is a very frequent mental disorder characterized by an early onset, a chronic unremitting course, severe psychosocial impairments and high socioeconomic costs. To date, no manual for the psychodynamic treatment of social phobia exists. After a brief description of the disorder, a manual for a short-term psychodynamic treatment of social phobia is presented. The treatment is based on Luborsky s supportive-expressive (SE) therapy, which is complemented by treatment elements specific to social phobia. The treatment includes the characteristic elements of SE therapy, that is, setting

Re-reading "Little Hans": Freud's case study and the question of competing paradigms in psychoanalysis.

Psychoanalysts have long recognized the complex interaction between clinical data and formal psychoanalytic theories. While clinical data are often used to provide "evidence" for psychoanalytic paradigms, the theoretical model used by the analyst also structures what can and cannot be seen in the data. This delicate interaction between theory and clinical data can be seen in the history of interpretations of Freud's "Analysis of a Phobia in a Five-Year-Old Boy" ("Little Hans"). Freud's himself revised his reading of the case in 1926, after which a number of psychoanalysts--including Melanie Kl

King George III of England and Queen Maria I of Portugal: bipolar disorder and prince regents as common features of their reigns.

Humanity is sporadically subjected to leaders with deviant behavior, ego problems, or psychiatric disorders, potentially leading to social instability. Bipolar disorder is not common in all populations, but, coincidentally, studies suggest that it affected two sovereigns that were contemporaries, King George III of England, who died 201 years ago, and Queen Maria I of Portugal, who died 205 years ago. They lived during a time when Europe was in turmoil with the French Revolution and Napoleonic Wars, which also coincided with the rise of psychiatry. Both monarchs were forced to have prince rege…

Mindfulness as a means of reducing aggressive behavior: dispositional and situational evidence.

Recent research and theory suggest that mindfulness, or enhanced attention and awareness in the present moment [Brown and Ryan, 2003], may be linked to lower levels of ego-involvement and, as a result, may have implications for lowering hostility and aggressive behavior. Accordingly, we conducted two studies to examine the potential aggression-mitigating role of mindfulness. In Study 1, we found that dispositional mindfulness correlated negatively with self-reported aggressiveness and hostile attribution bias. In Study 2, participants made mindful before receiving social rejection feedback dis…

Gastric acid secretion in relation to personality, affect and coping ability in duodenal ulcer patients. A multivariate analysis. Hvidovre Ulcer Project Group.

The role of personality, mood state (affect) and coping ability (ego strength) on basal and stimulated gastric acid secretion were assessed in 56 duodenal ulcer patients using the Minnesota, Multiphasic Personality Inventory. The patients had high scores on most MMPI scales, but basal acid output was related significantly only to the hypochondriasis scale and the validity scale L. Peak acid output was related significantly only to the validity scale L. Both basal and peak acid output were related significantly, but positively to coping ability. No relations were found to mood state. The person…

Motion sickness--key to neurobiologic variation.

Individual variation in motion sickness susceptibility correlates with a wide variety of perceptual, cognitive, and personality variables suggesting that it may be a unique index of individual difference in fundamental mechanisms of the orientation process. Data from a large ongoing clinical study involving over 1,500 psychiatric inpatients indicate a clear-cut association between a number of the MMPI scales (including Social Isolation, Ego Strength and Schizophrenia) and motion sickness susceptibility. Marked difference across the motion sickness susceptibility spectrum in the condition on di…
