Electroencephalography features of primary epileptogenic regions in surgically treated MRI-negative infantile spasms.

OBJECTIVE: To evaluate the surgical outcome for intractable, MRI-negative infantile spasms (IS), and to identify diagnostic targets in the focal epileptogenic area by methods other than MRI. METHODS: We retrospectively studied 9 patients who had had surgery for intractable IS, and whose lesions did not appear on MRI. We analyzed video/electroencephalography (EEG), single photon emission computed tomography (SPECT) and positron emission tomography (PET) findings and their surgical outcomes. In 7 patients who were seizure free after surgery, we analyzed the EEG parameters for characteristics ex…

The efficacy of routine hyperventilation for seizure activation during prolonged video-electroencephalography monitoring.

Hyperventilation (HV) is considered to be one of the activation procedures that provokes epileptic potentials and clinical seizures. However, the true clinical yield of HV is not well established. We retrospectively reviewed the records of all patients admitted to JFK Hospital, Edison, New Jersey, between October 2001 and December 2004 for long-term video-electroencephalography (EEG). A total of 475 patients (193 males; 282 females; age range 5-89 years) were included in the study. All patients underwent routine 3-minute HV as part of the evaluation of their clinical episodes. During the initi…

Amplitude-integrated electroencephalography in patients with acute encephalopathy with refractory, repetitive partial seizures.

We report amplitude-integrated EEG findings in two children with acute encephalopathy with refractory, repetitive partial seizures. Both patients had a febrile illness one week before the onset of seizure. They had reduction of consciousness and repetitive seizures refractory to first-line antiepileptic drugs. Seizure frequency rapidly increased and evolved into status epilepticus. Continuous seizure monitoring with amplitude-integrated EEG revealed frequent subclinical seizures which were missed by direct observation. In addition, the site of origin of seizures was multifocal, and seizure foc

Midazolam or diazepam administration during electroencephalography helps to diagnose subacute sclerosing panencephalitis (SSPE).

Although diagnostic contribution of intravenous diazepam administration during electroencephalography (EEG) recording in subacute sclerosing panencephalitis has been known, no another drug with less potential side effects has been studied in this procedure. In this study, diazepam is compared with midazolam in 25 subacute sclerosing panencephalitis-diagnosed children and 10 children with subacute sclerosing panencephalitis-suggesting symptoms, normal EEG findings and no certain diagnosis. Neither midazolam nor diazepam affected typical periodic slow-wave complexes. However, in the patients wit…

Myoclonus in Creutzfeldt-Jakob disease: polygraphic and video-electroencephalography assessment of 109 patients.

We used electroencephalography (EEG)-polygraphic recordings to classify myoclonus in 109 patients with Creutzfeldt-Jakob disease (CJD) on the basis of its electromyography (EMG) pattern, time course, distribution, and EEG correlates. We recorded myoclonic jerks in 55 patients (50.4%), and we classified them as periodic myoclonus in 28, rhythmic in 13, and irregular in 20 (6 patients showed two types of myoclonus). Myoclonus occurred as a prominently negative event (interrupting the EMG discharge) in 10. Periodic sharp-wave complexes (PSWCs) were present in all but one patient with myoclonic je…

Should amplitude-integrated electroencephalography be used to identify infants suitable for hypothermic neuroprotection?

OBJECTIVE: Amplitude-integrated electroencephalography (aEEG) has been used adjunctively to identify infants suitable for hypothermic neuroprotection following severe intrapartum asphyxia. To determine whether an early aEEG predicts short-term adverse outcome in infants with significant hypoxic-ischemic encephalopathy (HIE) evaluated for hypothermic neuroprotection. STUDY DESIGN: The aEEG recordings were obtained within 6 h of birth in infants >or=36 weeks' gestational age during evaluation for possible selective head or whole-body cooling. Recordings were subsequently re-evaluated for both b…

Predictive value of amplitude-integrated electroencephalography pattern and voltage in asphyxiated term infants.

The aim of this study was to correlate amplitude-integrated electroencephalography soon after birth with neurodevelopmental outcome in children who suffered from hypoxic-ischemic encephalopathy. Near term infants with hypoxic-ischemic encephalopathy and amplitude-integrated electroencephalography recording before 6 hours of age were included. Neurologic and cognitive outcome were assessed at 30 months of age and over. Outcome was correlated with either the pattern or voltage of the tracing. Thirty-nine infants were included. Eight died in the immediate neonatal period. At the age of 3 and 6 ho…

Tuberous sclerosis complex and epilepsy: prognostic significance of electroencephalography and magnetic resonance imaging.

Tuberous sclerosis complex is a disease that affects many organs, including the central nervous system. Nervous system involvement in the form of hamartomas often results in seizures. In this study we wanted to determine the outcome of epilepsy in tuberous sclerosis complex and determine whether interictal electroencephalograms (EEGs) and hamartoma burden as seen with magnetic resonance imaging (MRI) are predictive of degree of seizure control. The study population consisted of 30 patients. For each patient two sets of EEG and MRI data, separated by at least 12 months, and information on seizu

Prediction of neurological outcome after birth asphyxia from early continuous two-channel electroencephalography.

OBJECTIVE: To determine whether two-channel continuous electroencephalography (EEG) applied within 12 h of birth can predict the severity of neurological complications and neurodevelopmental outcome following birth asphyxia. METHODS: A continuous two-channel EEG was performed within 12 h of birth in 22 infants suspected of having suffered birth asphyxia and 11 healthy control infants (22 infants at a general and 11 at a specialist paediatric unit). Criteria to categorise normal and abnormal EEG records were defined and compared with the severity of hypoxic/ischaemic encephalopathy (HIE) and w…

Prognosis of hypoxic-ischaemic encephalopathy in full-term newborns--value of neonatal electroencephalography.

This study was performed to assess the predictive value of EEG in HIE following acute fetal distress. For 38 full-term neonates, EEG was recorded at least before 48 hours of life and between 2 and 7 days. Neurological outcome was evaluated at a minimum age of one year. Normal or slightly abnormal early EEGs (14 cases) were observed with normal outcome (13 cases) or minor sequelae (1 case). Extremely abnormal early EEGs were associated with death (5 cases), severe sequelae (4 cases) or normal outcome (1 case); among the "intermediate" group (14 cases), improvement of background activity before

A Time-Frequency Decoupled Contrastive Learning Framework for Electroencephalography-Based Parkinson's Disease Diagnosis.

Parkinson's disease (PD), a prototypical neurodegenerative disorder, poses significant challenges for early diagnosis. Motivated by recent advances in representation learning, a novel EEG-based diagnostic framework for PD, termed the time-frequency decoupling contrastive learning network (TDCNet), is introduced. The framework adopts collaborative contrastive learning between an online network and a momentum network to optimize latent representations in a self-supervised manner. It incorporates a temporal-evolution contrastive loss together with a frequency-aware contrastive loss, enabling adap

Interictal epileptiform discharges on scalp electroencephalography: Reproducibility of visual interpretation-A systematic review.

OBJECTIVE: Interictal epileptiform discharges (IEDs) are a key diagnostic finding in first seizure, early epilepsy, and acute brain dysfunction. Variability in visual electroencephalographic (EEG) analysis contributes to diagnostic uncertainty. We performed a systematic review of the reproducibility of IED detection in clinical EEG. METHODS: We updated a systematic review. We searched four databases (Pubmed, Embase, PsycInfo, Cochrane) for studies reporting interrater or intrarater reproducibility of clinical scalp EEG interpretation, covering records until April 2025. Four raters independent

Electroencephalography distinguished anti-N-methyl-D-aspartate receptor encephalitis and Creutzfeldt-Jakob disease.

Creutzfeldt-Jakob disease (CJD) and anti-NMDA receptor (anti-NMDAR) encephalitis are two diseases with different progression and prognosis, so it is crucial to distinguish them early. Here we retrospectively studied the medical records of patients with CJD and anti-NMDAR encephalitis diagnosed between November 1, 2011 and March 1, 2022. More patients had seizures in anti-NMDA receptor encephalitis group than in CJD group. Anti-NMDAR encephalitis patients had much higher cerebrospinal fluid (CSF) leukocyte counts while patients with CJD had higher CSF albumin count. Extensive cortical signal ab

Emotional food-cue-reactivity in anorexia nervosa and bulimia nervosa: An electroencephalography study.

OBJECTIVE: Food-cue-reactivity entails neural and experiential responses to the sight and smell of attractive foods. Negative emotions can modulate such cue-reactivity and this might be central to the balance between restrictive versus bulimic symptomatology in Anorexia Nervosa (AN) and Bulimia Nervosa (BN). METHOD: Pleasantness ratings and electrocortical responses to food images were measured in patients with AN (n = 35), BN (n = 32) and matched healthy controls (HC, n = 35) in a neutral state and after idiosyncratic negative emotion induction while electroencephalography (EEG) was recorded…
