The tension-free vaginal tape (TVT) procedure has long been considered the gold standard for female stress incontinence. Random gunfire deposited a bullet in the proximal tibial metaphysis of a 9-year-old girl. The wound was not incapacitating and was treated conservatively. Within 17 months, soreness developed in the proximal leg, and radiography revealed a large unicameral cyst within which the bullet freely tumbled. Eventually, fear of impending fracture prompted further radiography, computed tomography, surgical intervention and pathological examination of the cyst wall. We believe this is only the second description in the English-language literature of this rare sequence of events.

Certain conditions produce a characteristic smell and several diseases are associated with a specific odor. We report an illustrative case and review olfactory physiology, the value of smell in physical examination, and finally discuss several illnesses associated with unusual odors.

Scorched eyelashes and burned eyelids are a common finding in facial burns, though the eye itself is often spared. Affected patients often complain of foreign body sensation and ocular discomfort in the absence of objective physical findings, other than scorched eyelashes and burned eyelash particles in the conjunctival sacs. We report a case of a boy with a long-standing foreign body aspiration in the left main bronchus causing severe pneumonia of the left lung. During intravenous therapy with antibiotics there was a life-threatening event with acute dislocation of the foreign body into the right main bronchus, leading to acute respiratory insufficiency. We conclude that in chronic foreign body aspiration antibiotic therapy preceding bronchoscopy should only be performed in in-patients and with strict monitoring.

We report a case of a man in his 40s who presented with a swollen optic disc and a metallic intraocular foreign body (IOFB) impacted on the retinal surface of the same eye. The IOFB was removed by vitrectomy and the swelling of the optic nerve slowly resolved. Literature search revealed one previous report of a retained IOFB associated with optic disc swelling as part of an ocular siderosis case series. The possibility that the optic disc swelling in our patient was as a result of a local toxic reaction to the metallic foreign body is discussed.

Inflammation-driven foreign body reactions, and the frequently associated encapsulation by fibrogenic fibroblasts, reduce the functionality and longevity of implanted medical devices and materials. Anti-inflammatory drugs, such as dexamethasone, can suppress the foreign body reaction for a few days post-surgery, but lasting drug delivery strategies for long-term implanted materials remain an unmet need. We here establish a thin-coating strategy with novel low molecular weight corticosteroid dimers to suppress foreign body reactions and fibrotic encapsulation of subcutaneous silicone implants.

BACKGROUND: Duraplasty refers to the neurosurgical process of reconstructing dural defect. Variety of materials is used for such reconstruction, including natural, semisynthetic, and synthetic materials.
