An Integrative Review of Nurse-to-Nurse Incivility, Hostility, and Workplace Violence: A GPS for Nurse Leaders.

Bullying, lateral violence, and incivility are real phenomena in the nursing workplace and remain widespread across all care settings. The American Nurses Association recommends zero tolerance for any form of violence from any source and adopting evidence-based strategies to mitigate incivility and bullying. This integrative review examined the evidence regarding nurse-to-nurse incivility, bullying, and workplace violence for 4 nurse populations-student, new graduate, experienced, and academic faculty. Ganong and Cooper's review methodology structured the evidence synthesis. Twenty-one article

The use of medical orders in acute care oxygen therapy.

The life of every living organism is sustained by the presence of oxygen and the acute deprivation of oxygen will, therefore, result in hypoxia and ultimately death. Although oxygen is normally present in the air, higher concentrations are required to treat many disease processes. Oxygen is therefore considered to be a drug requiring a medical prescription and is subject to any law that covers its use and prescription. Administration is typically authorized by a physician following legal written instructions to a qualified nurse. This standard procedure helps prevent incidence of misuse or oxy

Evidence for using air or fluid when identifying the epidural space.

Lumbar epidural analgesia is frequently employed to provide pain relief for women during labor. Anesthesia providers use various methods to identify the epidural space. Some providers use air, some use fluid, and others use a combination of air and fluid during the loss of resistance technique. Loss of resistance to air has been speculated to result in a lesser quality of analgesia compared with loss of resistance to only fluid. A search strategy focusing on preappraised sources was used to locate evidence from interventional and observational studies. Four evidence sources were located, inclu

Integrating a multidisciplinary mobility programme into intensive care practice (IMMPTP): a multicentre collaborative.

BACKGROUND: ICU immobility can contribute to physical deconditioning, increased ICU and hospital length of stay and complications post discharge. Despite evidence of the beneficial outcomes of early mobility, many ICUs and providers lack necessary processes and resources to effectively integrate early mobility into their daily practice. OBJECTIVE: To create a progressive mobility initiative that will help ICU teams to address key cultural, process and resource opportunities in order to integrate early mobility into daily care practices. METHODS: An initiative to integrate the latest evidence

Troubleshooting common problems associated with long-term catheters.

A catheter complications survey study, carried out over a 2-year period in three Bristol health districts, recorded 506 emergency referrals during a 6-month period (Kohler-Ockmore and Feneley, 1996). A further detailed study on 54 of the patients showed that 48% experienced catheter blockage and 37% reported urine bypassing. Urinary catheterization can cause many health problems; bacteriuria is inevitable in long-term catheterized patients (Cravens and Zweig, 2000). This article identifies some of the common problems that can occur with long-term catheters, makes recommendations and applies an

Are first-time mothers who plan home birth more likely to receive evidence-based care? A comparative study of home and hospital care provided by the same midwives.

BACKGROUND: "Place of birth" studies have consistently shown reduced rates of obstetric intervention in low-technology birth settings, but the extent to which the place of birth per se has influenced the outcomes remains unclear. The objective of this study was to compare birth outcomes for nulliparous women giving birth at home or in hospital, within the practice of the same midwives. METHODS: An innovative survey was generated following a focus group discussion that compared midwifery practice in different settings. Two groups of matched, low-risk first-time mothers, one group who planned t

An evidence-based approach to breastfeeding neonates at risk for hypoglycemia.

The revised standard of care for breastfeeding infants at risk of developing hypoglycemia during transitioning to extrauterine life was developed using the American Academy of Pediatrics (AAP) 2011 hypoglycemia guidelines, the Academy of Breastfeeding Medicine protocol, and staff input. A pre/postimplementation chart audit indicated support of infant safety by glucose stabilization, breastfeeding within the first hour of life, and breastfeeding frequency without an increase in blood sampling, formula use, or admissions to the special care nursery.

Improving evidence-based practice: use of the POP-Q system for the assessment of pelvic organ prolapse.

The focus of improving health care outcomes today relies on utilizing evidence-based practice. Pelvic organ prolapse affects women of all ages and negatively impacts their quality of life. Evidence-based tools for consistent assessment of prolapses have been developed, validated, and used by many clinicians. Use of these tools needs to become standard of practice for all clinicians who work in the women's health arena. The Brink scale and the POP-Q assessment tool can assist clinicians, direct quality care, and provide evidence-based practice.
