Retrograde locked nailing of pubic ramus fractures: a technical note.

Percutaneous screwing of the pubic ramus presents several limitations, such as the infeasibility within a bone corridor that is too narrow or too curved, or the lack of proximal locking, which may result in the mobilization of implants. This technical note presents an original method for percutaneous fixation of pubic ramus fractures using retrograde locked nailing. The implant used is the fibula rod system (Acumed), a proximally locking fibula nail. Four patients underwent surgery for pelvic ring fractures with anterior fixation using this technique, allowing immediate weight-bearing. All the

Isolated Proximal Fibular Stress Fracture In Osteoarthritis Knee Presenting As L5 Radiculopathy.

INTRODUCTION: Isolated proximal fibular stress fractures are rare and usually seen only in athletes and military recruits. Its occurrence with osteoarthritis of the knee is not documented. Diagnosis of stress fractures is not difficult, but they can mimic other pathologies at times. CASE REPORT: A 45-year-old male patient presented with pain and paresthesias in left lower thigh and leg. He was previously treated as L5 radiculopathy confirmed with lumbar spine magnetic resonance imaging (MRI). He received analgesics followed by nerve root blocks at another center. He was referred to our center

A comparison of the effects of ibuprofen and rofecoxib on rabbit fibula osteotomy healing.

BACKGROUND AND PURPOSE: Non-steroidal anti-inflammatory drugs (NSAIDs) inhibit cyclooxygenase (COX) activity, which is the rate-limiting enzyme in the synthesis of prostaglandins. Previous studies have indicated that NSAID therapy, and in particular NSAIDs that specifically target the inflammatory cyclooxygenase (COX-2), impair bone healing. We compared the effects of ibuprofen and rofecoxib on fibula osteotomy healing in rabbits to determine whether nominal, continuous inhibition of COX-2 with rofecoxib would differentially affect fracture healing more than cyclical inhibition of COX-2 using

Comparison of augmentation methods for internal fixation of osteoporotic ankle fractures.

BACKGROUND: Internal fixation of osteoporotic ankle fractures is associated with failure of fixation. This study compared different augmentation methods biomechanically. MATERIALS AND METHODS: In nine paired fresh-frozen cadaver legs, an identical supination-external rotation type II ankle fracture was created. Fractures were stabilized using an eight-hole locking plate. In four pairs, two screws were inserted across the syndesmosis for purchase in the tibial metaphysis. One leg from each pair was randomly selected for injection of calcium sulphate-calcium phosphate graft into the screw holes

Predictors of Peripheral Vascular Injury in Patients with Blunt Lower Extremity Fractures.

BACKGROUND: Early identification of peripheral vascular injuries is crucial to prevent acute limb ischemia and amputation. We sought to identify predictors of vascular injury in patients with blunt lower extremity fractures. METHODS: A single institutional retrospective analysis of patients with blunt lower extremity fractures at a university-affiliated, county hospital over a 2-year period was performed. Patients with lower extremity vascular injury were compared to patients without lower extremity vascular injury. Multivariate logistic regression analysis was performed to identify independe

Comparison of Pediatric Intercalary Allograft Reconstructions with and without a Free Vascularized Fibula.

BACKGROUND: Lower extremity intercalary allograft reconstruction has traditionally been used to facilitate limb salvage. Combining the allograft with a free vascularized fibula graft has been described; however, there is a paucity of data comparing the outcome of these reconstructions. METHODS: Twenty-nine pediatric patients (15 boys and 14 girls) underwent lower extremity limb salvage with the use of intercalary cadaveric allograft at a mean age of 12 years. The most common diagnosis was osteosarcoma (n = 18). Patients underwent reconstruction with an allograft alone (n = 11) or supplemented

Isolated bilateral fibular insufficiency fractures: Case report of an unusual entity.

Insufficiency fractures are a type of stress fractures that occur due to physiological stress on weakened bones. These have certain sites of predilection of which an uncommon site is fibular shaft. Isolated bilateral fibular fractures have previously only been reported twice, where the causes were repeated trauma and fatigue fracture. Insufficiency fractures of both fibula in isolation have not previously been reported. We present this case where a young patient presented with these fractures and was found to have vitamin D deficiency on subsequent investigations.

Reconstruction of porcine critical-sized mandibular defects with free fibular flaps: the development of a craniomaxillofacial surgery model.

Vascularized bone flaps are the well-known standards of care for reconstruction of segmental mandibular defects ≥ 6 cm. We developed a large animal critical-sized mandibular defect model in which osseous free fibula flaps were used for reconstruction. In this study a total of eight 3-month-old Yorkshire pigs underwent 6 cm full-thickness resection of the left hemimandible. An osseous free fibula flap from the left leg was harvested and contoured to the mandibular defect. Bone placement and plate position was confirmed with fluoroscopy. Animals were followed with serial radiographs and clinical

Fibular head transfixion wire and its relationship to common peroneal nerve: cadaveric analysis.

Proximal tibio-fibular joint is routinely stabilised during leg lengthening, peri-articular fractures and deformity corrections of tibia. Potential injury to the common peroneal nerve at the level of the fibula head/neck junction during wire insertion is a recognised complication. Previous studies have mapped the course of the common peroneal nerve and its branches at the level of the fibular head, and guidelines are published regarding placement of proximal tibial wires. This study aims to relate the course of the common peroneal nerve to the placement of a lateral insertion fibula head trans

Association between ankle fractures and obesity.

Obesity is an epidemic in the United States and is associated with an increased risk of musculoskeletal problems. Rotational injuries of the ankle with a Weber C fibula fracture have a greater risk of syndesmosis disruption and instability. The goal of the present study was to explore the association between obesity and ankle fractures. Using a retrospective review, the radiographs of 280 patients with an ankle fracture were reviewed and classified using the Weber classification, which was then associated with the body mass index, gender, age, diabetes, tobacco use, and osteoporosis. Patients

An Epidemiological Study of Primary Bone Tumors of the Fibula.

BACKGROUND: Relatively low incidence has led to an under-rating of fibula tumors. This study aimed to evaluate fibula tumors as a whole and to give detailed information based on histological types according to the anatomic location of the tumors in the fibula. METHODS: Evaluation was made of all the primary bone tumors of the fibula recorded in our bone tumor registry and institute of pathology from 2007 to 2018. Of these, 62 cases were identified. Analysis included assessment of age, gender, tumor localization, the presenting symptoms, the duration of symptoms, and treatment methods. RESULT

Osteolysis after rigid fixation. The possible role of periosteal neural mechanoreceptors in bone remodeling.

Rigid intramedullary fixation in the rat tibiofibular bone of a tibial fracture caused the development of nonunion in the associated fibular fracture, with marked osteolytic resorption of the fibular fragments. In contrast, with flexible fixation, the associated fibular fracture healed normally. The tibial fractures healed uneventfully in both cases. Histologic examinations showed that there exists a plexus of encapsulated nerve endings (Pacinian corpuscles) in the periosteum of the fibula, at the distal tibiofibular junction. The rat fibula forms a bony contact with the lower midshaft of the

Comparison of different locking plate fixation methods in lateral malleolus fractures.

BACKGROUND: Several fixation methods may be used for displaced lateral malleolar fractures. We aimed to compare clinical and radiologic outcomes associated with use of locking one third tubular plate vs. anatomical distal fibula locking plate in lateral malleolar fractures. METHODS: A total of 62 orthopedic patients operated for lateral malleolus fracture were included in this retrospective study. Patients were divided into two groups regarding the plate used for fixation as locking one third tubular plate (group I; n=37) and locking anatomical distal fibula plate (group II; n=25). Data on Da

Operative Setup to Improve Sagittal Syndesmotic Reduction: Technical Tip.

We analyzed the operative measures that may be used to reduce the likelihood of sagittal syndesmotic malreduction. Hence, we propose a simple technical tip to avoid sagittal plane malreduction of the fibula within the syndesmosis in ankle fractures. Supporting the leg under the heel should be avoided when performing syndesmotic reduction for unstable malleolar fractures, and support under the calf should be used instead. Our observations have been confirmed in 6 cadaver specimens. We observed that there was a significant anterior subluxation of the fibula when the leg was supported under the h

Stability criteria for nonoperative ankle fracture management.

BACKGROUND: This study was designed to see if stability based criteria are useful in choosing between nonoperative and operative treatment of ankle fractures. MATERIALS AND METHODS: One hundred sixty ankle fractures in skeletally mature patients were retrospectively analyzed to obtain an epidemiological profile in a population of about 130,000. One hundred thirty patients had followup of more than 2 years. A decision between operative and nonoperative treatment was made by the surgeon-on-duty, based on accepted stability criteria. Fractures were classified according to Weber and Lauge-Hansen

Lower limb fracture presentations at a regional hospital.

UNLABELLED: We found that lower limb fractures, which were largely the result of minimal trauma, had high levels of hospitalisation, length of stay and surgery. It is therefore important to prevent fractures at all sites to avoid the associated morbidity and mortality. PURPOSE: Hip fractures are a major cause of morbidity and mortality, particularly in older women. In comparison, less is known about the epidemiology and burden of other lower limb fractures. The study aimed to investigate the epidemiology and burden of these fractures. METHODS: Incident fractures of the hip, femur, tibia/fibu
