The optimal surgical treatment for primary hyperparathyroidism in MEN1 patients: a systematic review.

BACKGROUND: The optimal surgical approach for patients with primary hyperparathyroidism (pHPT) and multiple endocrine neoplasia 1 (MEN1) is controversial. We sought to determine the optimal type of surgery for pHPT in MEN1. METHODS: We collected data on clinical presentation, surgery, and follow-up for MEN1 patients with pHPT at the University Medical Center Utrecht and affiliated hospitals between 1967 and 2008. Furthermore, we performed a systematic review of the literature and meta-analysis. Surgical procedures were classified into less than subtotal (<SPTX) versus subtotal (SPTX) and tota

Short-Term Outcomes of Surgery for Graves' Disease in Germany.

BACKGROUND: Surgical treatment of Graves' disease (GD) has a potentially increased incidence of postoperative hypoparathyroidism, recurrent laryngeal nerve palsy (RLNP) and bleeding. The aim of this study was to evaluate the current extent of surgery for the treatment of GD and its safety as a short-term outcome. METHODS: Patients who underwent thyroid resection for GD were identified from the prospective StuDoQ/Thyroid registry. Patient data were retrospectively analyzed regarding demographics, surgical procedures and perioperative outcomes. Statistics were performed with Student's t-test or

Evaluating temozolomide for pediatric adamantinomatous craniopharyngiomas using microspheroid-based drug screening.

PURPOSE: Pediatric adamantinomatous craniopharyngioma (ACP) is the most common tumor of the diencephalic-pituitary axis in children. Although histologically benign, pediatric ACP is frequently associated with substantial endocrine dysfunction and neurological complications. The gold-standard treatment is complete surgical resection; however, because of the tumor's location, total removal is often not feasible. Consequently, several authors recommend radiotherapy as an adjuvant option. Nevertheless, partial resections are frequently followed by recurrence, and repeated surgical interventions in

Risk factors of hypocalcemia after total thyroidectomy. A high volume center experience.

INTRODUCTION: Thyroidectomy is one of the most frequently performed surgical procedures worldwide. The most common complication of total thyroidectomy (TT) in the early postoperative period is hypocalcemia. This study aims to determine the risk factors for postoperative hypocalcemia after TT and to reveal their clinical value. As a secondary outcome, we assessed the effects of iatrogenic parathyroidectomy, surgical experience, and parathyroid transplantations on prolonged than one month hypocalcemia and intravenous calcium infusion requirement after TT. METHODS: We designed our study as a ret

The Role of Surgery in Chronic Pancreatitis.

Chronic pancreatitis is an inflammatory disease characterized by the progressive replacement of the functional pancreatic parenchyma with fibrotic tissue. This leads to exocrine and endocrine insufficiency. A typical clinical feature is recurrent, severe upper abdominal pain, which negatively affects the patient's quality of life. Conservative treatment as the method of first choice does not prevent irreversible changes in the pancreatic tissue. While endoscopic drainage can have some benefits in the early stages of the disease, it is generally unsuccessful in the long term. Based on recent st

Total Hip Arthroplasty for Neglected Pathological Femoral Neck Fracture in a Young Patient with Autoimmune Polyglandular Syndrome.

INTRODUCTION: Neglected pathological femoral neck fractures in young patients are rare and present significant surgical challenges, particularly in the presence of systemic metabolic disorders such as autoimmune polyglandular syndrome (APS). Poor bone quality, impaired healing potential, and endocrine instability complicate management, often limiting the success of head-preserving procedures. PURPOSE: The purpose of the study was to describe the surgical decision-making, technical considerations, and early outcomes of total hip arthroplasty (THA) in a young patient with a neglected pathologic

Robotics and general surgery.

Robotics are now being used in all surgical fields, including general surgery. By increasing intra-abdominal articulations while operating through small incisions, robotics are increasingly being used for a large number of visceral and solid organ operations, including those for the gallbladder, esophagus, stomach, intestines, colon, and rectum, as well as for the endocrine organs. Robotics and general surgery are blending for the first time in history and as a specialty field should continue to grow for many years to come. We continuously demand solutions to questions and limitations that are

Exoscope-Assisted Ovarian-Sparing Resection of Contralateral Mature Teratomas in Bilateral Pediatric Ovarian Disease: A Case Series.

BACKGROUND: Bilateral ovarian tumors in pediatric patients present a surgical challenge requiring complete tumor removal while preserving sufficient ovarian tissue for endocrine function and future fertility. In bilateral disease, preservation is often feasible only on the ovary that retains salvageable parenchyma. Exoscope systems provide high-definition magnified visualization of the operative field. To our knowledge, this is one of the first reports of exoscope-assisted ovarian-sparing surgery in pediatric patients. This study describes the technical feasibility of the technique and the com

Multiple endocrine neoplasia type 2A: an unusual clinical presentation and association with renal dysplasia.

We report what we believe to be the first case of a patient with multiple endocrine neoplasia type 2A (MEN 2A) and renal dysplasia associated with an RET 634 mutation. The proband presented at the age of 29 with medullary thyroid carcinoma (MTC), bilateral pheochromocytomas, and primary hyperparathyroidism. Screening of family members identified the syndrome in his father. Both the proband and his father carry RET 634 germline mutation resulting in cysteine to arginine amino acid substitution. The proband had a left nephrectomy at the age of 10 years. Histologic examination of the resected kid

Clinical performance of a machine learning-based model for detecting lymph node metastasis in papillary thyroid carcinoma: A multicenter study.

Papillary thyroid carcinoma (PTC) is a common endocrine malignancy with a generally favorable prognosis, but lymph node metastasis (LNM) complicates treatment and increases recurrence risk. Current preoperative methods like neck ultrasound often miss LNM, leading to unnecessary surgeries. This study developed a non-invasive, artificial intelligence (AI)-driven predictive model for LNM using gene expression data from 157 PTC patients and validated it with qRT-PCR across 807 participants from multiple centers. The model focused on three key genes - RPS4Y1, PKHD1L1, and CRABP1 - chosen for their

Robotic-assisted Lateral Pancreaticojejunostomy for Chronic Pancreatitis.

Chronic pancreatitis is a debilitating, benign inflammatory disorder that significantly alters a patient's quality of life. Pancreatic drainage procedures have long been demonstrated to be safe and feasible using laparoscopic and open surgical approaches but have largely been replaced by pancreaticoduodenectomy. Over the past several decades, perioperative outcomes using the robotic platform in hepatobiliary surgery have been rapidly improving and, in many cases, surpassing outcomes in open and laparoscopic approaches. In appropriately selected patients with pancreatic duct obstruction and wit

Rat models of frozen shoulder: Classification and evaluation.

Frozen shoulder (FS), also known as adhesive capsulitis, is a condition that causes contraction and stiffness of the shoulder joint capsule. The main symptoms are persistent shoulder pain and a limited range of motion in all directions. These symptoms and poor prognosis affect people's physical health and quality of life. Currently, the specific mechanisms of FS remain unclear, and there is variability in treatment methods and their efficacy. Additionally, the early symptoms of FS are difficult to distinguish from those of other shoulder diseases, complicating early diagnosis and treatment. Th

Updates in metabolic bariatric endoscopy.

Obesity and its associated comorbid conditions have been increasing globally. Endoscopic bariatric and metabolic therapies (EBMTs) were initially designed to replicate bariatric surgery physiology for those who are not or choose not to be surgical candidates. Now, newer procedures target the complicated pathophysiology underlying obesity and its comorbidities. EBMT has been categorized based on its therapeutic target (stomach or small intestine), but innovations have expanded to include extraintestinal organs including the pancreas. Gastric EBMTs, namely space-occupying balloons, gastroplasty
