Fever and Rash.

Infectious causes of fever and rash pose a diagnostic challenge for the emergency provider. It is often difficult to discern rashes associated with rapidly progressive and life-threatening infections from benign exanthems, which comprise the majority of rashes seen in the emergency department. Physicians must also consider serious noninfectious causes of fever and rash. A correct diagnosis depends on an exhaustive history and head-to-toe skin examination as most emergent causes of fever and rash remain clinical diagnoses. A provisional diagnosis and immediate treatment with antimicrobials and …

Fever with Rashes.

Fever with rashes is one of the commonest clinical problems a general practitioner or pediatrician has to face in day-to-day clinical practice. It can be a mild viral illness or a life-threatening illness like meningococcemia or Dengue hemorrhagic fever or it can be one with a lifelong consequence like Kawasaki disease. It is very important to arrive at a clinical diagnosis as early as possible with the minimum investigational facilities. The common causes associated with fever and rashes are infections, viral followed by other infections. There can be so many non-infectious causes also for fe…

Fevers and the rheumatologist.

Fevers in children are mainly due to infection, malignancy or inflammatory conditions. Rheumatologists have an important role in the care of inflammatory conditions, many of which are associated with fevers. Seven conditions, the hereditary recurrent fever syndromes, have been defined with the presenting symptom of recurring fever, and for which mutation of a single gene has been defined: Chronic infantile neurological articular syndrome (CINCA), Familial cold autoinflammatory syndrome (FACS), Familial Mediterranean fever (FMF), hyperimmunoglobulinemia D (HIDS), Muckle-Wells syndrome (MWS), Py…

[Fever and hyperthermia].

The knowledge of the physiological mechanisms for controlling body temperature, and the etiology and pathophysiology of fever and hyperthermia nursing care enable these interdisciplinary problems, allowing early detection to prevent its progression. It also allows the nurse the implementation of quality care through the adoption of specific measures to lower the temperature, the administration of medical therapy, hemodynamic monitoring, respiratory and metabolic control and water.

[Periodic fevers].

Periodic fever is defined as a series of unexplained febrile episodes, most often starting during childhood. The febrile episodes last usually few days, are of fixed or variable duration, and regress spontaneously, the intervals between episodes being asymptomatic. Fever is accompanied by clinical manifestations affecting peritoneal, pleural and/or mucous membranes, joints and skin. Four different etiologies are presently known. Three are hereditary diseases: familial mediterranean fever and periodic fever with hyperimmunoglobulinemia D which have a recessive autosomal transmission, and TNF re…

"Fever in the Hand".

A 14-year-old female adolescent was being treated for malnutrition secondary to anorexia nervosa. She complained of severe "shooting" pain, swelling, and color and temperature changes in her left (dominant) hand. She had recently suffered a fall during a hockey game and pounded on the table during a "temper tantrum." On exam, the left hand was cyanotic, edematous, and cold, with severe hyperesthesia. A bone scan showed marked reduction in flow to the left hand. She underwent eight sympathetic ganglion blocks resulting in transitory Horner's syndrome. Favorable plethysmographic changes occurred

[Fever and headache].

A 30-year-old male patient was admitted to our outpatient clinic because of fever, headache and cerebellar symptoms. Clinically he presented with a slight meningism. After exclusion of a focal intracerebral process by head scanning, a first diagnosis of an aseptic meningitis was made by the analysis of the cerebrospinal fluid. With the hint of repeated tick bites, the diagnosis of an early-summer meningoencephalitis was confirmed. The patient recovered without neurological residua within two months. The clinical course, dates of new epidemiologic studies and problems of vaccination are discuss…

[Fever with chills].

Abrupt onset of fever with chills represents a body response to various stimuli called exogenous pyrogens. These substances interact with monocytes and macrophages, thus releasing various cytokines including interleukin-1 and tumor necrosis factor (endogenous pyrogens). Endogenous pyrogens act centrally on the thermosensitive neurons in the hypothalamus to increase the core body temperature by increasing heat production and decreasing heat loss. In addition, endogenous pyrogens trigger a non-specific inflammatory response and amplify the specific immune response through activating the T- and B…
