Multiple open and endoscopic techniques have been described for recalcitrant cases of plantar fasciitis. Compared with open techniques, endoscopic plantar fasciotomy has been shown to be safe and effective with decreased postoperative pain and quicker recovery, as well as decreased risk of soft tissue and neurovascular injury, while retaining the ability to provide direct visualization of the plantar fascia to facilitate proper release. Single-portal endoscopic techniques may offer additional advantages including less portal site and postoperative pain, earlier return to activities, and cost-e…

Neonatal compartment syndrome (NCS) is a rare condition that is often initially misdiagnosed because its skin lesions mimic several other more common diseases of the newborn. It has not been described in the pediatric literature thus far. Early diagnosis along with fasciotomy may be limb and function-sparing, but only in certain cases, because the exact time and duration of the initial insult and the full extent of damage at presentation is unknown in many cases. We present a case of NCS that was initially thought to be amniotic band syndrome, and delayed fasciotomy and debridement were perfor…

Accessory soleus is a rare congenital anatomical variant, which may manifest in the second/third decade of life as an exertional ankle pain and swelling or as an asymptomatic postero-medial mass. The incidence of this condition ranges from 0.7 to 5.5%. Many treatment options have been described in literature, including conservative treatment, excision, fasciotomy, release and closure of blood supply. We report a symptomatic massive accessory soleus (17 x 5 x 4 cm) in an 18-year-old male semi-professional soccer player. Excision of the accessory soleus was performed. The patient went back to th…

BACKGROUND: Popliteal artery injuries continue to result in limb loss. This study identifies risk factors that predict amputation.
METHODS: Over a 5-year period, a retrospective chart review was conducted of 80 consecutive patients with 81 popliteal artery injuries.
RESULTS: The overall amputation rate was 16.5%. Blunt trauma carried a higher rate of amputation (47%) than penetrating injuries (6.2%); P < 0.0001). Associated fractures had a higher amputation rate, regardless of mechanism (odds ratio +2.7, 95% confidence limits 1.2 to 6.2). Fasciotomy at the time of operation was associated wi…

The authors have observed ten compartment syndromes of the leg. Nine occurred after tibial fractures, one after a contusive trauma. Early signs those of muscular and neural ischemia, with an acute pain when the toes are passively mobilised. The cast must be promptly removed. In 6 cases the two compartments were involved. In all cases the pulses were palpable. Arteriographs could occasionaly verify the integrity of the main arterial trunks. The treatment is based on early fasciotomies. It was realised in 5 cases, with good results when the fasciotomy was realised more than twelve hours after th…

BACKGROUND: This retrospective study aimed to evaluate anesthesia management, intraoperative support strategies, and 60-day clinical outcomes in patients requiring surgical intervention, including fasciotomy and/or amputation, due to crush and compartment syndrome following the February 6, 2023, Kahramanmaraş earthquakes. The study also sought to identify predictors of mortality and contribute to the field of disaster medicine.
METHODS: This single-center study reviewed the data of 64 patients who underwent emergency surgery between February 6 and April 6, 2023. Retrospectively collected and …

Reported infection rates following severe open fractures of the lower extremity sustained in combat have varied widely, from 23% to 85%. The infection rates have been either similar to or higher than those reported in the civilian trauma literature. Deployed surgeons have increased the frequency of fasciotomy procedures for limbs with or at risk for clinical compartment syndrome. The long-term sequelae of compartment syndrome and fasciotomies are not clearly defined. The definition of the term late amputation has varied in the literature, and studies have not consistently included information …

BACKGROUND: Fasciotomy with resection of nonviable muscle is often necessary when there is a delay in compartment syndrome (CS) diagnosis after revascularization. The reported rate of major amputation following missed CS or delayed fasciotomy ranges from 12% to 35%. Herein, the authors present a series of critically ill patients who experienced delayed CS diagnosis and required complete resection of the anterior and/or lateral compartments but still achieved limb salvage and function.
METHODS: A retrospective chart review identified five patients from April 2018 to April 2019 within a single …

INTRODUCTION: Compartment syndrome of the foot is a rare complication of injury to the foot. Treatment by decompression of the compartments is debatable. The debate surrounding decompression stems from the rarity of the condition, the lack of consensus regarding the anatomy of the foot compartments and whether to accept the inevitable contractures by not decompressing. The aim of this paper is two fold; firstly to sample current military orthopaedic experience and secondly establish if there exists a consensus of opinion in how and if to perform fasciotomy of the foot thereby providing guidanc…

Two cases of envenoming by Cerastes vipera are described Both were in snake collectors who were accidentally bitten on the finger while handling the snake. In both cases, local signs included a haemorrhagic bulla with fang marks, swelling and tenderness. These signs were mild in one case and moderately severe in the other, necessitating fasciotomy. No systemic signs were observed. Some coagulation abnormalities were found in both cases. In one, prolonged bleeding from the wound and a shortened euglobulin lysis time may suggest activation of the fibrinolytic mechanism. In the other, prolongatio…

Testicular torsion causes a compartment syndrome physiology in which detorsion alone may fail to restore perfusion because intratesticular pressure remains elevated after untwisting. This report describes a simple intraoperative adjunct practised by the author in a district hospital setting in Zambia: multiple needle punctures of the tunica albuginea immediately after detorsion, without formal fasciotomy or tunica vaginalis flap. The technique decompresses the intratesticular compartment, releases congested venous blood and allows clinical grading of testicular viability by the colour and spee…

Dupuytren disease is a common benign fibromatosis of the palmar and finger fascia caused by pathological cord formation. As both the retrovascular and lateral cords are jointly responsible for the flexion contracture of the distal interphalangeal (DIP) joint, isolated DIP joint contracture caused by Dupuytren disease is rare. We present a 34-year-old right-hand-dominant male patient with a 6-month history of an isolated DIP joint flexion contracture in the right ring finger due to Dupuytren disease. Surgical fasciotomy of the retrovascular cord improved the contracture without adverse events. …

Arthroscopic posterior cruciate ligament (PCL) reconstruction carries some risk of complications, including injury to the neurovascular structures at the popliteal region. We describe a delayed presentation of the right leg and foot compartment syndrome following rupture of popliteal artery pseudoaneurysm, which presented 9 days after an arthroscopic transtibial PCL reconstructive surgery. Fasciotomy, surgical exploration, repair of an injured popliteal vein and revascularisation of the popliteal artery with autogenous great saphenous vein interposition graft were performed. Owing to the close…

Our report describes the case of a 20 year old man who developed an acute exertional anterior tibial compartment syndrome after playing soccer for five minutes. No trauma was reported. Correct diagnosis was delayed for 18 hours, because pain was misinterpreted as resulting from acute muscular tetanic spasm induced by hyperventilation. Fasciotomy was contributed. After one year, the patient complained of early fatigue. Electromyographically the reinnervation had not yet been completed. This case illustrates the necessity for careful observation of non-trauma related acute tibial anterior pain a…

Between 1969 and 1981, 61 patients with 85 popliteal vascular injuries were seen. In the nine patients who required amputation, common risk factors included (1) extensive time delays (greater than 36 hours) between injury and repair, (2) associated bone and soft-tissue injuries leading to postoperative wound infection. amd (3) early occlusion of popliteal artery repair or delay in performance of fasciotomy. The low amputation rate in patients without operative delay or associated bone and soft-tissue injuries (2/54 = 3.7%) was attributed to the careful application of standard vascular surgery …

BACKGROUND: Compartment syndrome commonly occurs in patients with forearm and lower leg fractures. Compartment syndromes of the gluteal and thigh muscles are less common. It is imperative that compartment syndrome be diagnosed and treated with fasciotomy as soon as possible. However, there are few reports on the diagnosis and treatment strategies for compartment syndromes that occur simultaneously in multiple anatomical regions or in the ipsilateral gluteal region and thigh.
CASE PRESENTATION: We report on a 76-year-old man who was obliquely crushed under a tree extending from the right forea…

INTRODUCTION: Chronic exertional compartment syndrome of the forearm is probably underdiagnosed as a cause of forearm pain in the sportsman. Its pathological basis is a critical elevation of extracellular pressure. The clinical diagnosis is confirmed by measurements of intracompartmental pressures. We described a reliable original method of endoscopically assisted superficial fasciotomy for treating chronic exertional compartment syndrome of the forearm. The goal of the study is the physiological and clinical validation of this technique.

UNLABELLED: Acute compartment syndrome of the thigh has been infrequently reported in the literature. Closed femoral fractures and blunt soft tissue trauma are the main causes of this injury. The multiple injured patient in this case report developed a compartment syndrome of the thigh after intramedullary nailing of a comminuted fracture of the femur. Fasciotomy was performed two days after surgery because of extense swelling of the thigh in the ventilated and sedated patient. Sciatic and femoral nerve palsy was recognized after extubation of the patient nine days after the injury. During the…
