INTRODUCTION: Intraoperative technical complications are occasionally encountered while implanting INTERTAN™ nails for intertrochanteric fractures. Surgeons need to pay attention to the difficulties they may encounter during the implantation of an INTERTAN™ nail.
CASE PRESENTATION: We report four cases with intraoperative difficulties during the implantation of INTERTAN™ nails among Han Chinese patients from mainland China. In Case 1, during the operation on a 75-year-old woman, an anatomical specificity of excessive femoral shaft curvation at the coronal and sagittal planes was observed; a r

Bilateral zygomatic arch fractures are uncommon but may result in significant midfacial deformity and functional impairment, particularly when fractures are comminuted and structural continuity is compromised. This report describes the surgical management of a 49-year-old male who sustained severe facial trauma following a mountain-climbing accident, resulting in comminuted bilateral zygomatic arch fractures and marked midfacial disfigurement. Due to the complexity of the injury and loss of structural support, autogenous costal cartilage grafting combined with titanium mesh fixation was select

BACKGROUND: Using modern implants, even complex radial head and neck fractures can often be reconstructed. However, metaphyseal comminution is associated with delayed bone healing and an increased risk for loss of reduction. Hence, this biomechanical in-vitro study intended to evaluate the mechanical stability of a bone graft from the ipsilateral proximal ulna in plate fixation of comminuted radial neck fractures.
METHODS: Osteotomies at the level of the radial neck with a 3 mm defect were created on 20 fresh-frozen proximal radius specimens to simulate metaphyseal comminution. Fixation was p

BACKGROUND: A coronal comminuted femoral intertrochanteric fracture is a special type of fracture that easily leads to internal fixation failure, and the current internal fixation techniques remain controversial. This study aims to evaluate the effect of traction-bed-assisted reduction and double-plate internal fixation in the treatment of comminuted and coronally split intertrochanteric femoral fracture.
METHOD: Retrospective analyses of the clinical data of 83 patients diagnosed with, and treated for, comminuted and coronally split intertrochanteric femoral fracture from December 2017 to No

Open and comminuted mid-distal fractures often result from high-energy trauma, and a concomitant poor blood supply often leads to skin necrosis, infection, and bone union. To circumvent such complications, we used limited-reduction and bilateral-external fixators to treat open and comminuted mid-distal tibial fractures with compromised soft tissue. A retrospective series of 34 patients who had open and comminuted mid-distal tibial fractures and treated by bilateral-external fixators with limited-internal fixation were analyzed. Patients were followed for 10-25 months (mean: 12 months) post-tre

INTRODUCTION: Talus fractures are uncommon, accounting for <1% of all fractures, but they are clinically significant due to their high propensity for complications, such as avascular necrosis, post-traumatic arthritis, and non-union. The talus has a complex anatomy and a tenuous blood supply, making it particularly vulnerable to healing complications when not managed appropriately. Non-union of talus fractures is rare and often results from inadequate initial treatment, especially in displaced fractures that are managed conservatively.
CASE REPORT: We report the case of a 34-year-old male who

Acute popliteal artery thrombosis is a rare and limb-threatening traumatic event. It can lead to soft-tissue necrosis or even amputation of the lower limb. In patients suffering from trauma around the knee joint, the clinician should be highly vigilant to risk of popliteal artery acute thrombosis. It is essential to detect and treat acute popliteal artery thrombosis timely for limb salvaging. We present a case of acute popliteal artery thrombosis resulting from a closed comminuted tibia fracture and discuss its injury mechanism, diagnostic methods, and treatment measures.

PURPOSE: Comminuted olecranon fractures are commonly fixed with posterior locking plates (PLPs). Though biomechanically validated, this method comes with risks of symptomatic implant prominence and wound breakdown. Dual locking plates (DLPs) placed on the medial and lateral surfaces of the olecranon theoretically avoid these risks and allow for fixation of proximal fracture fragments in multiple planes. A biomechanical comparison of DLP and PLP fixation would help to validate the use of DLPs in comminuted olecranon fractures.
METHODS: Seven matched pairs of cadaveric upper extremities were ev

BACKGROUND: Isolated depressed zygomatic arch fractures are often treated with closed reduction. Reduction is usually performed through the Gillies approach or Keen approach. comminuted zygomatic arch fractures generally require open reduction and fixation to achieve good results. This article describes how to use a pair of surgical templates to assist in the fixation of comminuted zygomatic arch fractures with absorbable plates.
METHODS: A pair of computer-designed surgical templates were applied to restore the main part of zygomatic arch. Placing a surgical template on the medial side of th

Complete reconstruction of the hamate bone has been reported in the literature mostly following cancer excision or avascular necrosis. For the exiguity of the tissue deficit, bone grafting has usually been used as treatment option for its rapidity and easiness to perform, even if a variable amount of bone resorption may occur. In traumatic cases, microbial contamination may jeopardize the success of a well performed bone graft and vascularised bone grafts may represent a better reconstructive option. Here we describe the first case reported in the literature of a patient underwent complete ham
