Comparison of Sampling Methods for Detecting Protein in Gastrointestinal Endoscopes.

Background: Persistent microbial contamination of flexible endoscopes has been linked to infections and outbreaks. Valid and reliable sampling methods are critical for monitoring processing effectiveness in flexible endoscopes. In this study, the effectiveness of protein extraction via turbulent fluid flow (TFF) sampling was compared with flush-only sampling in manually cleaned gastrointestinal endoscopes. Methods: A crossover study design, in which both sampling methods were used in alternating order during each endoscope encounter, was utilized to assess protein levels after colonoscopes and

A review of Spaulding's classification system for effective cleaning, disinfection and sterilization of reusable medical devices: Viewed through a modern-day lens that will inform and enable future sustainability.

Despite advances in medicine and innovations in many underpinning fields including disease prevention and control, the Spaulding classification system, originally proposed in 1957, remains widely used for defining the disinfection and sterilization of contaminated re-usable medical devices and surgical instruments. Screening PubMed and Scopus databases using a PRISMA guiding framework generated 272 relevant publications that were used in this review. Findings revealed that there is a need to evolve how medical devices are designed, and processed by cleaning, disinfection (and/or sterilization)

UV light-based reprocessing of flexible endoscopes without working channel in Oto-Rhino-Laryngology: an effective method?

BACKGROUND: Reprocessing of flexible endoscopes (FEs) is often expensive, time consuming, and becomes increasingly complex, due to rising demands of hygiene. After beneficial results in reprocessing of rigid endoscopes using Impelux™ UV-C light technology, we tested the same method for reprocessing of FEs without working channel. MATERIALS AND METHODS: Testing was performed on FEs without working channel after routine clinical use (transnasal flexible endoscopy). Disinfection consisted of mechanical precleaning and 60 s exposure to Impelux™ UV-C light technology. Bacterial contamination was t

[Efficacy of Slightly Acidic Electrolyzed Water against Contamination of Water Line of Dental Units].

OBJECTIVES: The purpose of this study was to evaluate the efficacy of slightly acidic electrolyzed water (SAEW) against the contamination of the water line of dental units and the effects of SAEW on the water line. MATERIALS AND METHODS: The experimental material was a prototype dental unit equipped with a SAEW generator. SAEW is directly supplied to each device or part of this unit system. Experimental SAEW samples were collected from a high-speed handpiece (HS-1), an ultrasonic scaler, and a cup filler of the prototype dental unit. Control samples were taken before and after the prescribed

Evaluation of Measures against Exposure during Administration of Hazardous Drugs through a Feeding Tube.

Hazardous drugs (HD), which need to be handled with care, may be administered through a feeding tube using the simple suspension method. However, instrument contamination during HD administration with the simple suspension method remains unclear. Therefore, to minimize such contamination during the simple suspension method using an injector, we propose the following exposure countermeasures method: (1) Wear two layers of gloves. (2) Prepare injectors for administration and flushing. (3) Use caps. (4) Replace outer gloves after the removal of tablets from the press-through package (PTP) sheet.

Biofilms: Microbial Strategies for Surviving UV Exposure.

Biofilm communities are an ingenious form of protection of microbial cells which have been evolving for billion of years. In general, ultraviolet (UV) radiation presents poor penetration in the matrix of biofilms and only the first few top layers of microbial cells are exposed to its deleterious effects. For further protection against UV radiation, exposed cells can produce specialized compounds such as mycosporine-like amino acids and carotenoid pigments. In this chapter, the adaptation mechanisms presented by biofilms against UV radiation are presented, as well as the application of UV light

Multi-use Venturi nasal atomizer contamination in a clinical rhinologic practice.

INTRODUCTION: Cross-contamination of Venturi atomizers has been reported. METHODS: In phase I, 16 atomizers were sterilized and refilled with either 1% lidocaine or 0.1% Tyzine (day 0). During phase II, atomizers were wiped with isopropyl alcohol wipes between uses. In both phases, on days 7, 14, 21, and 28, the contents of the atomizer were sprayed onto two culture media. If a culture was found positive, cultures from the nozzle, lumen, and solution were taken and cultured in depth. RESULTS: Twelve lidocaine bottles and three Tyzine bottles were initially positive. There were more positive

Creutzfeldt-Jakob disease: implications for gastroenterology.

The current clinical views regarding variant Creutzfeldt-Jakob disease, and in particular transmission via endoscopy, of those representing both gastroenterology and the Spongiform Encephalopathy Advisory Committee are presented in an attempt to guide clinicians as to "best practice" given the current state of our knowledge.

[Intravascular device-related infection].

Intravascular device-related infection is one of the most important nosocomial infections. More than 200,000 cases with blood stream infection occur each year and most of them are related to the use of an intravascular device in United States. The microorganisms most frequently involved are Staphylococcus epidermidis, Staphylococcus aureus and Candida spp. It is very important to prevent and treat the catheter-related infections. Many researches have been done to develop methods for preventing and treating intravascular device-related infection. They include diagnosis of catheter-infections, a

[Bloodstream infection].

Bloodstream infection is the most serious disease and the third mortality rate among nosocomial infections. Recently, catheter-related bloodstream infection(CRBSI) is the major cause of the bloodstream infection, related to serious complication, leading to increase in morbidity and mortality. Coagulase-negative staphylococci, Staphylococcus aureus, enterococci and Candida spp. are most commonly causes of CRBSI. A variety of methods such as aseptic insertion techniques and appropriate catheter care have proved effective, which are also preferable to prevent CRBSI. If CRBSI occurs, empirical ant

Test Process and Results of Potential Masking of Sterilizing-Grade Filters.

Pre-use/post-sterilization integrity testing (PUPSIT) has been a widely debated topic for the last several years. To a large extent, the debate is because of the fact that scientific data were not available to provide additional clarity that could inform appropriate risk-based judgements and commensurate actions. To gain clarity, the Parenteral Drug Association (PDA) and BioPhorum Organizations Group (BioPhorum) formed the Sterile Filtration Quality Risk Management (SFQRM) consortium in late 2017. The consortium goals have been to fill existing gaps in scientific data as adequately as possible

Reprocessing safety issues associated with complex-design orthopaedic loaned surgical instruments and implants.

BACKGROUND: The acquisition of reusable surgical instruments (RSIs) through loaner system is a worldwide phenomenon, particularly in orthopaedic surgeries. Loaner sets contain high-complex design RSIs, which are very difficult to clean, and also include single use implants, such as screws, that remain in the surgical tray and are subjected to multiple reprocessing until they are implanted. Inadequate cleaning of SI and various exposures of single-use implants to physical, chemical and biological agents can compromise their quality/safety and promote biofilm formation. The difficulty in cleanin
