This study reports 36 cases of gynaecomastia surgically treated. The median age was 25-years old (range 14-65 years). In 27 patients (75%) gynaecomastia was bilateral, in the others only one breast was interested. According to Simon's classification; 12 patients with stage 1, 18 stage 2a, 3 stage 2b and 3 stage 3 were observed. A specific cause was discovered in 12 cases: 2 Klinefelter syndrome and 10 adipomastia. Other 24 cases had an idiopathic origin. Gynecomastia develops when there is an increase in the ratio of estrogen to androgens. Gynecomastia is a benign condition that frequently occurs in the male breast gland; however, the cytogenetic data on this entity are very limited. To our knowledge, three cases have been reported in the literature, and the only one with an abnormal karyotype had a concomitant breast carcinoma. CASE PRESENTED: A 32-year man asked for medical advice due to gynecomastia. He had raised estrogen levels and diminished testosterone/estradiol index. A testicular echogram showed a nodular image in the right testis. Orchidectomy was performed and the diagnosis of a Leydig cell tumor was confirmed. Gynecomastia, an excessive development of the mammary glands in men, is a known phenomenon among patients with spinal cord disorder, yet in the last 50 years it has not been fully described in relation to spinal cord disorder. A man with arthritis, gynecomastia and a rash histologically showing leukocytoclastic vasculitis, dermatitis herpetiformis and immunoglobulin G deposits at the dermoepidermal junction is reported. In contrast to cases of systemic lupus erythematosus (SLE) associated with vesiculobullous eruptions and similar histological features, our patient had neither bullae nor serological evidence of SLE. Conversely, no men already reported had evidence of feminization. Thus, our case appears to be unique.

Nineteen tissue specimens obtained at surgery of male mammary gland due to persisting gynecomastia were assessed for the presence of estrogen receptors. In 13 cases the receptors were detected with the use of charcoal-dextran technique and 3H-estradiol as a ligand, the detection limit of such method being 0.2 fmol/mg of total protein. The average number of estrogen binding sites (mean +/- S. D.) was 2.49 +/- 2.47 fmol/mg total protein with the apparent association constant 2.8 X 10(9) 1 mol-1. The specificity of ligand binding to the receptor was confirmed by gradient centrifugation.

Gynecomastia is seldom observed in prepubertal boys. Sixteen cases are reported in this work, 3 of them being related to a defect of 11 hydroxylase with virilizing adrenal hyperplasia and 13 being considered as idiopathic. These gynecomastias consist of a slight breast enlargement, often unilateral (10 of 16) and may spontaneously disappear after 3 to 12 months (5 of 16 cases). In the so-called idiopathic cases, the hormonal investigations performed gave normal results, so that no physiopathological explanation could be suggested.

Pathological data from 122 cases of gynecomastia are studied. According to the importance of epithelial hyperplasia and of specialised stroma, 3 types are described. Every type is compared with the age of patients, the duration of the illness and occasional etiologic circumstances. It is concluded that these 3 types are not evolutive stages, that type II is the most frequent at any age and that there is no relation between etiology and histological type, except between oestrogenic therapy and type I. The alterations of connective tissue, especially breakdown of elastic fibers, are important.

Massive gynecomastia can produce severe psychological problems for the patient. The surgical techniques employed for simple gynecomastia are not effective in correcting the displaced nipple and excess skin. Utilizing a dermal predicle technique, the excess fat and breast tissue have been removed, along with relocation of the nipple. Satisfactory results and acceptable scars have been obtained in four patients.

Peripheral blood levels of testosterone, estradiol, luteinizing hormone, and follicle-stimulating hormone and the metabolic clearance rates of testosterone and estradiol, as well as the peripheral conversion of testosterone into estradiol, were measured in 16 patients with hypertension.
