Polygenicity shapes distinct brain patterns and clinical profiles of bipolar disorder.

BACKGROUND: Bipolar disorder (BD) manifests both genetic predispositions and brain imaging abnormalities. Genetic analyses provide a powerful approach to disentangling the associations between BD and brain imaging-derived phenotype (IDP). However, in East Asian (EAS) populations, the genetic basis of these neuroimaging changes and their clinical relevance remains underexplored due to limited integrative evidence. METHODS: We investigated the genetic architecture and clinical relevance of BD and brain IDPs in East Asian populations using individual-level data from the Chinese Bipolar Disorder

Immune and inflammatory radiogenomics of radiotherapy-induced normal-tissue toxicity: genetic susceptibility, mechanisms, risk prediction, and clinical translation.

Radiotherapy is indispensable for cancer control, yet acute and late normal-tissue toxicities remain major limits to treatment intensity and survivorship. Considerable interindividual variation persists after clinical and dosimetric factors are considered, motivating radiogenomic studies of inherited susceptibility. This review compares common and rare germline variation, association and sequencing designs, polygenic risk scores, and functional evidence through an immunological framework. Radiation-induced DNA damage and organelle stress release danger-associated signals and cytosolic DNA that

Sex-specific early cognitive changes are linked to global and pathway-specific genetic risk for Alzheimer's disease in at-risk individuals.

Alzheimer's disease (AD) is a progressive neurodegenerative condition in which genetic predisposition plays a key role, yet the sex-specific mechanisms linking genetic risk to early cognitive changes remain unclear. This study examined the impact of polygenic risk scores (PRS) on early cognitive changes in 318 cognitively unimpaired participants from the ALFA+ cohort, a nested longitudinal cohort from the ALFA study (see details in Study Participants Section, Methods). Participants were followed for three years, with assessments across five cognitive domains and a preclinical composite (PACC).

Genetic Candidate Variants in Two Multigenerational Families with Childhood Apraxia of Speech.

Childhood apraxia of speech (CAS) is a severe and socially debilitating form of speech sound disorder with suspected genetic involvement, but the genetic etiology is not yet well understood. Very few known or putative causal genes have been identified to date, e.g., FOXP2 and BCL11A. Building a knowledge base of the genetic etiology of CAS will make it possible to identify infants at genetic risk and motivate the development of effective very early intervention programs. We investigated the genetic etiology of CAS in two large multigenerational families with familial CAS. Complementary genomic

Genetic determinants of blood pressure traits are associated with carotid arterial thickening and plaque formation in patients with type 2 diabetes.

OBJECTIVES: The aim of this study is to explore the contribution of genetically driven cardiometabolic risk factors for development of carotid arterial thickening in patients with type 2 diabetes. METHODS: In total, 12 genetic risk scores for blood pressure, blood lipids and glycaemic traits were constructed. The genetic risk scores were tested for association with carotid intima-media thickness and plaques in patients with type 2 diabetes ( n = 401) and in non-diabetic individuals ( n = 648) and for association with glucose levels in two population-based cohorts ( n = 1328 and n = 6161). Rare and Common Genetic Variation Underlying the Risk of Hypertrophic Cardiomyopathy in a National Biobank.

IMPORTANCE: Hypertrophic cardiomyopathy (HCM) is a leading cause of sudden cardiac death in young people. Although rare genetic variants are well-established contributors to HCM risk, common genetic variants have recently been implicated in disease pathogenesis. OBJECTIVE: To assess the contributions of rare and common genetic variation to risk of HCM in the general population. DESIGN, SETTING, AND PARTICIPANTS: This cohort study of the UK Biobank (data from 2006-2010) and the Mass General Brigham Biobank (2010-2019) assessed the relative and joint contributions of rare genetic variants and

Development of a predictive model for pancreatic cancer integrating serum metabolomics and polygenic risk score.

BACKGROUND: Pancreatic cancer (PC) is highly aggressive and associated with extremely poor prognosis. Due to the lack of specfic symptoms in the early stage, most patients have already lost the opportunity for curative surgical resection at the time of diagnosis. Therefore, improving the early identification of individuals at elevated risk of PC is clinically important. METHODS: Using data from the UK Biobank, 157,235 participants were included. Cox proportional hazards models were applied to systematically evaluate the associations of clinical biomarkers, polygenic risk scores (PRS), and 143

Association of a polygenic risk score with diagnosis and outcomes in idiopathic pulmonary fibrosis.

RATIONALE: Idiopathic pulmonary fibrosis (IPF) is characterized by chronic progressive pulmonary fibrosis and high mortality. Genetic markers, summarized into a polygenic risk score (PRS), associate with IPF in well-phenotyped research cohorts. OBJECTIVES: To evaluate the performance of the PRS using real-world data from routinely captured electronic health care records. METHODS: We conducted an observational study evaluating the association of a PRS for IPF with electronic health care record diagnosis of IPF as well as lung transplant-free survival in 4 independent cohorts: the Mass General

Polygenic resilience scores capture protective genetic effects for Alzheimer's disease.

Polygenic risk scores (PRSs) can boost risk prediction in late-onset Alzheimer's disease (LOAD) beyond apolipoprotein E (APOE) but have not been leveraged to identify genetic resilience factors. Here, we sought to identify resilience-conferring common genetic variants in (1) unaffected individuals having high PRSs for LOAD, and (2) unaffected APOE-ε4 carriers also having high PRSs for LOAD. We used genome-wide association study (GWAS) to contrast "resilient" unaffected individuals at the highest genetic risk for LOAD with LOAD cases at comparable risk. From GWAS results, we constructed polygen

Genetic variation at the CYP2C19 gene associated with metabolic syndrome susceptibility in a South Portuguese population: results from the pilot study of the European Health Examination Survey in Portugal.

BACKGROUND: Metabolic syndrome (MetS) is a cluster of conditions that occur together, increasing the risk of heart disease, stroke and diabetes. Since pathways implicated in different diseases reveal surprising insights into shared genetic bases underlying apparently unrelated traits, we hypothesize that there are common genetic components involved in the clustering of MetS traits. With the aim of identifying these common genetic components, we have performed a genetic association study by integrating MetS traits in a continuous MetS score. METHODS: A cross-sectional study developed in the co

Genetics of response to cognitive behavior therapy in adults with major depression: a preliminary report.

Major depressive disorder is heritable and a leading cause of disability. Cognitive behavior therapy is an effective treatment for major depression. By quantifying genetic risk scores based on common genetic variants, the aim of this report was to explore the utility of psychiatric and cognitive trait genetic risk scores, for predicting the response of 894 adults with major depressive disorder to cognitive behavior therapy. The participants were recruited in a psychiatric setting, and the primary outcome score was measured using the Montgomery Åsberg Depression Rating Scale-Self Rated. Single-…

Polygenic Score for the Prediction of Postoperative Nausea and Vomiting: A Retrospective Derivation and Validation Cohort Study.

BACKGROUND: Postoperative nausea and vomiting (PONV) is a key driver of unplanned admission and patient satisfaction after surgery. Because traditional risk factors do not completely explain variability in risk, this study hypothesized that genetics may contribute to the overall risk for this complication. The objective of this research is to perform a genome-wide association study of PONV, derive a polygenic risk score for PONV, assess associations between the risk score and PONV in a validation cohort, and compare any genetic contributions to known clinical risks for PONV. METHODS: Surgerie

Genetic Susceptibility, Diet Quality, and Two-Step Progression in Drusen Size.

PURPOSE: To investigate the relationship of growth in drusen size with genetic susceptibility and adherence to the alternate Mediterranean diet. METHODS: Participants in this analysis had complete ocular, genetic, and dietary data with mean follow-up time of 10.2 years in the Age-Related Eye Disease database. Maximal drusen size was graded on an ordinal scale and two-step progression was determined. A genetic risk score using variants associated with advanced AMD and derived from a stepwise regression model yielded 11 variants in 8 genes. Adherence to the alternate Mediterranean diet was asse…

Associations of plasma proteomic and polygenic profiling with incident psoriasis risk: a prospective cohort study.

BACKGROUND: Psoriasis is a common chronic immune-mediated disease with substantial systemic and public health burden, yet early molecular signatures associated with its onset remain insufficiently characterised. Although both circulating proteins and genetic susceptibility may contribute to psoriasis development, their prospective and joint associations with incident psoriasis remain unclear. We aimed to evaluate the individual and combined effects of proteomic and polygenic profiling on psoriasis risk. METHODS: We identified psoriasis-related protein signature using plasma proteomic data (29

Association between polygenic risk and survival in breast cancer patients.

BACKGROUND: Polygenic risk scores (PRS) estimate an individual’s germline genetic predisposition to a quantitative trait and/or risk of disease. Several PRS have been developed for cancer risk with the goal of improved risk screening. Here, we sought to establish whether PRS for cancer risk and other common traits may influence survival for patients with cancer. METHODS: We conducted a PRS survival analysis using 23,770 cancer patients of European ancestry from the Dana-Farber Cancer Institute Profile cohort. RESULTS: We identified an association between PRS for breast cancer risk and longer p

Polygenic risk scores for cardiometabolic traits demonstrate importance of ancestry for predictive precision medicine.

Polygenic risk scores (PRS) have predominantly been derived from genome-wide association studies (GWAS) conducted in European ancestry (EUR) individuals. In this study, we present an in-depth evaluation of PRS based on multi-ancestry GWAS for five cardiometabolic phenotypes in the Penn Medicine BioBank (PMBB) followed by a phenome-wide association study (PheWAS). We examine the PRS performance across all individuals and separately in African ancestry (AFR) and EUR ancestry groups. For AFR individuals, PRS derived using the multi-ancestry LD panel showed a higher effect size for four out of fiv

Plasma Lipidome Is Dysregulated in Amyotrophic Lateral Sclerosis and Associated With Life-Course Adiposity Polygenic Risk.

OBJECTIVE: Although both pre-symptomatic adiposity and dysregulated lipid metabolism are implicated in amyotrophic lateral sclerosis (ALS), the mechanistic pathways linking adiposity to disease remain unclear. This study aimed to investigate how genetic predisposition to adiposity might influence ALS through specific lipid metabolic pathways. METHODS: We conducted mass spectrometry-based quantitative lipidomics on plasma from 104 ALS patients and 78 age- and sex-matched controls. Composite Summary Scores Capture Obesogenic Eating Risk Among Preadolescents Using the Child Eating Behaviour Questionnaire.

BACKGROUND: The Child Eating Behaviour Questionnaire (CEBQ) measures multiple appetitive traits. We used a data-driven approach to identify simplified composite scores across traits to summarise obesogenic eating risk. METHODS: From 354 unrelated 9-to-12-year-olds, we examined 29 items from CEBQ domains measuring food approach or avoidance: food responsiveness, enjoyment of food, emotional over-eating, desire to drink, satiety responsiveness, slowness in eating, and emotional under-eating. Factor analysis assuming a single factor was used to reduce the number of items in one composite, obesog

Digestive System Diseases, Genetic Risk, and Incident Dementia: A Prospective Cohort Study.

INTRODUCTION: Although digestive system disease affects gut microbiota and their metabolites associated with dementia risk, the association between digestive system diseases and incident dementia has not yet been established. METHODS: This cohort analysis included 458,181 participants free of baseline dementia in the UK Biobank (2006-2021). The associations of 14 digestive system diseases with dementia incidence were examined in 2022 using Cox proportional hazards regression models. Analyses were performed to differentiate the associations for early-onset (age <65 years) and late-onset (age ≥
