Cleanability of dental instruments--implications of residual protein and risks from Creutzfeldt-Jakob disease.

Cleaning of dental instruments is the first line of control in reducing the adherent bioburden. The threat of vCJD and the difficulty in removing prion protein has provided a new challenge for cleaning surgical and dental instruments. Prion proteins are also more resistant to many disinfection and sterilisation techniques. A number of different methods are currently available in primary care for cleaning instruments including manual washing, ultrasonic cleaners and washer disinfectors. Manual cleaning of dental instruments is time-consuming, introduces operator error and the risk of puncture w

Reuse of negative CLOtest kits in children.

BACKGROUND: The rapid urease test, CLOtest, is used frequently in endoscopy suites. In the developed world, a negative CLOtest result is extremely common. The question of whether reuse of previously negative CLOtest kits is appropriate in children and the cost saving of such a practice were investigated. METHODS: Children who underwent diagnostic endoscopic procedures were prospectively recruited to the study. In each procedure gastric biopsy specimens were obtained for 2 rapid urease tests, one a new kit and the other a used negative kit obtained from previous procedures. In addition gastric

Web-based Cellpose‑SAM segmentation enables quantification of cross-sectional area in formalin-fixed paraffin-embedded skeletal muscle sections stained with hematoxylin and eosin.

Cross‑sectional area (CSA) of skeletal myofibers is a primary morphometric indicator of atrophy and regeneration, yet it is most commonly quantified on cryosections after immunofluorescent labeling of basal lamina proteins, which requires specialized reagents and equipment and limits reuse of routine histology slides. Here we present a practical, fully web‑based workflow that leverages Cellpose‑SAM on a public browser interface to segment myofibers directly from formalin-fixed paraffin-embedded (FFPE) skeletal muscle sections stained with hematoxylin and eosin (H&E) or toluidine blue (TB), fol

N95 mask reuse in a major urban hospital: COVID-19 response process and procedure.

BACKGROUND: The shortage of single-use N95 respirator masks (NRMs) during the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic has prompted consideration of NRM recycling to extend limited stocks by healthcare providers and facilities. AIM: To assess potential reuse via autoclaving of NRMs worn daily in a major urban Canadian hospital. METHODS: NRM reusability was assessed following collection from volunteer staff after 2-8 h use, sterilization by autoclaving and PortaCount fit testing. A workflow was developed for reprocessing hundreds of NRMs daily. FINDINGS: Used NRM

Autoclaving and clinical recycling: effects on mechanical properties of orthodontic wires.

BACKGROUND: About half of the orthodontists recycle and reuse orthodontic wires because of their costs. So when talking about reuse and sterilization of wires, their effects on mechanical properties of wires should be clarified. The purpose of this study was to assess the effects of sterilization and clinical use on mechanical properties of stainless steel wires. MATERIALS AND METHODS: Thirty stainless steel orthodontic wires were divided into three equal groups of control, autoclave (sterilized by autoclave), and recycle group (wires were used for orthodontic patients up to 4 weeks, cleaned

Evidence for decontamination of single-use filtering facepiece respirators.

Single-use filtering face respirators (FFRs) are critical pieces of personal protective equipment for healthcare workers treating patients with suspected upper respiratory tract pathogens. Experiences during pandemics in the 2000s, as well as the ongoing COVID-19 pandemic caused by the SARS-2-CoV-2, have highlighted concerns over the pressures that sustained respiratory virus pandemics may have on supplies of FFRs globally. Decontamination of FFRs has been posited as one solution to support the re-use of FFRs with a growing body of literature over the last 10+ years beginning to examine both t

Environmental sustainability in anaesthesia and critical care.

The detrimental health effects of climate change continue to increase. Although health systems respond to this disease burden, healthcare itself pollutes the atmosphere, land, and waterways. We surveyed the 'state of the art' environmental sustainability research in anaesthesia and critical care, addressing why it matters, what is known, and ideas for future work. Focus is placed upon the atmospheric chemistry of the anaesthetic gases, recent work clarifying their relative global warming potentials, and progress in waste anaesthetic gas treatment. Life cycle assessment (LCA; i.e. 'cradle to gr

Cotton fever: an evanescent process mimicking sepsis in an intravenous drug abuser.

BACKGROUND: Although many complications of intravenous drug abuse are well described, "cotton fever" has had little mention in recent medical literature. Cotton fever is street terminology for the post-injection fever experienced by many drug users after "shooting up" with heroin reclaimed from a previously used cotton filter. CASE REPORT: We report on a 22-year-old man with a history of intravenous drug abuse with fever 30 min after injecting heroin. He was intensely diaphoretic, tachycardic, and febrile. His workup was negative for any infectious etiology and he later admitted to reusing th

Decontamination Methods for Reuse of Filtering Facepiece Respirators.

IMPORTANCE: The novel coronavirus disease 2019 (COVID-19) has proven to be highly infectious, putting health care professionals around the world at increased risk. Furthermore, there are widespread shortages of necessary personal protective equipment (PPE) for these individuals. Filtering facepiece respirators, such as the N95 respirator, intended for single use, can be reused in times of need. We explore the evidence for decontamination or sterilization of N95 respirators for health care systems seeking to conserve PPE while maintaining the health of their workforce. OBSERVATIONS: The filtra

Efficacy and safety evaluation of a reusable advanced hemostatic device (Vi-Sealer) during total laparoscopic hysterectomy in South Korea (KGOG4009/Vi-TLH trial): study protocol for a multicenter, open-label, non-inferiority randomized controlled trial.

BACKGROUND: Advanced hemostatic devices (AHDs) are widely utilized in gynecologic minimally invasive surgeries. These AHDs control ultrasonic or bipolar electric/thermal energy with a feedback mechanism and effectively seal vessels without thermal spread. However, most AHDs are single-use, potentially increasing surgical costs. Vi-Sealer, a reusable AHD with an interchangeable blade, aims to address this issue. This study assesses operative outcomes and complications to evaluate the efficacy and safety of Vi-Sealer compared to other AHDs. METHODS: This multicenter, open-label, non-inferiority

Advances in Endoscope Reprocessing Technology and Its Impact on Pathogen Transmission.

Pathogen transmissions via flexible endoscopes have been documented in the literature and have been historically related to human error or omission of steps in the reprocessing cycle. The 2008 Centers for Disease Control and Prevention report challenged manufacturers of automated endoscope reprocessors to improve and advance technology to automate more of the reprocessing steps. A review and synthesis of the literature following the 2008 Centers for Disease Control and Prevention report was performed to evaluate whether advances in reprocessing technology have occurred and whether these have h

Filtration Efficiency of Hospital Face Mask Alternatives Available for Use During the COVID-19 Pandemic.

IMPORTANCE: Procuring respiratory protection for clinicians and other health care workers has become a major challenge of the coronavirus disease 2019 (COVID-19) pandemic and has resulted in nonstandard practices such as the use of expired respirators and various decontamination processes to prolong the useful life of respirators in health care settings. In addition, imported, non-National Institute for Occupational Safety and Health (NIOSH)-approved respirators have been donated or acquired by hospitals as a potential replacement for limited NIOSH-approved N95 respirators. OBJECTIVE: To asse
