COVID-19 Pandemic Reduced Utilization Of Interventional Techniques 18.7% In Managing Chronic Pain In The Medicare Population In 2020: Analysis Of Utilization Data From 2000 To 2020.

BACKGROUND: Multiple publications have shown the significant impact of the COVID-19 pandemic on US healthcare and increasing costs over the recent years in managing low back and neck pain as well as other musculoskeletal disorders. The COVID-19 pandemic has affected many modalities of treatments, including those related to chronic pain management, including both interventional techniques and opioids. While there have not been assessments of utilization of interventional techniques specific to the ongoing COVID-19 pandemic, previous analysis published with data from 2000 to 2018 demonstrated a

Life-Course Access to Speech and Language Therapy: A Rights-Based AAAQ Policy Mapping in Türkiye and Four Comparator Settings.

BACKGROUND AND AIMS: Access to Speech and Language Therapy (SLT) is often described in terms of whether a service exists. However, formal recognition alone does not ensure that individuals can obtain timely, affordable, coordinated, and sustainable care across the life course. The use of the rights-based Availability, Accessibility, Acceptability, and Quality (AAAQ) framework remains limited in comparative policy research on SLT access. England (UK), Sweden, Australia, and the United States were purposively selected to capture contrasting arrangements in statutory coordination, financial prote

Study of Total Ankle Arthroplasty and Ankle Arthrodesis Complications and Costs Based on Surgeon Specialty.

BACKGROUND: Total ankle arthroplasty (TAA) and ankle arthrodesis (AA) remain viable treatment options for end-stage ankle arthritis. Both procedures are performed by orthopaedic surgeons and podiatric surgeons. The literature comparing outcomes based on surgeon specialty is sparse. In this study, we studied postoperative outcomes for both TAA and AA based on surgeon specialty in a Medicare cohort of both fee-for-service and health maintenance organization claims. Our primary outcome was surgical complications, and secondary outcome was 90-day episode-of-care costs, based on surgeon specialty.

Contemporary Trends in Outcomes After Heart Failure Hospitalization Among Medicare Beneficiaries.

BACKGROUND: Contemporary trends in outcomes after heart failure hospitalization (HFH), particularly during and after the COVID-19 pandemic, are not well characterized. OBJECTIVES: This study analyzed temporal trends in outcomes after HFH among Medicare beneficiaries from 2016 to 2022. METHODS: This retrospective cohort study used national Medicare data and included patients who were hospitalized with a principal discharge diagnosis of heart failure between January 2016 and December 2022. Trends in adjusted 30-day mortality, readmission, and home time (days alive and out of health care facili

Medicare Beneficiary Receipt of Methadone by Drive Time to Opioid Treatment Programs.

IMPORTANCE: To combat the ongoing opioid crisis, policy makers and public health officials are developing novel policies to increase the availability of medications for opioid use disorder (OUD). An important question is to what extent geographic availability of opioid treatment programs (OTPs) is associated with treatment receipt. Understanding this association may help with developing additional policies to increase medication for OUD dispensing and improve population health outcomes. OBJECTIVE: To quantify trends in dispensing methadone to Medicare beneficiaries based on proximity to an OT

Medication Adherence, Health Care Utilization, and Costs Among Patients Initiating Oral Oncolytics for Multiple Myeloma or Chronic Lymphocytic Leukemia/Small Lymphocytic Lymphoma.

BACKGROUND: Oral oncolytic therapies have improved survival in hematologic cancers, such as chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL) and multiple myeloma (MM), which are now being managed like chronic conditions. However, compared with other cancers, there is a lack of studies assessing adherence, health care resource utilization, and costs in patients with these cancers. OBJECTIVE: To assess factors associated with adherence to oral oncolytic therapies, health care utilization, and costs in patients with CLL/SLL or MM. METHODS: A retrospective database study was con

Frequency of Disparities in Laboratory Testing After Statin Initiation in Subjects ≥65 Years.

Laboratory testing is important for the safety of older adults initiating statins, but there has been little examination of laboratory testing disparities by race/ethnicity, age, gender, Medicaid eligibility, and multimorbidity. The study's purpose was to examine disparities in guideline-concordant baseline laboratory testing and abnormal laboratory values among a retrospective cohort of 76,868 Medicare fee-for-service beneficiaries from 10 states in the eastern United States who had dyslipidemia and initiated a statin from July 1 to November 30, 2011. Guideline-concordant assessment of alanin

Head-to-head comparison of fee-for-service and diagnosis related groups in two tertiary referral hospitals in Switzerland: an observational study.

BACKGROUND: Reimbursement for inpatients in Switzerland differed among states until 2012. Some hospitals used diagnosis related groups (DRG) and others used fee-for-service (FFS). We compared length of hospital stay (LOS), patient satisfaction and quality of life between the two systems before a nation-wide implementation of DRG. METHODS: In a prospective, two-centre observational cohort study, we identified all patients with a main diagnosis of either community-acquired pneumonia, exacerbation of chronic pulmonary obstructive disease, acute heart failure or hip fracture from January to June

Economic Burden of Treatment-Resistant Depression in Privately Insured U.S. Patients with Physical Conditions.

BACKGROUND: Little is known about the economic burden of treatment-resistant depression (TRD) in patients with physical conditions. OBJECTIVE: To assess health care resource utilization (HRU) and costs, work loss days, and related costs in patients with TRD and physical conditions versus patients with the same conditions and non-TRD major depressive disorder (MDD) or without MDD. METHODS: Adults aged < 65 years with MDD treated with antidepressants were identified in the OptumHealth Care Solutions database (July 2009-March 2017). Patients who received a diagnosis of MDD and initiated a third

Description of the outcomes of prior authorization of palivizumab for prevention of respiratory syncytial virus infection in a managed care organization.

BACKGROUND: Respiratory syncytial virus (RSV) is the leading cause of upper and lower respiratory tract infections in infants and young children. Most children are exposed to the virus before they are 2 years old and experience such symptoms as cough, fever, and irritability. In a select population of infants, the virus can cause hypoxemia and hospitalization. To avoid hospitalization, good infection control practices should be employed, and for those infants at high risk, prophylaxis with palivizumab is indicated. Palivizumab has been shown to reduce hospitalization rates in high-risk infants

Association of Outpatient Practice-Level Socioeconomic Disadvantage With Quality of Care and Outcomes Among Older Adults With Coronary Artery Disease: Implications for Value-Based Payment.

BACKGROUND: Medicare patients with coronary artery disease (CAD) have been a significant focus of value-based payment programs for outpatient practices. Physicians and policymakers, however, have voiced concern that value-based payment programs may penalize practices that serve vulnerable populations. This study evaluated whether outpatient practices that serve socioeconomically disadvantaged populations have worse CAD outcomes, and if this reflects the delivery of lower-quality care or rather, patient and community factors beyond the care provided by physician practices. METHODS AND RESULTS:

Unequal access to restorative coastal blue-green places: social vulnerability and place-based well-being opportunities in a coastal metropolis.

BACKGROUND: Urban blue-green environments are vital components of the urban exposome, shaping daily opportunities for physical activity, stress mitigation, and broader public health. However, these benefits depend on daily accessibility. In compact coastal metropolises, extensive nature may coexist with highly unequal residential access. This study investigates whether socially vulnerable residential areas in Shenzhen experience reduced exposure to public green and coastal blue spaces. METHODS: Using 4,071 residential patches, a residential-scale exposure framework was developed. Social vulne

Extreme heat and hospitalization with Parkinson's disease among older adults.

BACKGROUND: The projected rise in U.S. Parkinson's Disease (PD) cases makes understanding risk factors crucial to plan for changing healthcare utilization patterns. Factors such as extreme heat exacerbated by shifts in regional climate conditions may present additional risks among the PD population. OBJECTIVE: We aim to determine whether nationwide and climate region-specific acute heat index exposure is associated with increased rates of hospitalizations with PD among the Medicare population. METHODS: We follow a time-stratified case-crossover study design for a population of Medicare fee-f

Fertility preservation trends, perspectives, and reflections in gender diverse adults across the life course.

STUDY QUESTION: What is the fertility preservation (FP) utilization rate among transgender and gender diverse (TGD) patients at the Transgender and Intersex Specialty Care Clinic (TISCC) within the Mayo Clinic? SUMMARY ANSWER: The rate of utilization of FP services by TGD patients at Mayo Clinic's TISCC was determined to be 6.62%. WHAT IS KNOWN ALREADY: TGD patients encounter several barriers when pursuing FP, which may contribute to a lower utilization rate of these services. These barriers include financial constraints and lack of insurance coverage, as well as societal and healthcare prov

Is There Variation in Procedural Utilization for Lumbar Spine Disorders Between a Fee-for-Service and Salaried Healthcare System?

BACKGROUND: Whether compensation for professional services drives the use of those services is an important question that has not been answered in a robust manner. Specifically, there is a growing concern that spine care practitioners may preferentially choose more costly or invasive procedures in a fee-for-service system, irrespective of the underlying lumbar disorder being treated. QUESTIONS/PURPOSES: (1) Were proportions of interbody fusions higher in the fee-for-service setting as opposed to the salaried Department of Defense setting? (2) Were the odds of interbody fusion increased in a f
