Pediatric obstructive sleep apnea syndrome (OSAS) is primarily caused by adenotonsillar hypertrophy. However, tongue base hypertrophy is increasingly being recognized as a cause, even after adenotonsillectomy. We report three cases of pediatric OSAS successfully treated by transoral robotic reduction of the tongue base. In all children, we were able to achieve improved retrolingual patency while avoiding significant procedure-related morbidity. In conclusion, tongue base reduction by transoral robotic surgery appears to be a feasible solution for the base of tongue obstruction due to lingual t

Macroglossia may occur as a congenital or acquired condition. The enlarged tongue has both functional and cosmetic deformity, which may affect the oral airway, speech, and the development of the jaws. We discuss the various tongue-reduction procedures and present cases to illustrate the uses of this procedure, its potential complications, and the results.

BACKGROUND: Schwannoma, in the English literature mostly called neurilemmoma, are well known in the ear-nose-and-throat-medicine as benign tumors of the nerve-sheath, for example as neuroma of the acoustic nerve. They are seldom found at other peripheral nerves of the head. The localisation in the tongue has so far only been described in some individual cases.
CASE REPORT: We present a case of a 28-year-old woman with a schwannoma of the tongue, measuring 4.0 x 2.2 x 3.2 cm. The case history and the radiological investigations suggested a malignant tumor. During the operation the lump reveale

Chylous fistula is a distressing complication resulting from thoracic duct injury during neck dissections. We have successfully managed chylous fistula using negative-pressure wound therapy (NPWT) in a case where all conservative treatments failed. A 60-year-old man with tongue cancer underwent subtotal glossectomy and bilateral neck dissections. On postoperative day 4, a chylous fistula with large drainage developed in the right neck. Conservative treatments were not effective, therefore, NPWT was started from postoperative day 9. The drainage volume then began to decrease, and the chylous fi…

Two patients with cardiac metastasis from head and neck cancer are reported. Cardiac metastasis located in the left atrium was detected on a follow-up computed tomography (CT) scan 15 months after partial glossectomy for a tongue carcinoma in a 60-year-old man. The diagnosis was confirmed as cardiac metastasis of squamous cell carcinoma (SCC) by surgical excision of the cardiac lesion. The patient died 3 weeks after surgery. In a 69-year-old man with a partial maxillectomy for primary soft palate cancer, a follow-up CT scan 5 months after surgery revealed a mass in the right atrium and ventric…

METHODS: A combination of orthodontics and a partial glossectomy was necessary to successfully address the open bite associated with an enlarged tongue.
RESULTS: The need for orthognathic surgery treatment was eliminated and the patient was satisfied with the post-treatment aesthetics, function and speech.

OBJECTIVES: A case of congenital macroglossia is reported. The most important sign of macroglossia is tongue protrusion through the lips. Tongue protrusion might influence skeletal growth and can cause anterior open bite, proclination of upper and lower incisors and development of diastemas.
MATERIALS AND METHODS: A 4 year-old female patient was diagnosed with congenital macroglossia. Parents referred an abnormal tongue dimension since birth and the development of a progressive anterior open bite. The treatment of macroglossia included tongue reduction by partial glossectomy. She was seen reg…

INTRODUCTION: The tongue is the most common site of oral malignancies, with surgical resection the mainstay of treatment. Tongue cancer resections have significant functional impacts. Ideal reconstruction would restore speech intelligibility, facilitate swallowing of liquids and solids without significant dysphagia, and have minimal morbidity. We aimed to review and synthesize published outcomes of tongue reconstructive methods into a useful clinical treatment algorithm.
METHODS: A systematic electronic search was conducted in accordance with the Preferred Reporting Items for Systematic Revie

A protuberant shape and sufficient volume are the most important parameters for total tongue reconstruction. The conventional pectoralis major myocutaneous (PMMC) flap undergoes collapse due to atrophy of the denervated muscle. In a new technique, this flap was rolled up like sushi to reshape the neotongue. This study explored the feasibility and effect of the 'sushi roll' technique for precise total functional reconstruction of the tongue using a PMMC flap. Thirty patients scheduled for total glossectomy and PMMC flap reconstruction were recruited. The sushi roll technique was performed in 15…

INTRODUCTION: Macroglossia is a term used to describe a large tongue which protrudes outside of the mouth while in a resting position (Balaji, 2013). It is a cardinal sign in children with Beckwith-Wiedemann syndrome and can also be found in children with Down syndrome and Klippel-Trenaunay-Weber syndrome. Macroglossia can lead to airway and feeding difficulties, as well as problems with speech, drooling, and cosmesis. We present a review of tongue reduction operations performed for macroglossia over a 10-year period in Northern Ireland.
METHODS: We performed a retrospective review of the med

INTRODUCTION: Free flap reconstructive surgery of middle size and large oral and facial defects enables aesthetic and functional rehabilitation. Microvascular flap reconstructive surgery, with flap survival success rate more than 90-98%, is a gold standard in head and neck extensive reconstructions. Currently, head and neck reconstructive surgery is focused not only on defect occlusion and adequate aesthetical result, but the same emphasize is aimed at functional result. Functional result post tongue and lip resection means defect occlusion with the possibility of movement restoration. Free gr

Objective:The aim of this study is to investigate the effect of modified uvulopalatopharyngoplasty (UPPP) combined with endoscopic hypothermic Plasma glossectomy (Eco-TBR) on the severe Obstructive sleep apnea syndrome. Method:Sixty patients with severe OSAHS were diagnosed by polysomnography, and their obstructive plane located in the oropharynx and tongue base. Patients were divided into the control group (30 cases of simple H-UPPP) and the experimental group (30 cases of H-UPPP combined with Eco-TBR) according to their random hospital sequence. SPSS 20.0 software package was used to analyze…

INTRODUCTION: The suprasternal space, termed Burns space, lies between the sternocleidomastoid and sternohyoid muscles. Although metastasis in this area frequently involves papillary thyroid carcinoma, it is rarely reported in head and neck squamous cell carcinoma.
CASE PRESENTATION: We report a case of a 79-year-old man who underwent a partial glossectomy for tongue cancer, followed by neck dissection and radiotherapy. One year posttreatment, the patient developed suprasternal (Burns space) lymph node metastasis accompanied by bone invasion of the sternum and clavicle. An en bloc resection, …
