[Imaging in paediatric epilepsy].

Epilepsy is a chronic disease, often with an onset during childhood and characterized by spontaneous and recurrent seizures. It concerns 0.5-1% of children under 16 years of age. A classification proposed by the International League Against Epilepsy (ILEA) in 2001 takes into account recent genetic factors involved in epilepsy and attenuates the sharp demarcation between generalized and partial seizures. This classification tends to define whether imaging is indicated or not. Imaging is useless in simple cases of fits associated with hyperthermia and in benign idiopathic epilepsy. It is debated

Imaging idiopathic generalized epilepsy.

It has been traditionally held that there is no radiological abnormality in patients with idiopathic generalized epilepsy (IGE). Sophisticated image processing and quantitative magnetic resonance imaging (MRI) studies suggest that, in some cases, there may be a subtle structural abnormality. Magnetic resonance spectroscopy indicates neuronal dysfunction with differing patterns of abnormality in the IGE sub-syndromes, and high levels of glutamate and glutamine have been suggested in the frontal lobes, and low GABA levels in the occipital lobe. Studies reflecting cerebral blood flow at the time

Brain stimulation for epilepsy.

Neural stimulation is a promising new technology for the treatment of medically-intractable seizures. Vagus-nerve stimulation (VNS) is licensed in several countries as an adjunctive therapy. VNS is as effective as antiepileptic drug therapy, and serious complications are rare. Transcranial magnetic stimulation is simple, non-invasive, and widely used in neurophysiology. Therapeutic results in a few studies are equivocal at best. Deep brain stimulation, although experimental, has been applied to the cerebellum, caudate nucleus, centromedian thalamus, anterior thalamus, subthalamus, hippocampus,

[Surgical treatment of epilepsy].

In this article the possibilities, indications, methods and results of surgery in epilepsy are summarized in general with the Hungarian experience emphasized. Surgery may provide effective treatment in about 5-10% of the epileptic population. Surgical solution nowadays became an essential treatment in medial temporal epilepsy, if hippocampal sclerosis or other lesion is present, in therapy resistant lesional extratemporal epilepsies and in catastrophic childhood epilepsies if the epileptic disorder is restricted to one hemisphere (Rasmussen syndrome, hemimegalencephaly, Sturge-Weber disease an

Sulthiame in childhood epilepsy.

BACKGROUND: Sulthiame is a central carbonic anhydrase inhibitor found to be effective for both partial and generalized seizures. It has been in use in some European countries and in Israel for over 30 years. The aim of the present study was to evaluate the efficacy and tolerability of sulthiame in childhood epilepsy by conducting a multicenter, retrospective study of patients who received this drug. METHODS: The charts of 125 consecutive epilepsy patients treated with sulthiame as monotherapy or add-on therapy were reviewed. RESULTS: Twenty-nine out of 39 patients with benign focal epilepsy

Epilepsy in neurofibromatosis 1.

Neurofibromatosis 1 is the most common neurocutaneous disease. Neurologic manifestations are mainly represented by tumors such as optic gliomas, focal areas of high T2-weighted signal known as unidentified bright objects, and mental retardation or learning disabilities. The prevalence of seizures has been reported to range from 3.8 to 6%. In the present study, we evaluated prevalence, type, and etiology of epilepsy in a neurofibromatosis 1 population. A retrospective analysis of 198 patients affected by neurofibromatosis 1 was performed. Fourteen patients (7%) were found to be epileptic. Every

Adolescent issues in epilepsy.

Approximately one third of new cases of epilepsy have their onset before age 20 years. Many children will enter adolescence with epilepsy or have an onset of seizures during adolescence. Adolescence is a time of dramatic change in growth, hormonal, psychologic, and social situations. Seizure frequency, teenage pregnancy, driving, and alcohol and drug use often become major issues during the adolescent years. Furthermore, adolescents often have difficulty accepting the chronicity of epilepsy and complying with medications, which can result in physical injury and perceived or real obstacles to e

Epilepsy in Aging Populations.

Older adults pose special challenges when seizures or epilepsy occur. Increased risk for seizures is often unrecognized. Older adults may have milder epilepsy, and may not require or tolerate standard doses of anti-epileptic drugs (AEDs). Polypharmacy and drug interactions are potential problems, and altered protein binding, hepatic function, renal clearance, and gastric function can affect AED dosing. Side effects such as ataxia, somnolence, and confusion are of heightened concern in this group. Older adults typically suffer localization or focal onset seizures. Although many AEDs treat these

[Epilepsy. Classification and genetics].

A common symptom of epilepsies are recurrent unprovoked epileptic seizures. These can be classified as focal or generalised. Thus, we can speak of focal or generalised epilepsies. Considering the rest of the medical history and the results of EEG and brain imaging the terms idiopathic, symptomatic and cryptogenic (i.e. probably symptomatic) are used to further characterise an epilepsy in a single patient. These classification scheme which is used since the end of the eighties allows decision making in therapy and an estimation of prognosis. Due to progress in epileptology during the last decad

Magnetoencephalography in extratemporal epilepsy.

Epilepsy surgery candidates with extratemporal foci represent a particular diagnostic and therapeutic challenge, because of anatomic and functional features of the pertaining areas. In the last decade, novel developments in the field of electrophysiological techniques have offered new approaches to detailed localization of specific epileptic discharges as well as eloquent regions. Magnetoencephalography, in combination with neuroimaging data and simultaneously recorded EEG, yields promising results to clarify centers of epileptic activity and their relationship to structural abnormalites and f

Risk factors are multiple probably involving a combination of seizure, family and child variable, behavioral problems (depression, occur frequently but may be unrecognized). Anxiety disorders and psychoses are less common. DEVELOPMENT: The learning and behavioral found in epileptic children is multifactorial, first the effects of the conditions causing the epilepsy, in addition the epilepsy itself may cause, change in the nervous syst

Epilepsy and Cortical Dysplasias.

Focal cortical dysplasias (FCD) and diffuse cortical dysplasias (DCD) are a heterogeneous group of disorders defined by abnormal cerebral cortical cytoarchitecture that are associated with epilepsy. Patients with either DCD or FCD may suffer from a variety of epilepsy subtypes and these are often refractory to most anti-epileptic drugs (AEDs) despite polytherapy. The etiologies of cortical dysplasias (CD) are diverse, and include inherited genetic syndromes such as Miller-Dieker or X-linked lissencephaly, subcortical band heterotopia, and the tuberous sclerosis complex, as well as nongenetic e

Catastrophic epilepsy in childhood.

Although for most children epilepsy is a relatively benign disorder, for some, epilepsy can be designated as "catastrophic" because the seizures are so difficult to control and because they are strongly associated with mental retardation. The catastrophic childhood epilepsies include uncommon disorders such as early infantile epileptic encephalopathy with suppression burst, severe myoclonic epilepsy of infancy, and epilepsy with myoclonic-astatic seizures. There are other syndromes that are relatively common such as infantile spasms, Lennox-Gastaut syndrome, and Sturge-Weber syndrome. Many chi

[Epilepsy and autonomic diseases].

This review article focuses on the functional anatomy of the central autonomic nervous system and the autonomic symptoms and dysfunctions occurring with epileptogenic activity involving areas of the central autonomic nervous system. Clinical experiences have demonstrated a close relation between epileptic and central autonomic activity. Autonomic symptoms are frequent signs of epileptic seizures and may cause dysfunctions in almost every organ system. Cardiorespiratory dysfunction has been described interictually. The increased frequency of sudden unexplained death in epilepsy patients may be

Legal aspects of epilepsy.

This article discusses issues of concern for physicians who treat patients with epilepsy. It particularly focuses on driving laws and employment of people with epilepsy. Physicians are often asked to provide certification to state driving authorities regarding whether their patients are fit to drive; sometimes, they are required to report patients who experience seizures to the state. Potential liability areas for physicians are also discussed in this article. In addition, this article addresses the increasing importance of the physician's role in determining whether a person who has seizures

Ethanol, GABA and epilepsy.

Ethanol exerts its behavioral effects largely by interacting with receptors to brain neurotransmitters. The molecular mechanisms involving these interactions are still not well known since an ideal model for their study is currently unavailable. In addition, responses to alcohol may vary due to factors such as genetic predisposition, ethanol concentration consumed, and stimuli such as stress, socialization, etc. The chronic consumption of alcohol, similar to that of other drugs such as benzodiazepines and barbiturates, is linked to GABAergic neurotransmission. GABA is the predominant inhibitor

Depressive disorders in epilepsy.

Depression is a common occurrence among epileptic patients and constitutes, along with anxiety disorders, the most frequent psychiatric condition in these patients. The relationship between depression and epilepsy is two-directional, because patients with major depression also have a higher frequency of epilepsy. In epileptic patients, depressive disorders can present as unipolar, bipolar, or dysthymic disorders. More characteristically, however, they present as an atypical depression, which can often go unrecognized for long periods of time. In the diagnostic evaluation of these patients, cli
