Unilateral S-1 root compression syndrome caused by fracture of the sacrum.

Unilateral S-1 nerve root compression after an S-1 sacral fracture was found in an 18-year-old man after a motor vehicle accident. The positive myelogram, myelogram-computed tomogram, and magnetic resonance studies led to surgical intervention. Marked bony callous formation contiguous with the S-1 alar fracture protruded into the canal and was responsible for tethering the S-1 nerve root. A right L-5 hemilaminectomy, an L-4, L-5 and L-5, S-1 medial facetectomy, and foraminotomy facilitated nerve root decompression. Postoperatively, the patient was markedly improved. The authors suggest a more …

A Rare Case of Radiculopathy: Monostotic Fibrous Dysplasia of the Sacrum.

Fibrous dysplasia is a bone disease characterized by an osteoblastic dysfunction resulting in a fibrous replacement of the normal medullary bone. We describe the case of a 33-year-old who presented with low back pain irradiating to her right leg. Both the computed tomography scan and magnetic resonance imaging showed an osteolytic, multicystic lesion of the right hemi-sacrum with invasion of the right S1 foramen. She underwent foraminotomy and curettage of the lesion. Histological diagnosis was fibrous dysplasia, without features of malignant transformation. Three years after surgery the patie…

Treatment of kyphosis and lumbar stenosis in achondroplasia.

In summary, neurological symptoms in the achondroplast are related to the anatomical spinal canal with degenerative changes and aging altering the delicate biomechanical and microvascular balance. An accurate and detailed history is necessary for diagnosis. Decompression is via a posterior approach with a multi-level wide laminectomy with undercutting of the facets and usually multi-level foraminotomies. Kyphosis when present at the time of decompression, or if angular or progressive, requires treatment. A combined two-stage approach with an anterior disc excision and fusion combined with a po

Clinical and cost-effectiveness of PCF versus ACD in the treatment of cervical brachialgia (FORVAD trial).

BACKGROUND: Cervical radiculopathy occurs when a nerve root is compressed in the spine, if symptoms fail to resolve after 6 weeks surgery may be indicated. Anterior Cervical Discectomy (ACD) is the commonest procedure, Posterior Cervical Foraminotomy (PCF) is an alternative that avoids the risk of damage to anterior neck structures. This prospective, Phase III, UK multicentre, open, individually randomised controlled trial was performed to determine whether PCF is superior to ACD in terms of improving clinical outcome as measured by the Neck Disability Index (NDI) 52 weeks post-surgery. METHO…

Herpes Zoster-Mediated Radiculitis After Lumbar Spine Surgery: A Report on 2 Cases with Differing Outcomes.

CASES: Two cases of herpetic neuralgia after lumbar fusion are reported. Although in one case the diagnosis was evident and treatment promptly initiated, the second exhibited a less typical presentation of neuralgia predating the cutaneous rash leading to a misdiagnosis of persistent nerve root stenosis, with additional surgeries and a longer hospital stay. CONCLUSION: The cases presented highlight why postoperative neuralgia should be approached carefully and systematically investigated. Although rare, postoperative neuralgia may occur as a consequence of postoperative herpes zoster infectio

Navigation-guided endoscopic lumbar decompression on foramen and lateral recess in advanced scoliosis.

An 84-year-old woman presented with left leg radiating pain for 18 months and a numeric rating scale score of 8. From examination, motoric on left knee extension was grade 4, with dysesthesia and numbness along the left anterolateral thigh. Imaging showed left L3-4 foraminal and lateral recess stenosis with severe-degree scoliosis. The patient underwent navigation-guided endoscopic transforaminal foraminotomy and lateral recess decompression on the left L3-4 level with a good outcome. Three-years' follow-up showed a well-maintained clinical outcome and coronal sagittal balance. This video expl…

Neurological recovery after surgical treatment of giant cervical pseudomeningoceles extending to lumbar spine associated with previous brachial plexus injury.

The authors describe a case of 28-year-old man who presented with cervical myelopathy and lumbar radiculopathy due to the giant cervical pseudomeningocele extending to the lumbar spine at 10 years after previous brachial plexus injury. To evaluate the communicating tract between pseudomeningocele and subarachnoidal space, the multidetector-row helical CT with simultaneous myelography was performed preoperatively. The surgical treatment in the cervical spine included the resection of pseudomeningocele and the repair of dural defects communicating into the cyst following multi-level laminoplasty

A review of laminoforaminotomy for the management of lateral and foraminal cervical disc herniations or spurs.

BACKGROUND: Anterior versus posterior surgical management of lateral and foraminal cervical disc disease remains controversial. The key hole foraminotomy or laminoforaminotomy allows dorsal resection without the instability encountered with anterior cervical approaches, with more limited morbidity. Unilateral radiculopathy can be addressed with the laminoforaminotomy, while bilateral or multifocal radiculopathy with myelopathy may additionally require a laminectomy or laminoplasty. METHODS: Selection of patients for laminoforaminotomy should be based upon correlation of clinical findings and …

[The role of surgery in the treatment of sciatica. An experience of more than 4000 operations].

Sciatica must always be first considered as a medical problem. Operation is indicated only for refractory cases. Analysis of more than 4 036 operations shows the best indications for surgery. These consist of typical disc sciatica, sciatica with excessive pain, with paralysis and with cauda equina syndrome. Myeloradiculography was not carried out routinely, being reserved for atypical forms. A negative radiculography is not necessarily a contraindication. The results of foraminotomy are less constant than after removal of a disc prolapse. It benefits 60 p. cent of patients, however. Some reser…

Cervical nerve root synovial sarcoma in a child with chromosomal (X;18) translocation. Case report and review of the literature.

We report on an 11-year-old female with a history of cervicobrachialgia and progressive weakness of the right arm. Cervical spine MRI showed an enhancing heterogeneous intradural mass occupying the right C6-C7 foramen. She underwent a right C6-C7 foraminotomy with a complete macroscopic removal of the lesion. Pathological examination revealed a synovial sarcoma. Treatment was completed by chemotherapy and proton radiotherapy, and the girl remained free of symptoms for 3 years. After appearance of new symptoms, a local recurrence was confirmed, and despite aggressive treatment with salvage chem…

Cervical laminectomy: technique.

Spinal canal decompression via cervical laminectomy with or without foraminotomy is a mainstay of treatment of cervical spondylotic myelopathy and myeloradiculopathy. The goal of this surgery is to expand the cervical canal dorsally by removing the spinous processes, laminae, ligamentum flavum, and bony hypertrophy that are contributing to the canal stenosis. In selecting this particular approach to decompression, the surgeon must take into account the spinal geometry and the primary pathology of the patient: an "effective" cervical kyphosis is a contraindication to a dorsal approach, and spin…

Anterior cervical discectomy with and without interbody bone graft.

Fifty-one patients with symptomatic cervical disc disease refractory to conservative management were allocated at random to one of two treatment groups. The standard anterior approach devised by Cloward was used for 25 patients, and radical discectomy and foraminotomy for the other 26. All patients were followed for 6 months or longer with interview, physical examination, and radiographic evaluation. There was no difference in the success rate between the two groups. The large majority (92%) of patients in both groups were pleased with results of their operation. Because of technical factors r…

[Lumbar lateral recess stenosis. Clinical symptoms, diagnostic modalities and surgical management].

BACKGROUND: In spite of its importance for lumbar radicular syndroms only a few reports about lumbar lateral recess stenosis exist in literature. Most important clinical symptom is radicular pain, which may be exercise-dependent. Confirmation of diagnosis and differentiation from disc herniation requires imaging like CT, MRI or post-myelo-CT. Surgical treatment options include selective decompression techniques like foraminotomy, medial facettectomy and undercutting as well as hemilaminectomy, laminotomy and wide laminectomy or fusion. METHOD: We treated 35 patients with selective decompressi…

Surgical Treatment of Cervical Radiculopathy: Meta-analysis of Randomized Controlled Trials.

STUDY DESIGN: Meta-analysis of published randomized controlled trials (RCTs). OBJECTIVE: To determine whether anterior cervical discectomy and fusion (ACDF), cervical disc replacement (CDR), or minimally invasive posterior cervical foraminotomy (MI-PCF) provides the best outcomes for patients with symptomatic single-level, single-side, and cervical radiculopathy. SUMMARY OF BACKGROUND DATA: The surgical treatment of cervical radiculopathy is still controversial. ACDF has been widely used as a "gold standard." CDR has evolved and become a motion-preserving alternative with a potentially lower…
