Classification and Outcome of Fracture-Dislocation of the Cuneiform Bones.

Fractures and dislocations of the cuneiform bones are rare injuries to the midtarsal foot. The injury severity is often unclear, and the prognostic factors are unknown. The purpose of the present study was to characterize our insights of the diagnostics, therapy, and fracture patterns. We questioned whether the number of involved cuneiform bones and the type of injury would affect the clinical outcome. With this information, we aimed to develop a classification system for injuries of the cuneonavicular joint. Five patients who had sustained complex fracture-dislocation of the cuneiform bones w

Navigation-Assisted Bone Grafting for Blowout Fracture.

BACKGROUND: Treatment of orbital floor fracture accompanied by extensive bone defect requires orbital floor reconstruction using a bone graft, but graft may deviate into the maxillary sinus when no bone capable of supporting the graft remains around the defect. In such cases, it is necessary to fix the grafted bone to the orbital margin, but the grafted bone placement site is subjectively decided based on inspection through a small incision in many cases, being dependent on the experience of operators, and it has been difficult to accurately determine the placement angle. METHODS: The authors

Biological Risk Factors for Nonunion of Bone Fracture.

Many risk factors for fracture nonunion are well supported in the orthopaedic literature, including location of the fracture site, surgical treatment, bone displacement, type of fixation, treatment delay, comminution, inadequate treatment, and wound infection. However, evidence from a systematic review suggests that patient-related or biological causes of nonunion may not be as well understood. Understanding the biological causes of nonunion is important for several reasons. Risk factors might identify patients prone to nonunion who could benefit from more aggressive intervention, and a clear

Epidemiology of Fracture Nonunion in 18 Human Bones.

IMPORTANCE: Failure of bone fracture healing occurs in 5% to 10% of all patients. Nonunion risk is associated with the severity of injury and with the surgical treatment technique, yet progression to nonunion is not fully explained by these risk factors. OBJECTIVE: To test a hypothesis that fracture characteristics and patient-related risk factors assessable by the clinician at patient presentation can indicate the probability of fracture nonunion. DESIGN, SETTING, AND PARTICIPANTS: An inception cohort study in a large payer database of patients with fracture in the United States was conduct

Vitamin D, bone metabolism and fracture risk.

Insufficient serum levels of 25OH vitamin D (25OHD) is a risk factor for osteoporosis. A new paradigm has emerged with the locally synthesized 1,25(OH)2D within osteoblasts and osteoclasts as the essential pathway for the effects of 25OHD in regulating bone remodeling via direct or indirect activation of the specific receptor VDR. Vitamin D has positive effects on fracture risk but these results have been consistently observed whenever daily doses were above 800 UI/d administered to compliant patients together with adequate calcium supplementation and with an achieved biological target of seru

Surgical treatment of severe frontal bone fracture.

Craniofacial trauma can lead to several complications. The combined fractures of anterior and posterior walls of the frontal bone are almost always followed by lesions in nasofrontal orifices and disruption of nasofrontal ostia or ducts, a significant factor for the development of early and late complications after sinus fractures. This article reports a case of trauma patient, who underwent neurological evaluation and at first showed good general condition. Computed tomography noted fracture of the anterior and posterior walls of the frontal sinus and small foci of pneumocephalus in the cereb

Nasal bone fracture--ultrasonography or computed tomography?

AIM: The aim of the study is to estimate the diagnostic sensitivity and utility of high-resolution ultrasonography (HRUS) in comparison with a gold standard technique in the diagnosis of the nasal fracture. PATIENTS AND METHOD: Using a retrospective study design, facial Computed Tomography (CT) and HRUS examinations were performed on 87 consecutive patients with nasal trauma. Ultrasonograms were obtained with a high frequency linear transducer (10 MHz). All patients also underwent facial conventional radiography (CR). The sensitivity and specificity for diagnosis of nasal fracture was evaluat

Pathologic fracture of a calcaneal aneurysmal bone cyst.

A 21-year-old man presented with a pathologic fracture through the posterior aspect of the calcaneus into an aneurysmal bone cyst. The patient was treated using curettage, phenol, alcohol, and burr with open reduction and internal fixation. This is the first reported case of a pathologic fracture of an aneurysmal bone cyst of the calcaneus, highlighting the fracture potential of these lesions and the need for early management.

Isolated bone infarct of the calcaneus after fracture.

The calcaneus has a rich vascular supply; therefore, avascular necrosis of the calcaneus is extremely rare. We report the first case of bone infarct of the calcaneus 9 months after a fracture. We also review the literature on osteonecrosis of the calcaneus to offer potential mechanisms for bone infarction in the calcaneus after a fracture.

Pathologic fractures of long bones in Nigerian children.

BACKGROUND: Pathologic or spontaneous fractures are recognised causes of morbidity in children. There is a paucity of data on their aetiology and patterns in African children. METHODS: A combined retrospective and prospective study of 47 children aged 15 and below with pathologic fractures of long bones seen from January 2000 to December 2006. The setting was the Orthopaedic and Traumatology department of the Obafemi Awolowo University Teaching Hospitals Complex, Ile-Ife, Nigeria. RESULTS: A total of 47 patients with a mean age of 5.3 years +/- SD with pathologic fractures of 50 long bones w

Bone density, geometry, and fracture in elderly men.

Bone geometry is a major determinant of the mechanical resistance of bone. Mechanical strength of the vertebrae depends on the cross-sectional area of the vertebral body and on the size of the posterior arch. Smaller bone width is associated with higher risk of stress fracture. A longer femoral neck axis and a more open neck-shaft angle are associated with higher risk of cervical hip fracture. No consistent association between the femoral neck width and the cervical fracture risk was found. Areal bone mineral density (aBMD) is not a good tool for the identification of elderly men with high fra

Limb lymph node response to bone fracture.

In previous clinical studies, dilation of afferent lymphatics and enlargement of inguinal lymph nodes (LN) were observed in lymphoscintigrams from patients with persistent posttraumatic edema of lower extremities after fractures and trauma of soft tissues. In this study, changes in rat popliteal and iliac lymph nodes draining lymph from the site of tibial fracture and adjacent soft tissue injury were investigated. The observed parameters were lymph node weight, cell number, phenotype frequency, cell cytokine expression, and reactivity to mitogens. The key observations included: a) increase in
