Free-hand versus novel specialised jig guidance for the passing of intramedullary wires in olecranon fracture fixation: a comparative study.

The treatment of olecranon fractures frequently involves the use of tension-band fixation. Although associated with high union rates, this method has a high incidence of morbidity associated with soft tissue compromise and limitation of range of movement requiring frequent re-operation for removal of metal. We describe the use of a simple jig to ensure intramedullary placement of longitudinal K-wires and compare the accuracy of placement of Kirschner (K)-wires using this device with the traditional free-hand method. We found the distance from the centre of the medullary canal, the range and st

Retrograde Intramedullary Nailing Versus Locking Plate Fixation in Extra-Articular Distal Femur Fractures: A Prospective Study.

INTRODUCTION: Distal femur fractures require stable fixation that permits restoration of alignment and early functional recovery. Retrograde intramedullary nailing distal femur nailing (DFN) and distal femoral locking plate fixation locking compression plate (LCP) are commonly used for extra-articular fractures, but their comparative functional and radiological outcomes remain debated. AIM: This study compared operative, functional, radiological, and complication outcomes of DFN and LCP in adult patients with extra-articular distal femur fractures. MATERIALS AND METHODS: This prospective stu

Bilateral Mandibular Fracture Repair with an External Skeletal Fixator Intramedullary Pin Tie-in in a Mallard Duck (Anas platyrhynchos).

An approximately 5-year-old male mallard duck (Anas platyrhynchos) was presented for assessment and treatment of bilateral mandibular ramal fractures. Physical examination revealed the presence of complete, open mandibular fractures at the rostral third of both mandibles, with ventral displacement of the rostral segments. An initial repair attempt consisted of a suture passed between the proximal and rostral fragments to form a simple interrupted suture on each side, which failed soon after the procedure. The second repair used a 1.1-mm nonthreaded Steinman pin inserted through the fracture si

Outcomes of Metacarpal Shaft Fractures Treated by Open Reduction Internal Fixation With Plates and Screws Versus Intramedullary Screws.

BACKGROUND: Metacarpal fractures are among the most common orthopedic injuries. When metacarpal shaft fractures require surgical fixation, common options include closed reduction percutaneous pinning, open reduction internal fixation (ORIF) using plates and screws (PS), and intramedullary screw fixation (IMS). Certain metacarpal shaft fractures are amenable to ORIF with either PS or IMS. The purpose of this study is to compare the functional outcomes and complication rates between these techniques. METHODS: A retrospective review was performed of all patients that underwent ORIF with PS or IM

Comparison of elastic stable intramedullary nailing versus retrograde screw fixation for pubic ramus fractures-a biomechanical study.

BACKGROUND: Percutaneous minimally invasive retrograde screw fixation is a widely accepted treatment strategy for patients with superior ramus fractures and has shown good biomechanical stability compared to plating. Recently, elastic stable intramedullary nailing (ESIN) devices have been proposed as an alternative in the treatment of superior ramus fractures. However, biomechanical studies on this new treatment are lacking. Thus, the aim of this study was to compare the biomechanical stability of ESIN in pubic ramus fractures versus retrograde screw fixation. METHODS: Standardized pubic ramu

Retrograde Intramedullary Nailing vs. Lateral Locking Plate Fixation for Distal Femur Fractures: Which Technique is Superior?

OBJECTIVE: Distal femur fractures are challenging to treat, especially in elderly osteoporotic patients. Retrograde intramedullary nailing (RIMN) and lateral locking plate (LP) fixation are commonly used surgical methods for these fractures. This study aimed to compare the clinical and radiological outcomes of these two techniques. METHODS: In this retrospective study, a total of 72 patients with distal femur fractures classified according to the AO/OTA system and treated surgically were included. Patients were divided into two groups: RIMN (n=36) and LP (n=36). Demographic data, fracture cla

Clinical outcomes of percutaneous intramedullary headless screw fixation with an oblique technique for the treatment of proximal and middle phalanx fractures.

BACKGROUND: Intramedullary headless screw fixation has become significant in the treatment of phalangeal fractures. While intramedullary headless screw fixation is a simple and rigid approach, common techniques for guide wire insertion, including retrograde, antegrade-intra-articular, and trans-articular techniques, damage the articular cartilage and extensor tendon, particularly the central slip. To mitigate these risks, we applied a new method by percutaneously inserting the screw obliquely from the radial or ulnar base of the proximal or middle phalanges. This study aimed to assess the clin

Intramedullary Nailing vs. Plate Fixation for Trochanteric Femoral Fractures: A Systematic Review and Meta-Analysis of Randomized Trials.

Background/Objectives: Trochanteric femoral fractures pose significant surgical challenges, particularly in elderly patients. Intramedullary nailing (IMN) and plate fixation (PF) are the primary operative strategies, yet their comparative efficacy and safety remain debated. This meta-analysis synthesizes randomized controlled trials (RCTs) to evaluate clinical, functional, perioperative, and biomechanical outcomes of IMN versus PF specifically in trochanteric fractures. Methods: A systematic search of six databases was conducted up to 20 May 2024, to identify RCTs comparing IMN and PF in adult

Comparison of intra-operative outcomes following internal fixation with trochanteric stabilisation plate or intramedullary nail in intertrochanteric fractures.

PURPOSE: Intertrochanteric fractures can be classified as stable (AO-OTA 31-A1) or unstable (AO-OTA 31-A2/3). For A3 fractures there is no recommended treatment, often fixed with either an intramedullary nail (IMN) or a dynamic hip screw and trochanteric stabilisation plate (DHS/TSP). This study retrospectively reviews peri-operative outcomes of patients treated with either fixator. METHODS: Pre-operative demographics, operative information and patient outcome data from 213 patients who suffered intertrochanteric fractures and were treated with either DHS/TSP or IMN at a major trauma centre f

Comparing external fixators and intramedullary nailing for treating open tibia fractures: a meta-analysis of randomized controlled trials.

BACKGROUND: External fixators (EFs) and intramedullary nailing (IMN) are two effective methods for open tibial fractures. However, both methods have advantages and disadvantages, and the optimal surgical approach remains controversial. Thus, we performed a meta-analysis of randomized controlled trials (RCTs) to compare EF with IMN to evaluate their efficacy and safety. METHODS: A systematic study of the literature was conducted in relevant studies published in PubMed, Embase, the Cochrane Library, Web of Science, CNKI, CBM, Wanfang and Weipu from database inception to April 2022. All eligible

Intramedullary nailing versus external ring fixator for treatment of tibial fractures - a study protocol for a randomised clinical trial.

INTRODUCTION: Tibial shaft fractures are among the most common lower extremity fractures. Treatment of tibial shaft fractures with intramedullary nailing has become the treatment of choice in adults. However, commonly reported outcomes include knee pain, limitations in activities of daily living and reduction in quality of life (QOL). The literature lacks high-quality studies to document superiority of intramedullary nailing versus other surgical treatment methods. The present study aims to compare the 12-month Knee Injury and Osteoarthritis Outcome Score (KOOS) - sport and recreation activiti
