STUDY DESIGN: Prospective observational study.
PURPOSE: To assess the safety, efficacy, and benefits of computed tomography (CT)-guided C1 fracture fixation.
OVERVIEW OF LITERATURE: The surgical management of unstable C1 injuries by occipitocervical and atlantoaxial (AA) fusion compromises motion and function. Monosegmental C1 osteosynthesis negates these drawbacks and provides excellent functional outcomes.
METHODS: The patients were positioned in a prone position, and cranial traction was applied using Mayfield tongs to restore the C0-C2 height and obtain a reduction in the displaced frac

Lateral process fractures of the talus may occur either in isolation or in combination with a talar neck or body fracture. Screw fixation has been well described as a means of stabilization; however, many patients have multifragmentary fragments, unable to be controlled by isolated screw fixation alone. Plate fixation of the lateral process has been yet to be described in detail with presence of a clinical series. Here, we describe our technique of plate fixation for both isolated lateral process fractures and those that occur in conjunction with talar neck or body fracture.

Proximal fifth metatarsal fractures, specifically zones 2 and 3, are often treated surgically to lower risk of nonunion and shorten recovery and rehabilitation period. However, even with the advancement of surgical strategies, techniques, and implants, nonunions remain a challenge. One notable risk factor for a primary or recurrent Jones fracture is the cavovarus foot. If this is identified and a recurrent fifth metatarsal base fracture occurs, the surgeon should strongly consider addressing the malalignment in addition to revision open reduction internal fixation. This article provides guidel

OBJECTIVE: The mini-invasive technique, through a sinus tarsi approach (STA), has become an increasingly popular choice for the treatment of calcaneal fractures. However, despite the good results reported, the complications related with internal fixation have not yet been analyzed. Using 3D reconstruction CT scans, we proposed doing biometric description of the calcaneus with the purpose of performing osteosynthesis as treatment for calcaneal fractures.
METHODS: A total of 24 CT scans were retrospectively obtained from 22 consecutive patients with healthy calcanei and no history of surgery or

UNLABELLED: Fractures of the posteromedial process of the talus are frequently the result of high-energy trauma to the lower extremity. The treatment of these uncommon injuries may be unfamiliar and challenging to many surgeons. Significant pain and disability can result if these injuries are not recognized or treated appropriately. Open reduction and internal fixation via a posteromedial approach with screws is a mainstay of operative treatment for simple fractures. In cases of fracture impaction and/or comminution, a medial uniplanar external fixator may be used to improve intraoperative fra

BACKGROUND: Olecranon fractures represent 10% of upper extremity fractures. There is a growing body of literature to support the use of plate fixation for displaced olecranon fractures. The purpose of this survey was to gauge Canadian surgeons' practices and preferences for internal fixation methods for displaced olecranon fractures.
METHODS: Using an online survey tool, we administered a cross-sectional survey to examine current practice for fixation of displaced olecranon fractures.
RESULTS: We received 256 completed surveys for a response rate of 31% (95% confidence interval [CI] 30.5-37.

Pilon or tibial plafond fractures usually result from high-energy injuries with rotation and/or axial compression. They occur in an area of relatively poor soft tissue coverage and frequently present a surgical challenge in deciding which incisions will be best for performing open reduction internal fixation. A variety of anterior and posterior approaches have been described based on the ease of fracture reduction and internal fixation with plates. Some of the incisions are fracture specific, that is, planned for a limited approach to the pilon. But in more complex cases, a wider exposure is i

The aim of this study was to retrospectively determine the risk factors for delayed union in 117 consecutive pediatric both-bone forearm fractures treated with internal fixation. Eight patients (7%, 8/117) had delayed unions, all were boys treated with intramedullary fixation for a fracture in the middle-third of the bone; and in seven patients, the ulna was the site of the delayed union. Older age, double-bone fixation, increased initial fracture displacement, and opening a closed ulna fracture were associated with longer time to union (P<0.05). Identification of risk factors will aid in the

BACKGROUND: Open reduction with internal fixation for unstable ankle fractures is relatively predictable with excellent outcomes. However, the management of ankle fractures in the elderly remains less predictable secondary to the various co-morbidities associated with advanced age.
METHODS: A retrospective chart review of 216 patients over the age of 60 that sustained an ankle fracture, was performed to determine the incidence of complications after ORIF of ankle fractures in an elderly population in the perioperative course. Secondly, the incidence of complications in patients that had locki

Pelvic ring disruption with an associated sacral fracture is typically a result of high-energy injury. Due to significant local soft tissue trauma and the complex neurovascular anatomy in the region, exposure and reduction of sacral fractures has proven to be difficult, hazardous, and fraught with complications. We describe a modified reduction technique to address displacement in sacral fractures to show that it can be safe and effective through evaluation of radiographic, clinical, and functional outcomes. We retrospectively reviewed the radiographic and clinical results of consecutive patie

BACKGROUND: Ankle fractures are among the most common injuries treated by orthopaedic surgeons. The purpose of the present investigation was to examine the risks of complications after open reduction and internal fixation of ankle fractures in a large population-based study.
METHODS: With use of California's discharge database, we identified 57,183 patients who had undergone open reduction and internal fixation of a lateral malleolar, bimalleolar, or trimalleolar ankle fracture as inpatients in the years 1995 through 2005. Short-term complications were examined on the basis of the rates of re

We describe our experience using a new device that results in a bloodless field in open repair of distal radius fractures. The device, an exsanguinating tourniquet (HemaClear model/40, OHK Medical Devices, Haifa, Israel), replaces the traditional methods of limb elevation, Esmarch bandaging, pneumatic tourniquet pressurizing and the associated components. HemaClear/40 is an elastic silicon ring with a tubular elastic sleeve rolled onto it. The device has attached straps that, when pulled, unroll the sleeve, rolling the ring mesially on the limb. The pressure exerted by rolling HemaClear/40 is
