Thoracic endovascular aortic repair (TEVAR) has emerged as the preferred treatment for descending thoracic aortic aneurysms (TAA), but one of its major complications, type 1A endoleaks, can lead to aneurysm rupture and mortality if not managed promptly and effectively. This case report details an 87-year-old male patient who developed a type 1A endoleak following a TEVAR procedure for a large descending TAA. The patient's complex vascular anatomy, including a dominant right vertebral artery and an aneurysmal left subclavian artery, necessitated a tailored surgical approach. A right-to-left car

BACKGROUND: The longitudinal clinical significance of type II endoleaks (T2ELs) remains controversial. Specifically, the real-world incidence, need for reintervention, and associated mortality referable to T2ELs remain unknown. Moreover, current professional society clinical practice guidelines recommend differing aneurysm sac growth thresholds to prompt intervention. Therefore, the purpose of this analysis was to better quantify the prevalence of early T2ELs after infrarenal endovascular aortic aneurysm repair (EVAR) and determine its association with reintervention and survival.

Type II endoleak is one of the most common and problematic complications after endovascular aneurysm repair. It has been suggested that the inferior mesenteric artery (IMA) embolization could prevent further adverse events and postoperative complications. This article is a systematic review and meta-analysis following PRISMA guidelines. The Medline, PubMed, Embase, and Cochrane databases were used to identify studies that investigated the effect of IMA embolization on the occurrence of type II endoleaks and secondary interventions in a group of patients with abdominal aortic aneurysm who under

We report a case of combined types IIIb and Ia endoleak that developed 6 years after endovascular aneurysm repair (EVAR) with the Endurant II® endograft for abdominal aortic aneurysm (AAA). The patient presented with post-EVAR AAA rupture and underwent emergency open repair. We observed types IIIb and Ia endoleak and successfully performed felt banding to preserve the stent graft. Type IIIb endoleak with the Endurant® endograft is rare, and treatments have not been fully established. We summarized the case reports regarding type IIIb endoleak with the Endurant® endograft and mainly discussed t

Objectives: Aneurysm shrinkage after EVAR is the strong factor of favorable outcomes after endovascular abdominal aortic aneurysm repair (EVAR), and type II endoleaks is the risk factor of no aneurysm shrinkage or aneurysm enlargement in the long term. In this study, we evaluate the aortic side branches relate to early postoperative type II endoleak, and performed coil embolization for those vessels for prevention of type II endoleak. Methods: Patency and diameter of aortic side branches including inferior mesenteric artery (IMA) and lumbar artery (LA) were evaluated in 56 consecutive patients

Fenestrated endovascular repair (FEVAR) has become a safe and effective option for the treatment of complex aortic aneurysms (CAA). Complications can occur, including endoleaks like type IIIc endoleak, characterized by flow between a fenestration and its bridging stent. Although some endoleaks may resolve spontaneously, most require secondary interventions to prevent further expansion of the aneurysmal sac and other complications. We describe a percutaneous "stent-and-plug" rescue maneuver performed in 5 consecutive patients with refractory type IIIc endoleaks after company-manufactured FEVAR

PURPOSE: Persistent type 2 endoleaks (pT2ELs) following endovascular aortic aneurysm repair (EVAR) pose surveillance challenges. Although previous studies demonstrated that low D-dimer levels at single time points indicate lower risk of aneurysm enlargement (AnE), the value of longitudinal D-dimer monitoring remains unexplored. This study examined associations between D-dimer dynamics and AnE in patients with pT2ELs.
METHODS: This is a retrospective single-center cohort study. Consecutive patients undergoing elective EVAR (2007-2022) were reviewed. Inclusion criteria were isolated pT2ELs conf

OBJECTIVE: Type II endoleaks (T2ELs) are commonly identified after endovascular aneurysm repair (EVAR) and may occur either at the completion of the procedure or during follow-up. However, the impact of T2EL on reintervention and survival remains poorly described. This study aims to evaluate the outcomes associated with T2EL in a real-world cohort using the Vascular Quality Initiative linked Medicare claims (Vascular Quality Initiative-Medicare) database.
METHODS: We retrospectively reviewed all elective EVARs in the Vascular Quality Initiative-Medicare (part of the Vascular Implant Surveilla

Endovascular aneurysm repair (EVAR) is a minimally invasive treatment for abdominal aortic aneurysms (AAAs). Common complications include endoleaks, which are continued blood flow into the aneurysm sac external to the graft. Type I endoleaks occur proximally or distally, resulting from inadequate seals between the graft and artery. Type III endoleaks stem from defects between components in modular grafts or fabric tears. Re-intervention is indicated for type I and III endoleaks due to pressurization of the aneurysm sac resulting in a high risk of rupture. A 68 year-old man presented with an in

OBJECTIVE: Fenestrated or branched endovascular aortic arch repair (fb-arch repair) is an effective option for treating complex aortic arch lesions, including thoracic aortic aneurysms and aortic dissections. However, the relatively high rate of re-intervention due to target vessel (TV)-related endoleaks have raised concerns. This study aimed to determine risk factors for TV-related endoleaks after fb-arch repair.
METHODS: This was a retrospective analysis of all patients undergoing fb-arch repair between 2017 and 2021in nanjing drum tower hospital of China. All the patients underwent compute

Primary aortoenteric fistula (AEF) is an uncommon but life-threatening condition. We present a case of primary AEF in an octogenarian with previous endovascular aortic repair, type II endoleak and end stage liver and renal disease. He was successfully treated with accessory renal artery ligation, duodenojejunostomy, aneurysm sac debridement and irrigation and closure of the aneurysm sac over a drain. The patient made an excellent recovery and was discharged on POD #7, with no complications noted after over a year of follow up. This approach may represent a valuable option to manage primary AEF

OBJECTIVE/BACKGROUND: Proximal type I endoleaks are associated with abdominal aortic aneurysm (AAA) growth and rupture and necessitate repair. The Nellix™ EndoVascular Aneurysm Sealing (EVAS) system is a unique approach to AAA repair, where the appearance and treatment of endoleaks is also different. This study aimed to analyse and categorise proximal endoleaks in an EVAS treated cohort.
METHODS: All patients, treated from February 2013 to December 2015, in 15 experienced EVAS centres, presenting with proximal endoleak were included. Computed tomography scans were analysed by a core laborator

The purpose of the study was to assess the effectiveness of contrast-enhanced ultrasonography (CEUS) in endoleak classification after endovascular treatment of an abdominal aortic aneurysm compared to computed tomography angiography (CTA). From May 2001 to April 2003, 10 patients with endoleaks already detected by CTA underwent CEUS with Sonovue to confirm the CTA classification or to reclassify the endoleak. In three conflicting cases, the patients were also studied with conventional angiography. CEUS confirmed the CTA classification in seven cases (type II endoleaks). Two CTA type III endole

OBJECTIVE: This study aimed to perform hemodynamic simulations using patient-specific models of abdominal aortic aneurysms (AAA) following endovascular aneurysm repair (EVAR) to investigate the association between energy loss (EL) and Type I endoleak.
METHODS: Pre- and post-EVAR models were reconstructed from computed tomography angiography (CTA) for four patients with Type I endoleak and four without. Component quantification of EL, along with wall shear stress (WSS)-related hemodynamic parameters, were employed to evaluate the potential of endoleak.
RESULTS: Endoleaks following EVAR tended

OBJECTIVE: Type II endoleak (T2EL) remains the most common endoleak after endovascular aneurysm repair. We evaluated whether the cuff-first technique (CFT), in which an aortic cuff is deployed at the branch-vessel level before main-body deployment, is associated with lower early T2EL rates.
METHODS: We performed a single-center retrospective comparative study of infrarenal endovascular aneurysm repair (October 2020-December 2024). Patients without contrast-enhanced computed tomography at 6 months and other predefined exclusions were excluded. The primary endpoint was T2EL on 6-month computed
