Background Hepatic infarction is an exceedingly rare complication of hemolysis, elevated liver enzymes, and low platelets syndrome. Few cases have been described in the medical literature and the true incidence remains unknown. It can lead to fulminant liver failure, liver transplant, or death if not promptly addressed. Case Report A 22-year-old primigravida presented with right upper quadrant and epigastric pain at 28 weeks' gestation. She had severely elevated blood pressures requiring intravenous antihypertensives as well as proteinuria, thrombocytopenia, and mild transaminitis. Within 6

HELLP syndrome remains a leading cause of maternal and neonatal mortality and morbidity worldwide, which symptoms include hemolysis, elevated liver enzymes and low platelet count. The objective of this study was to determine whether HELLP is associated with AT1-AA. The positive rate and titer of AT1-AA in plasma from pregnant women were determined, and the correlation of AT1-AA titer with the grade of HELLP was analyzed. A HELLP rat model established by intravenous injection of AT1-AA. Our experimental results show the AT1-AA titer and positive rate were significantly higher in HELLP group, an…

Subcapsular liver hematoma is a rare but potentially life-threatening complication of preeclampsia and hemolysis, elevated liver enzymes, and low platelets syndrome. It may present with nonspecific signs and symptoms, none of which are diagnostic, and can mimic pulmonary embolism of cholecystitis. There is no consensus on the management of subcapsular liver hematoma. Unruptured liver hematoma can be conservatively managed. When rupture occurs, surgical, endovascular approaches and, rarely, liver transplantation, may be required. Actual literature is scant and retrospective in nature. Data on f

A 30-year-old female patient, 18 weeks gestational age, with no prior medical history was admitted to hospital complaining severe right upper quadrant pain. The patient was admitted to intensive care unit (ICU) after emergency surgery to treat intraperitoneal hemorrhage caused by rupture of liver hematoma. Despite the absence of high blood pressure, the patient was diagnosed with hemolysis, elevated liver enzymes, and low platelet count (HELLP) syndrome on the basis of abnormal levels of blood aspartate aminotransferase/alanine aminotransferase, lactate dehydrogenase, total bilirubin, direct b

Thrombotic microangiopathies (TMAs) are a group of disorders characterized by occurrence of thrombi of fibrin and/or platelets with microvascular occlusion and organ ischemia especially the kidney and brain. Hemolysis with a microangiopathic blood smear, elevated liver enzymes, and low platelet count (HELLP syndrome) is a type of TMA peculiar to pregnancy and may be associated with neurological complications. Visual complications in HELLP are usually related to cortical blindness. We present the first case of HELLP associated with bilateral anterior ischemic optic neuropathy (AION) and blindne…

PURPOSE: To determine the relationship between pregnancy-associated plasma protein-A (PAPP-A) levels and preeclampsia and HELLP syndrome at last trimester and to investigate if the severity of preeclampsia would be associated with PAPP-A levels.
METHODS: Four groups were constructed; mild-preeclampsia group consisted of 19 women, severe-preeclampsia group 17, HELLP group 5 and control group 32 women. All groups were matched strictly for gestational age at last trimester. Maternal blood samples for PAPP-A were collected as soon as the patients were diagnosed as preeclampsia or HELLP syndrome a…

In preeclampsia and hemolysis, elevated liver enzymes and low platelet (HELLP) syndrome, impaired trophoblast invasion and excessive fibrin deposition in the placental intervillous space is associated with fetal compromise. However, little information is available whether modulation of placental protease expression--potentially causing impaired trophoblast invasion--is associated with the HELLP syndrome. Total RNA and protein were extracted from placental tissue from 11 females with HELLP syndrome and 8 controls matched for gestational age. mRNA expression of matrix metalloprotease (MMP) -2 an…

The HELLP syndrome is a variant of severe preeclampsia. Its leading symptoms are: hemolysis, increased liver enzymes and low platelet count. The treatment of the HELLP syndrome remains controversial as there are no standards of proven efficacy. The article presents a case of a 27- year-old patient, G1P0, 30 wk of gestation, reporting no fetus movements for several hours, who was admitted to The Materno-Fetal Medicine Department, Polish Mother's Memorial Hospital in Lódź, with symptoms of the onset of the HELLP syndrome. The authors present the course of the therapy based on steroids administra…

After cesarean section, a 30-year-old patient with HELLP syndrome (hemolysis, elevated liver enzymes, and low platelet count in association with preeclampsia) had fever, jaundice, loss of ascites, and a cardiac murmur. Magnetic resonance imaging showed a tumor extending from the left lobe of the liver into the inferior vena cava, right atrium, and ventricle. The biopsy specimen revealed an embryonal sarcoma of the liver. On the second day of chemotherapy, the patient died of intra-abdominal hemorrhage from the tumor.

BACKGROUND: The HELLP-syndrome is a rare complication to pregnancy and a potential fatal condition.
CASE: We report such a case in a 32-year-old para 2, gravida 1 woman with no known risk factors. A lower transverse uterotomy was performed at 29 weeks and 6 days' gestation based on an indication of HELLP-syndrome. Uncharacteristically lower back pain developed on the second day postoperatively and a gynaecological examination was performed. A partial necrotic cervix uteri was found with many trombosed vessels and necrotic tissue.
CONCLUSION: Partial necrosis of cervix uteri can be seen as a

OBJECTIVE: HELLP syndrome is a severe form of preeclampsia, characterized by hemolysis (H), elevated liver enzymes (EL), and low platelets (LP), whose pathogenesis is unclear. Autoimmunity is thought to play an important role. After the observation of development of type 1 diabetes in a patient with HELLP syndrome, we assumed a possible disease association based on autoimmune reactions.
RESEARCH DESIGN AND METHODS: We examined 70 women with HELLP syndrome for the presence of autoimmune markers and glucose intolerance. Free thyroxine, triiodothyronine, thyroid-stimulating hormone, anti-thyrogl…
