Half-ring external fixation in the management of tibial plafond fractures.

OBJECTIVE: To investigate the management of tibial plafond fractures with a half-ring external fixator using half pins. DESIGN: Prospective. SETTING: University teaching hospital. PATIENTS: Twenty-four patients with AO Type A or C tibial plafond fractures. OUTCOME MEASURES: Outcome was assessed by using standard criteria of time to union, incidence of infection and malunion, and incidence of pin-tract sepsis. Functional return was assessed by measuring hindfoot function. A scoring system, incorporating clinical and radiological results, was used to rate the results. RESULTS: When using cl

Distraction-lengthening of digital rays using a small external fixator.

We present 19 ray lengthenings in 14 patients done with a small external fixator. In six cases the thumb was lengthened and in the other 13 cases, other digital rays. The most frequent reason for lengthening was an amputation sustained in a work accident. All the lengthenings were done by an osteotomy and subsequent gradual distraction with a small external fixator. The mean lengthening achieved was 20 mm (range, 0-32). An iliac-crest graft was needed in nine cases, corrective osteotomy in five cases and a deepening of the web in the six cases of thumb lengthening. In five cases an additional

Augmented external fixation of distal radius fractures: a biomechanical analysis.

An in vitro model of an unstable extra-articular distal radius fracture was created in 8 fresh-frozen cadaveric specimens and stabilized with an external fixator. Rotation and translation kinematics of the distal radial fracture fragment were measured in relation to the proximal radius during physiologic loading, using infrared light-emitting diodes and a 3-dimensional motion-sensing device. The effect of supplemental single and combination Kirschner wire (K-wire) fixation on fracture fragment stability was assessed. Fixation of supplemental K-wires to the fixator frame via a custom-developed

Long-term results of the external fixation of distal radius fractures.

OBJECTIVE: To evaluate the long-term results of external fixation of distal radius fractures. METHODS: A retrospective follow-up study (median follow-up, 5.3 years) of 49 patients with 50 distal radius fractures treated with an external fixator was carried out. An external fixator (Minifixator, Stratec Medical, Waldenburg, Switzerland) was used. The operative procedure is described in detail. A personal evaluation including clinical and radiologic assessment of both wrists was performed. RESULTS: The functional results, including the parameters strength, daily activities, range of motion, an

[Loosening of Schanz screws in external fixator montage of the lower extremity].

We analyzed the failure of Schanz pins in 234 patients with fractures of the lower extremity. All patients had the AO external fixator and stainless steel pins (ISO 58/32-1). The mean duration of treatment was 14 weeks. During this time 88 of a total of 1147 pins became loose. The first failures occurred 2 weeks after insertion. Most pins failed after 5-6 weeks. Early loosening was not accompanied signs of local infection, while late failures regularly developed pintract infections. Pins next to the fracture gap exhibited the highest rate of loosening. Intermediate pins had the lowest one. We

Management of recurrent, complex instability of the elbow with a hinged external fixator.

We have treated 16 patients with recurrent complex elbow instability using a hinged external fixator. All patients had instability, dislocation or subluxation of the ulnohumeral joint. The injuries were open in eight patients and were associated with 20 other fractures and five peripheral nerve injuries. Two patients had received initial treatment from us; 14 had previously had a mean of 2.1 unsuccessful surgical procedures (1 to 6). The fixator was applied at a mean of 4.8 weeks (0 to 9) after the injury and remained on the elbow for a mean of 8.5 weeks (6 to 11). After treatment we found the

Closed reduction and external fixation of unstable fractures of the distal radius.

The anatomical and functional outcome and complications after closed reduction and external fixation of unstable fractures of the distal radius were reviewed in 29 patients. The fractures were reduced by longitudinal traction and closed manipulation under fluoroscopic control, and the reduced position was retained by a standard half-frame Hoffmann external fixator for six to eight weeks. The mean follow-up time was four (3-7) years. The reduction failed in three patients after too early removal of the fixator (three to five weeks). For the rest of the patients the dorsal angulation, the radial

Early complications with external fixation of pediatric femoral shaft fractures.

A retrospective study of 27 pediatric patients with femoral shaft fractures treated by external fixation was made to identify complications and evaluate outcomes. The average age at the time of injury was 8 years, 9 months (range 5 years, 6 months to 13 years, 2 months). Sixteen fractures were isolated, and nine were associated with polytrauma. There was only one open fracture. Data obtained from chart review (n = 27), radiographs (n = 27), physical exam (n = 16), and questionnaire (n = 21) identified eight major complications (30%) in six patients and 29 minor complications (107%) in 20 patie

The pinless external fixator--relevance of experimental results in clinical applications.

The pinless external fixator was intended as a stable, temporary, minimally invasive fixator for severe tibial fractures ensuring safer conversion to an intramedullary nail. An in vitro study showed that the pinless fixator was mechanically not as stiff as the conventional AO tubular device, the main problem being low axial stiffness. This study involving initial clinical trials with the pinless fixator on tibial fractures in St. Gall is based on the experimental work and previous clinical experience of the main author. From June 1992 to June 1994 10 tibial fractures (eight II degrees and III

External fixation of distal radius fractures. Indications and technical principles.

External fixation of unstable intra-articular fractures of the distal radius has become an effective tool in the management of these difficult injuries. Careful assessment of the fracture pattern, appropriate patient selection, meticulous surgical technique, the appropriate choice of fixation device and pins, recognition of the need for augmentation with limited internal fixation or bone grafting, and aggressive postoperative rehabilitation provide the foundation for successful management of these fractures while minimizing complications. The surgeon must remember that the pin-bone interface i

Treatment of femoral shaft fractures with the Hoffmann external fixator in prepuberty.

Twenty-two children with femoral shaft fractures, between the age of 10 and 14 years, were treated with the Hoffmann external fixation apparatus from 1980 to 1985. This method of treatment is very simple. It requires less surgical exposure and allows earlier ambulation and easy nursing care. In addition, it makes patients comfortable in multiple trauma conditions. The average time to union and removal of fixators was 72 days (range, 44 to 80 days). There were no deep wound infections, nonunions, or rotationary deformities. On followup, all patients regained the range of motion of knee joints i

Stabilization of the lower thoracic and lumbar spine with external skeletal fixation.

For stabilization of the lower thoracic and lumbar spine, an external spinal skeletal fixation (ESSF) system has been developed and clinically used since 1977. The system consists of two pairs of Schanz screws and an adjustable external fixation device. The Schanz screws are firmly anchored through the pedicles into the vertebral bodies. Application of ESSF offers some unique characteristics beneficial in the treatment of unstable spinal injuries and spinal osteomyelitis. In contrast to other fixation systems, the number of vertebrae immobilized can be reduced with ESSF. Versatility of the sys

The A-O external skeletal fixator in the treatment of severe tibia fractures.

A series of 33 cases of Grade II and III open tibia fractures were treated with local wound care followed by application of the A-O external fixator. Two transfixing Steinmann pins were usually used above and two below the fracture site. In six cases one Steinmann pin and one anteroposterior Schanz half pin above and below the fracture were combined with a triangulated frame. Additionally, minimal internal fixation with lag screws was used in five cases. Union was achieved in 83% of tibiae in an average time of 9.9 months. Union occurred faster when the fixator was removed in less than 3.5 mon

External mandibular fixation for gunshot fractures: report of 2 cases.

High-energy ballistic injuries may cause comminuted facial fractures. Treatment of such fractures might be challenging because of in-fection and soft- and hard-tissue loss. These cases may not be amenable to open reduction and internal fixation. We present 2 cases of gunshot fractures, for which external fixation was used as a surgical step before definitive treatment. With the use of external fixation, existing infection had been controlled and soft tissues had been restored, which allowed oral rehabilitation with reconstruction plates and autogenous bone grafting, if needed.
