About differences in the availability of child and adolescent psychiatric hospital beds across Europe and possible implications for mental health care. Are more or less hospital beds the solution to the youth mental health crisis? : Based on a debate at the 21st International Congress of the European Society for Child and Adolescent Psychiatry, Strasbourg, June 30, 2025.

A number of indicators point to a decline in youth mental health globally over the past 15 years. Access to medical treatment and assistance is becoming increasingly difficult as the social divide is widening due to an imbalance between supply and demand. Hospitalization remains a critical yet costly component of child and adolescent mental health care, reserved for the most severe cases. While necessary in some situations, inpatient treatment carries substantial drawbacks, including family separation and social disruption. This paper discusses if increasing hospital beds across Europe can be

Implementation and Evaluation of an Emergency Department Medications for Opioid Use Disorder Program in a Regional Health System Without an Addiction Medicine Consult Service.

BACKGROUND: Opioid overdose deaths in the United States have recently declined overall but remain high and are increasing among minority populations. Medications for opioid use disorder (MOUD) decrease mortality for patients with opioid use disorder (OUD) but are under prescribed nationwide. OBJECTIVE: We hypothesized that our health system's four community emergency departments (EDs), without an addiction medicine consult service, could safely implement an ED MOUD Quality Improvement Initiative to increase prescriptions for buprenorphine/naloxone, with a low rate of adverse events and withou

The EMS response to the Oklahoma City bombing.

This is a descriptive study of the Emergency Medical Services response to a bombing of a United States Federal Building in Oklahoma City, Oklahoma on 19 April 1995. The explosion emanated from a rented truck parked in the front of the building. The force of the explosion destroyed three of the four support columns in the front of the building and resulted in a pancaking effect of the upper floors onto the lower floors. There were three distinct phases of the medical response: 1) Immediately available local EMS ambulances and staff; 2) Additional ambulances staffed by recalled, off-duty personn

Code Social: Integrating SDoH into Emergency Resident Education.

AUDIENCE: This small group curriculum is geared toward medical students and residents of all levels of training in emergency medicine programs. INTRODUCTION: Emergency departments serve as critical first points of contact for patients with acute health needs, often exacerbated by social determinants of health (SDoH). Incorporating SDoH awareness into emergency medicine practice is essential for addressing immediate health concerns, preventing future crises, and reducing the burden on healthcare systems. By identifying and addressing social factors, emergency providers can break cycles of recu

Self-management of health dilemmas among Chinese distant-water fishing vessel crew: a descriptive qualitative study based on Orem's self-care theory.

BACKGROUND: Distant-water fishing is one of the world's most hazardous occupations, characterized by extreme isolation from medical facilities, prolonged voyages, and high-intensity labor. Despite documented risks, qualitative evidence on how Chinese crew members manage their health during extended voyages remains scarce. OBJECTIVE: This study explored health self-management dilemmas among Chinese distant-water fishing vessel crew members, guided by Orem's Self-Care Theory. METHODS: A descriptive qualitative design was employed. Purposive and maximum variation sampling recruited 19 crew memb

Breaking barriers and fighting inequality in Parkinson's disease care: Personal reflections of movement disorders clinicians.

Parkinson's disease (PD) care remains profoundly unequal across and within countries, despite major advances in understanding and treatment. Drawing on PD specialists' personal stories of real-world experiences from diverse regions across Asia, Africa, and South America, this paper highlights the key barriers to equitable PD care and identifies pragmatic, scalable solutions. Across settings, several major barriers consistently emerge: a critical shortage and uneven distribution of trained specialists; geographic disparities limiting access to care; substantial financial barriers, particularly

Racial, Ethnic, and Socioeconomic Disparities in Prehospital Encounters for Children with Asthma.

OBJECTIVE: Asthma represents one of the most common medical conditions among children encountered by emergency medical services (EMS). While care disparities for children with asthma have been observed in other healthcare settings, limited data exist characterizing disparities in prehospital care. We sought to characterize differences in prehospital treatment and transport of children with suspected asthma exacerbations by race and ethnicity, within the context of community socioeconomic status. METHODS: We conducted a multi-agency retrospective study of EMS encounters in 2019 for children (2

The emerging trend of self-circumcision and the need to define cause: Case report of a 21 year-old male.

INTRODUCTION: Male circumcision is traditionally carried out by ascribed practitioners, so self-circumcision is very rare. It is intriguing why the victims should not seek available modern health care. This article highlights another case of self-circumcision, the related causes, complications and their management. The gradual increase in numbers is contributing to the surgical disease burden. CASE PRESENTATION: A 21year old male circumcised himself using a razor-blade 13days prior to presentation at the hospital. He attributes this to reluctance to have him circumcised. He sustained extensiv

A randomised controlled trial to prevent hospital readmissions and loss of functional ability in high risk older adults: a study protocol.

BACKGROUND: Older people have higher rates of hospital admission than the general population and higher rates of readmission due to complications and falls. During hospitalisation, older people experience significant functional decline which impairs their future independence and quality of life. Acute hospital services comprise the largest section of health expenditure in Australia and prevention or delay of disease is known to produce more effective use of services. Current models of discharge planning and follow-up care, however, do not address the need to prevent deconditioning or functiona

Promoting smoke-free environments and tobacco cessation in residential treatment facilities for mental health and substance addictions, Oregon, 2010.

INTRODUCTION: Although traditional patient-centered medical homes (PCMHs) are effective for patients with complex needs, it is unclear whether homeless-tailored PCMHs work better for homeless veterans. We examined the impact of enrollment in a Veterans Health Administration (VHA) homeless-tailored PCMH on health services use, cost, and satisfaction compared with enrollment in a traditional, nontailored PCMH. METHODS: We conducted a prospective, multicenter, quasi-experimental, single-blinded study at 2 VHA medical centers to assess health services use, cost, and satisfaction during 12 months

Investigating the impact of community first responders on out-of-hospital cardiac arrest outcomes in the North East of England: a retrospective cohort study.

INTRODUCTION: Community first responders (CFRs) are integrated within UK ambulance services to deliver early life-saving interventions prior to the arrival of emergency medical services (EMS). Although CFR attendance is associated with earlier cardiopulmonary resuscitation (CPR) and defibrillation, evidence regarding their independent impact on survival following out-of-hospital cardiac arrest (OHCA) remains mixed, and regional data from North East England are limited. The aim of this study was to evaluate the association between CFR attendance and survival outcomes following OHCA attended by

COVID-19 Disruptions to Social Care Delivery: A Qualitative Study in Two Large, Safety-Net Primary Care Clinics.

BACKGROUND: Social care integration refers to the incorporation of activities into health systems that assist patients with health-related social needs (HRSNs) that negatively impact the health outcomes of their patients, such as food insecurity or homelessness. Social care integration initiatives are becoming more common. The COVID-19 pandemic strained health systems while simultaneously increasing levels of unmet social needs. OBJECTIVE: To describe the effects of the COVID-19 pandemic on established social care delivery in a primary care setting. DESIGN: We used qualitative semi-structure

Advancements in Family-Based Treatment of Adolescent Anorexia Nervosa: A Review of Access Barriers and Telehealth Solutions.

Anorexia Nervosa (AN) is a psychiatric illness with serious medical and physiological implications. Anorexia Nervosa is characterised by significant disruptions in weight, growth and physical health resulting from disordered behaviours such as food restriction, purging and inappropriate exercise. The illness is associated with substantial physical, psychological, social and economic burdens affecting all areas of functioning. Typically emerging in adolescence, AN can have a chronic course and high risk of mortality, with evidence suggesting that approximately 10% of individuals diagnosed with

Temporal Trends in Incidence and Survival From Sudden Cardiac Arrest Manifesting With Shockable and Nonshockable Rhythms: A 16-Year Prospective Study in a Large US Community.

STUDY OBJECTIVE: The proportion of nonshockable sudden cardiac arrests (pulseless electrical activity and asystole) continues to rise. Survival is lower than shockable (ventricular fibrillation [VF]) sudden cardiac arrests, but there is little community-based information on temporal trends in the incidence and survival from sudden cardiac arrests based on presenting rhythms. We investigated community-based temporal trends in sudden cardiac arrest incidence and survival by presenting rhythm. METHODS: We prospectively evaluated the incidence of each presenting sudden cardiac arrest rhythm and s

Change in suicidal ideation, depression, and anxiety following collaborative care in the community.

BACKGROUND: The Collaborative Care Model (CoCM) increases access to mental health treatment and improves outcomes among patients with mild to moderate psychopathology; however, it is unclear how effective CoCM is for patients with elevated suicide risk. METHODS: We examined data from the Penn Integrated Care program, a CoCM program including an intake and referral management center plus traditional CoCM services implemented in primary care clinics within a large, diverse academic medical system. In this community setting, we examined: (1) characteristics of patients with and without suicidal

Exploring national and international experiences with community first responder models: protocol for a scoping review.

INTRODUCTION: Timely arrival of emergency medical services (EMS) is pivotal for effective prehospital care, and efforts by EMS leaders and policymakers to reduce response times, especially in cardiac arrests responsible for 70%-80% of coronary heart disease-related deaths, underscore the global urgency. With approximately 55 out-of-hospital cardiac arrests per 100 000 people annually, survival rates hinge on timely cardiopulmonary resuscitation, emphasising its initiation within a 4-6 min window. Trauma, causing 6 million deaths and nearly 40 million injuries a year, further underscores the ne

Needs Assessment for Simulation Training for Prehospital Providers in Botswana.

BACKGROUND: In June 2012, the Botswana Ministry of Health and Wellness (MOHW; Gaborone, Botswana) initiated a national Emergency Medical Services (EMS) system in response to significant morbidity and mortality associated with prehospital emergencies. The MOHW requested external expertise to train its developing workforce. Simulation-based training was planned to equip these health care providers with clinical knowledge, procedural skills, and communication techniques. OBJECTIVE: The objective of this study was to assess the educational needs of the pioneer Botswana MOHW EMS providers based on

PCaGuard: A Software Platform to Support Optimal Management of Prostate Cancer.

BACKGROUND AND OBJECTIVE: Prostate cancer (PCa) is a severe public health issue and the most common cancer worldwide in men. Early diagnosis can lead to early treatment and long-term survival. The addition of the multiparametric magnetic resonance imaging in combination with ultrasound (mpMRI-U/S fusion) biopsy to the existing diagnostic tools improved prostate cancer detection. Use of both tools gradually increases in every day urological practice. Furthermore, advances in the area of information technology and artificial intelligence have led to the development of software platforms able to

New kid on the block: An investigation of the physical, operational, personnel, and service characteristics of recovery community centers in the United States.

BACKGROUND: Professional treatment and non-professional mutual-help organizations (MHOs) play important roles in mitigating addiction relapse risk. More recently, a third tier of recovery support services has emerged that are neither treatment nor MHO that encompass an all-inclusive flexible approach combining professionals and volunteers. The most prominent of these is Recovery Community Centers (RCCs). RCC's goal is to provide an attractive central recovery hub facilitating the accrual of recovery capital by providing a variety of services (e.g., recovery coaching; medication assisted treatm

Capacity of the 45 and Up Study to mobilise evidence-based improvements in cancer control: lung cancer case study.

OBJECTIVE: Over the 15 years since the 45 and Up Study (the Study) was established, researchers have harnessed its capacity for enabling rigorous, comprehensive investigation of cancer causes, care, and outcomes. For the first time in Australia, the entire cancer-control continuum could be investigated by linking questionnaire data with cancer registry notifications, hospital records, outpatient medical services and prescription medications at scale. Here, we use lung cancer as a case study to demonstrate the Study's potential to improve cancer control. METHOD: Narrative description. RESULTS
