A 49-year-old man presented with dysphagia and the sensation of a foreign body in the throat, after he had swallowed chicken meat that contained bones during the previous evening. There were no respiratory symptoms. A plain lateral soft-tissue X-ray of the neck and oesophagogastroduodenoscopy showed a grossly swollen epiglottis. His condition improved rapidly with intravenous antibiotic therapy. As acute epiglottitis may be a sudden life-threatening condition, a high index of suspicion should be maintained for patients who present with an obstructive sensation in the throat.

The aim of this study was to examine the outcome of epiglottitis in a small paediatric institution. Over the 5 year period October 1986 to September 1991, 44 children (of whom 15 were retrieved) with epiglottitis were admitted to the only paediatric intensive care facility in the Hunter Region. The so-called classical features were often absent. Forty of the 44 children were intubated. The mean length of intubation was 22 h and the mean length of hospital stay was only 2 1/2 days. One child had a respiratory arrest prior to intubation but made a complete recovery; one child had a coexistent su…

Invasive laryngeal candidiasis is an uncommon cause of epiglottitis, seen most often in immunocompromised or antibiotic-treated patients. We present the clinical and radiographic findings in an HIV-infected infant who developed significant stridor due to candidal epiglottitis. Stridor or hoarseness of unknown cause in an immunocompromised patient should lead to prompt evaluation for opportunistic laryngeal infection.

This paper reports the experience in the application of modified epiglottic laryngoplasty in 20 extended hemilaryngectomies for glottic carcinoma. The Kambic-Sedlacek-Tucker (K-S-T) technique using the epiglottis in glottic reconstruction can lead to early decannulation with an excellent airway, but a wide glottis formed after operation will increase the chance of aspiration and poor voice. Sphincteric function preservation of the glottis is a key point to success in partial laryngectomy. In order to prevent aspiration and to improve the quality of voice, the reconstructive technique as modifi

We present a case of Haemophilus influenzae lobar pneumonia in the father of a child admitted with acute epiglottitis caused by the same organism. The suggestion that adult, as well as child, contacts for Haemophilus influenzae epiglottitis should be prophylactically treated is discussed.

Although the importance of proper management of acute epiglottitis has been well recognized, little attention has been paid to the vulnerable period of time before the patient reaches the hospital. The courses of 23 children with acute epiglottitis were reviewed to determine prehospitalization management and morbidity.

The incidence of acute epiglottitis treated in Melbourne, Australia at the Royal Children's Hospital (RCH) has increased from an average of 14 cases per year during 1975-79 to 49 cases per year during 1980-81. Less marked increases have occurred for H. influenzae meningitis and bacteremia and H. influenzae (untyped) isolation from respiratory tract cultures. A review of 171 cases of acute epiglottitis showed no significant differences between the 1975-79 and 1980-81 patients with respect to sex, age, seasonal incidence, prodromal length, prodromal symptoms, geographical location, polymorphonuc…

A non-instrumentation treatment protocol for acute epiglottitis that features the use of dexamethasone to reduce edema and positive pressure ventilation by bag and mark to support ventilation, if needed, has been used in 40 cases, 1974--1977, with no mortality. The presence of the airway specialist (pediatric surgeon) in the Intensive Care Unit with the patient further assured the airway. The importance of having a protocol in place that is workable for the specific local circumstances of each institution is emphasized.

Acute epiglottitis is a pediatric airway emergency. No deaths occurred in 61 cases treated by elective tracheostomy at The Montreal Children's Hospital between 1966 and 1974. Seven patients (11.5%) showed evidence of aspiration pneumonia prior to tracheostomy and seven others developed pulmonary atelectasis in the first 24 hours following tracheostomy but only 9.8% of the total had a complication which continued in evidence 24 hours following surgery. A further 19.5% developed further complications unrelated to this earlier morbidity prior to discharge from hospital. The mean cannulation time …

We report an unusual case of a neonate with aphonia due to epiglottitis with a concomitant methicillin-resistant S taphylococcus aureus (MRSA) infection of the genitalia and associated septic emboli to the groin area and mouth. We postulate that the MRSA infection caused a transient bacteremia that seeded the epiglottis, likely causing the epiglottitis. In the evaluation of a neonate with aphonia, while the two primary differentials to consider are vocal cord paralysis and laryngeal web, among other considerations is epiglottitis (supraglottitis).

Acute epiglottitis (AE) is inflammation of the epiglottis and contiguous tissues, which carries a potential for complete airway obstruction. With routine pediatric immunization for Hemophilus influenzae serotype b, epiglottitis is now more prevalent in adults, with a shift in the causative organisms and a change in the natural history of this disease. Over the past 5 decades, Serratia marcescens has gone from being recognized as a harmless saprophyte to an important opportunistic human pathogen. It is known to be associated with outbreaks of nosocomial infections, but it is an uncommon cause o…

OBJECTIVES: We studied the incidence and bacterial epidemiology of acute epiglottitis presenting in the first 16 years following the introduction of general childhood vaccination against Haemophilus influenzae type b. Our main objectives were to analyse (1) the incidence of Streptococcus pneumoniae epiglottitis in adults and (2) the distribution of pneumococcal serotypes involved.
MATERIALS AND METHODS: The medical records of patients with acute epiglottitis (International Classification of Disease code J05.1) were investigated. Streptococcus pneumoniae serotyping was performed using gel prec…

BACKGROUND: After deployment of the Haemophilus influenzae vaccination, the range of pathogens causing acute epiglottitis has changed, as has the epidemiology from a primarily pediatric syndrome towards more frequent adult onset.
OBJECTIVES: We present a case of acute-onset meningococcal epiglottitis in an adult patient, to our knowledge one of a few reported cases in the medical literature. We review the historic changes and outcomes of similar episodes.
CASE REPORT: A 37-year-old diabetic man presented to our Emergency Department in acute respiratory distress. Examination revealed epiglott…
