Below- or above-elbow immobilization in conservative treatment of distal radius fractures: a systematic review and meta-analysis.

INTRODUCTION: There is no consensus regarding the range of immobilization in the conservative treatment of distal radius fractures (DRFs). Therefore, this systematic review and meta-analysis aimed to compare the clinical outcome of patients with DRFs treated conservatively with below- or above-elbow immobilization. MATERIALS AND METHODS: Following the Preferred Reporting Items for Systematic Reviews and Meta-analysis (PRISMA) guidelines, two independent reviewers searched MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, Clinicaltrials.gov and World Health Organization Internat

Below-elbow cast for metaphyseal both-bone fractures of the distal forearm in children: a randomised multicentre study.

INTRODUCTION: Minimally displaced metaphyseal both-bone fractures of the distal forearm in children are often treated with an above-elbow cast (AEC). Treatment with a below-elbow cast (BEC) could give more comfort, but might lead to fracture displacement reducing pronation and supination. Because this has not been systematically investigated, we set up a randomised multicentre study. The purpose of this study was to find out whether BEC causes equal limitation of pronation and supination but with higher comfort level, compared with AEC. PATIENTS AND METHODS: In four hospitals, consecutive chi

Pediatric trans-olecranon fracture dislocation of the elbow associated with fracture of the radial head and with medial collateral ligament lesion: A case report and review of the literature.

INTRODUCTION: We report what is, to the best of our knowledge, the first case of pediatric trans-olecranon fracture dislocation of the elbow associated with a radial head fracture and with a medial collateral ligament disruption. CASE PRESENTATION: A 7-year-old girl presented to the emergency department after a fell on his right elbow while playful activity at home. The elbow X-ray showed acute trans-olecranon fracture dislocation of the elbow associated with a radial head fracture. A pre-operative 3D TC scans confirmed and clarified the injury pattern. However, stress radiographs performed i

Trends, complications, and comorbidity risk in total elbow arthroplasty versus open reduction and internal fixation for distal humerus fractures.

BACKGROUND: Treatment of distal humerus fractures (DHF) in older patients presents challenges due to high complication rates. Total elbow arthroplasty (TEA) has emerged as an alternative to open reduction and internal fixation (ORIF). This study investigates trends, outcomes, and risk factors associated with TEA and ORIF for the treatment of DHF. METHODS: Using the PearlDiver Mariner database, patients aged 55 years and older treated with either TEA or ORIF for DHF between 2010 and 2021 were identified. Trends in procedure volume, 90-day complications (postoperative bleeding, wound disruption

Transarticular Cartilage Retrograde Intramedullary Pinning Surgery for Humeral Transcondylar Fractures: An Effective Approach to Preventing Postoperative Elbow Joint Contracture.

As a treatment for humeral transcondylar fractures, conservative therapy is prone to false joints. Although various surgeries are available, plate fixation is highly invasive to soft tissues and prone to contracture of the elbow joint, and percutaneous pinning results in weak fixation and tends to damage the ulnar nerve. In this report of two cases, we describe a new transarticular cartilage retrograde intramedullary pinning surgical method that is less invasive to soft tissues, has an accurate reduction and strong fixation, and does not damage the ulnar nerve. This surgery was performed on tw

Correlation between modified trochleocapitellar index and post-traumatic elbow stiffness in type C2-3 distal humeral fractures among adults.

BACKGROUND: The purpose of this study was to propose the modified trochleocapitellar index (mTCI), assess its reliability, and evaluate its correlation with post-traumatic elbow stiffness in type C2-3 distal humeral fractures among adults. METHODS: From January 2013 to June 2017, a total of 141 patients with type C2-3 distal humeral fractures were included. The mTCI was calculated as the ratio between the modified trochlear and capitellar angles relative to the humeral axis (mTCI-HA), lateral humeral line (mTCI-LHL), and medial humeral line (mTCI-MHL) from anteroposterior radiographs taken im

Total Elbow Arthroplasty Versus Open Reduction and Internal Fixation for the Management of Distal Humerus Fractures in the Elderly.

Distal humerus fractures, which are challenging injuries seen in geriatric patients, are often repaired using open reduction and internal fixation or total elbow arthroplasty. This updated meta-analysis, which includes more recent open reduction and internal fixation techniques, examined outcomes following distal humerus fractures repaired using open reduction and internal fixation vs total elbow arthroplasty. Although distal humerus fractures treated with total elbow arthroplasty achieved statistically higher functional scores relative to those treated with open reduction and internal fixatio

Open reduction and internal fixation of Mason type III radial head fractures with and without an associated elbow dislocation.

PURPOSE: The purpose of this study was to evaluate the treatment of patients with Mason type III radial head fracture with and without elbow dislocation by open reduction with internal fixation, collateral ligament repair, and early mobilization. An additional purpose was to investigate whether there is any effect of elbow dislocation on the severity and functional outcome. METHODS: Twenty-five patients were treated with open reduction with internal fixation using low-profile miniplate and screws for comminuted fracture of the radial head. Eighteen fractures were Mason type III, and 7 fractur

Arthroscopically-assisted fixation of anteromedial coronoid facet fracture and lateral ulnar collateral ligament repair for acute posteromedial rotatory fracture dislocation of the elbow.

If left untreated, varus posteromedial rotatory injuries of the elbow result in poor functional outcomes. Surgical treatment allows restoration of elbow kinematics, minimizing the chances of chronic varus instability and early onset osteoarthritis. However, large exposures are associated with extensive soft tissue stripping, a high risk of infection, nerve injury, poor visualization of the articular surface and longer recovery. Consequently, there has been renewed interest in the use of elbow arthroscopy to circumvent these problems. Arthroscopic treatment offers the potential advantage of a s

Undisplaced scaphoid waist fractures: is 4 weeks' immobilisation in a below-elbow cast sufficient if a week 4 CT scan suggests fracture union?

This prospective study investigated a cohort of 59 scaphoid waist fractures which were treated nonoperatively in a below-elbow plaster cast for 4 weeks and then underwent a Week 4 CT scan to assess displacement and progress to union. Forty-three were classed as undisplaced and 37 of these 43 were also classed as 'united'. All the 37 undisplaced and 'united' fractures united with up to 8 weeks' cast immobilisation, including 26 which were taken out of plaster at 4 weeks and mobilised. We conclude that scaphoid waist fractures which appear to be undisplaced and united on a week 4 CT scan will un

Self-assessment of Elbow Mobility as a Reliable Method in the Postoperative Follow-up Examination of Radial Head Fractures.

Sufficient follow-up of fractures is often difficult, due to age-related, health-related or geographical reasons. Self-assessment of the patient's functional outcome could be an alternative to a personal medical examination. There is a lack of validated questionnaires to record these parameters for elbow injuries. The aim of the study was to validate the patient's self-assessment of the range of motion following surgical treatment of complex radial head fractures.50 patients (42% women) with a mean age of 49.7 ± 13.8 years (range 25-82 years) with a surgically treated radial head fracture (Mas

Elbow hemiarthroplasty for acute distal humeral fractures and their sequelae: medium- and long-term follow-up of 41 cases.

BACKGROUND: The earliest distal humeral hemiarthroplasty (or elbow hemiarthroplasty [EHA]) implants, which date back to the late 1940s, were performed in patients with severe elbow joint injuries as an alternative to arthrodesis. After some clinical reports and case studies with a short follow-up, published in the 1990s, a new "anatomically convertible" EHA model was introduced in 2005 and became a common surgical option to treat complex elbow fractures and their sequelae. We describe the mid- and long-term outcomes of EHA performed to treat acute intra-articular fractures or their sequelae. …

Above-versus below-elbow casting for conservative treatment of distal radius fractures: a randomized controlled trial and study protocol.

BACKGROUND: A variety of cast options are available for the non-surgical treatment of distal radius fractures (DRF) in adults. However, the literature is inconclusive regarding the need to immobilize the elbow joint after reduction in order to prevent rotation of the forearm in order to maintain the reduction of DRF. This study aimed to evaluate the best method of immobilization between above-elbow (AE) and below-elbow (BE) cast groups at the end of six-month follow-up. METHODS: This is a randomized clinical trial with parallel groups and a blinded evaluator. There are two non-surgical interv

Acute, avulsion fractures of the medial epicondyle while throwing in youth baseball players: a variant of Little League elbow.

HYPOTHESIS: The young throwing athlete is susceptible to medial elbow injury due to valgus overload. We hypothesized that this injury can occur during the throwing motion with an acute episode of medial elbow pain resulting in an inability to effectively participate in throwing activities. In addition, appropriate treatment of acute, medial epicondyle avulsion fractures in baseball players can result in an asymptomatic elbow with subsequent return to play within a year of injury. MATERIALS AND METHODS: A case series of all youth baseball players with medial epicondyle avulsion fractures that

Terrible Triad Injury of the Elbow Associated With an Olecranon Fracture: A Case Report With Three-Year Follow-Up.

Terrible triad injuries of the elbow consist of elbow dislocation associated with fractures of the radial head and coronoid process and are characterized by significant instability. Variants that include an associated olecranon fracture are rare, and data regarding long-term functional outcomes and patient-reported outcome measures remain limited. We report the case of a 74-year-old female patient who sustained a terrible triad injury of the elbow associated with an ipsilateral olecranon fracture following a low-energy fall. This injury pattern has occasionally been referred to as a "terrible

Pseudoaneurysm and arteriovenous fistula of the brachial artery presenting 25 years after elbow fracture: a case report.

INTRODUCTION: Post-traumatic pseudoaneurysms (PSAs) are uncommon vascular complications resulting from partial disruption of the arterial wall, leading to a contained hematoma that maintains communication with the arterial lumen. Although most PSAs develop shortly after trauma or vascular intervention, markedly delayed presentations are exceedingly rare and may pose significant diagnostic and therapeutic challenges. When associated with arteriovenous fistula (AVF) formation, these lesions carry an increased risk of progressive enlargement and sudden hemorrhage. CASE PRESENTATION: We report a
