A patient suffering from ankylosing spondylitis required surgical excision of a large anterior osteophyte of the cervical spine. Fibreoptic nasal intubation was difficult due to distortion of the airway by the osteophyte. This cause of difficult flexible fibreoptic intubation has not been described previously.

The authors report a case of plasma cell granuloma of the lung, an inflammatory tumour occurring in a young Egyptian aged 23 without any history of chest diseases. The patient had had three weeks before his hospital admission light haemoptysis and associated with weight loss. The diagnosis was made on a bronchial biopsy. Thus a diagnosis could be made without recourse to surgery because of the occurrence of bronchial invasion. No aetiological factor was found to explain the pathogenesis of the granuloma.

As the first screening for colorectal neoplasms, a total of 2243 examinations by fiberoptic sigmoidoscopy (FS) were performed on 1573 asymptomatic patients. One hundred seventy neoplasms were found in 134 patients (6 percent of total examinations), including nine adenocarcinomas (0.4 percent). A flat, intramucosal cancer and a small, 5-mm cancer detected by FS could not be demonstrated by barium enema examination. On further investigation, additional adenomas were identified in the proximal colon in 22 percent of patients in whom neoplasms had first been detected by FS and in only 4.4 percent

Eight hundred and twenty patients were examined by fiberoptic sigmoidoscopy (SIG) and double contrast barium enema (DCBE) to detect colonic cancers or adenomas. Cancer or adenoma in the bowel tract proximal to the upper limit of SIG insertion was detected in 4 patients and in 12 on DCBE. Monopolar electrosurgical cutting was used to correct epiglottal entrapment in 5 horses. The operations were carried out in the conscious animal, using topical anesthesia. In two patients with non-specific lesions of oesophagitis, malpighian cells infected with type I herpes simplex virus were collected by brush cytology. The cytological appearance was typical of herpes infection. Within a specimen comprising many highly necrotic areas, giant multi-nucleated malpighian cells were seen, with marginate nuclear chromatin and characteristic inclusion bodies in the nuclei. The discovery of herpes lesions in the oesophagus raises important disease questions, both from the point of view of opportunistic infection and the development of cancer.

Spastic dysphonia is a speech disorder, characterized by a "strangled voice quality," irregularly occuring phonatory arrests, and a jerky pattern of speech. This is often extremely disruptive to the patient's ability to communicate. Two cases are presented in which a new technique, videofiberoptic laryngoscopy, was employed in the study of these patients. The characteristic laryngeal behavior of this disorder is documented and recorded, particularly noting the increase in supraglottic activity and how different vocal maneuvers alter this activity.

The endoscopic tamponade method, using a new transparent balloon tube, has been developed for the emergency control of bleeding esophageal varices. The introduction of the tube is performed while observing the esophagus utlizing a fiberoptic endoscope, which is inserted within the lumen of the tube so that the accidental injury to the mucosa can be avoided.
