Prediction of Cisplatin-Induced Acute Kidney Injury Using an Interpretable Machine Learning Model and Electronic Medical Record Information.

Predicting cisplatin-induced acute kidney injury (Cis-AKI) before its onset is important. We aimed to develop a predictive model for Cis-AKI using patient clinical information based on an interpretable machine learning algorithm. This single-center retrospective study included hospitalized patients aged ≥ 18 years who received the first course of cisplatin chemotherapy from January 1, 2011, to December 31, 2020, at Nagoya City University Hospital. Cis-AKI-positive patients were defined using the serum creatinine criteria of the Kidney Disease Improving Global Outcomes guideline within 14 days

Machine Learning Models to Predict Risk of Maternal Morbidity and Mortality From Electronic Medical Record Data: Scoping Review.

BACKGROUND: A majority (>80%) of maternal deaths in the United States are preventable. Using machine learning (ML) models that are generated from electronic medical records (EMRs) may be a promising approach to predict the risk of adverse maternal outcomes and enable proactive intervention to prevent maternal mortality. Current evidence syntheses of such ML approaches either focus only on specific maternal outcomes, aspects other than risk prediction, or do not consider the full pipeline of studies from the development to implementation in clinical practice. OBJECTIVE: The goal of this scopin

Cracking the code: an integrated electronic medical record approach to early diagnosis of genetic kidney disease in children with microscopic haematuria.

BACKGROUND: Microscopic haematuria (MH) is a common incidental finding in childhood that typically resolves spontaneously but, if persistent, can be a sign of underlying genetic kidney disease. Best practice indicates that all MH samples should be followed up to ensure resolution. Repeat sampling often does not occur, and an opportunity to diagnose and manage genetic kidney disease is missed, with substantial resultant preventable morbidity. The quality gap between recommended follow-up and actual practice represented an opportunity to improve care. METHODS: A retrospective audit of the elect

Analysis of factors affecting difficulty in handling oral medicine using electronic medication notebook-based personal health records.

Tablets and capsules are widely used forms of oral medication, but some patients experience difficulty handling them, which can reduce medication adherence and affect health outcomes. This study aimed to identify factors contributing to perceived handling difficulty, using data from harmo®, a nationwide electronic medication notebook system. A questionnaire was distributed to adult users who had been prescribed oral medications, and the responses were linked with personal health records to analyze medication characteristics and patient backgrounds. Among the 1,230 respondents, 24% reported dif

Clinical and genetic contributions to medical comorbidity in bipolar disorder: a study using electronic health records-linked biobank data.

Bipolar disorder is a chronic and complex polygenic disease with high rates of comorbidity. However, the independent contribution of either diagnosis or genetic risk of bipolar disorder to the medical comorbidity profile of individuals with the disease remains unresolved. Here, we conducted a multi-step phenome-wide association study (PheWAS) of bipolar disorder using phenomes derived from the electronic health records of participants enrolled in the Mayo Clinic Biobank and the Mayo Clinic Bipolar Disorder Biobank. First, we explored the conditions associated with a diagnosis of bipolar disord

Predicting Patients Requiring Treatment for Depression in the Postpartum Period Using Common Electronic Medical Record Data Available Antepartum.

INTRODUCTION: Depression requiring treatment in the postpartum period significantly impacts maternal and neonatal health. Although preventive management of depression in pregnancy has been shown to decrease the negative impacts, current methods for identifying at-risk patients are insufficient. Given the complexity of the diagnosis and interplay of clinical/demographic factors, we tested whether machine learning techniques can accurately identify at-risk patients in the postpartum period. METHODS: This is a retrospective cohort study of the NIH Nulliparous Pregnancy Outcomes Study: Monitoring

Substance use disorders and social determinants of health from electronic medical records obtained during Kentucky's "triple wave".

Social determinants of health (SDOH) play a critical role in the risk of harmful drug use. Examining SDOH as a means of differentiating populations with multiple co-occurring substance use disorders (SUDs) is particularly salient in the era of prevalent opioid and stimulant use known as the "Third Wave". This study uses electronic medical records (EMRs) from a safety net hospital system from 14,032 patients in Kentucky from 2017 to 2019 in order to 1) define three types of SUD cohorts with shared/unique risk factors, 2) identify patients with unstable housing using novel methods for EMRs and 3
