A 43-year-old male received intravenous propofol, atracurium and gentamicin and experienced cardiovascular collapse with the features of an acute drug reaction. Prolonged cardiopulmonary resuscitation with fluids and large amounts of adrenaline and noradrenaline led to full recovery without neurological deficit. No biochemical abnormality was detected.

OBJECTIVE: To evaluate the pharmacokinetics and cost of once-daily dosing with gentamicin in women with postpartum endometritis.
METHODS: Gentamicin in a single daily dose of 4.5 mg/kg was administered intravenously to 10 women with postpartum endometritis. Peak and trough gentamicin levels were measured, and nephrotoxicity and clinical ototoxicity were monitored. Pharmacokinetic data were analyzed, and a cost analysis of once-daily gentamicin administration was performed.
RESULTS: The mean elimination constant was 0.105 +/- 0.008 L/h, and the mean volume of distribution was 0.34 +/- 0.07 L/

During April 30-July 26, 1998, 20 patients at a major medical center (hospital A) in Los Angeles County, California, developed severe shaking chills often accompanied by fever, tachycardia, and/or a decrease of > or =20 mm Hg in systolic blood pressure within 3 hours after receiving intravenous (IV) gentamicin. The common preventive therapy in our department for newborns' eyes is tetracycline ophthalmic ointment. Due to temporary shortage of tetracycline ophthalmic ointment for one week in December 1991, the preventive therapy given to seventy-one newborns was gentamicin sulfate ophthalmic ointment. This analysis of gentamicin pharmacokinetics involved 15 neonates supported by extracorporeal membrane oxygenation (ECMO). Two sets of blood samples were obtained from each neonate while supported by ECMO. Severe unilateral cases of Menière's disease can be successfully treated with ototoxic antibiotics. Among them gentamicin sulfate gives the safest results. With the aid of a small syringe this antibiotic is introduced into the external auditory canal 5 times/day. From there it reaches the middle ear through a ventilating tube by using a Politzer bag. Gentamicin then penetrates the round and oval windows, where it influences inner ear function. Of 92 patients treated during the past 20 years, more than 90% have had no further attacks after therapy.

Gentamicin ototoxicity and nephrotoxicity were compared in two strains of rats, Sprague-Dawley and Fisher-344, and in the Hartley albino guinea pig.
