Use of reproductive and sexual health services among female family planning clinic clients exposed to partner violence and reproductive coercion.

To examine the associations of recent intimate partner violence (IPV) and reproductive coercion (RC) with frequency of use of reproductive and sexual health services, a cross-sectional survey was administered to 16-29 year old women seeking care in five family planning clinics (n = 1,262). We evaluated associations of recent experiences of IPV, RC, or both IPV and RC with recent care seeking for pregnancy testing, emergency contraception, and sexually transmitted infection testing using multinomial logistic regression. Sixteen percent of respondents reported IPV and 13.5 % reported RC in the p

Discharge-planning for long-term care needs: the values and priorities of older people, their younger relatives and health professionals.

Discharge-planning decisions about long-term care (LTC) can be difficult and distressing for older people, families and discharge-planning health professionals. Retrospective research suggests that despite good intentions and a shared focus on the best interests of the older person, stakeholders may hold very different values about good outcomes and how to decide them. We aimed to compare the opinions and values of frail elders living at home, younger relatives and health professionals experienced in discharge-planning, prospectively: before, not after, a LTC decision. We interviewed three typ

Evaluation of factors influencing the reduction of home mechanical ventilation dependency in patients planned to receive home health care.

BACKGROUND: With increasing life expectancy and the growing burden of chronic diseases, healthcare systems are shifting from hospital-centered models to community- and home-based care. The use of home mechanical ventilators (HMVs) facilitates early discharge from hospital settings yet creates significant physical, psychological, and social burdens for caregivers. Identifying strategies to reduce ventilator dependency during palliative inpatient care may alleviate these challenges. METHODS: This retrospective, cross-sectional, descriptive study employed a quantitative approach with purposive s

Fast-Tracking the Implementation of National Action Plan for Health Security (2018-2024) in Uganda: The Acceleration Team Model.

Uganda's 2017 Joint External Evaluation (JEE) revealed critical gaps in International Health Regulations (IHR) implementation, with limited progress despite ongoing efforts. Coordination mechanisms failed to address multisectoral implementation challenges. To accelerate progress, in 2018, Uganda established a dedicated National Action Plan for Health Security Acceleration (NAPHS) Team (A-Team)-a multidisciplinary unit embedded within government structures to facilitate cross-sector coordination and reduce bureaucratic barriers. We conducted a multimethod evaluation using outcome harvesting, ke

Facilitation of Health Plan-Clinic Partnerships to Improve Colorectal Cancer Screening for Rural Medicaid Enrollees: A Qualitative Study.

PURPOSE: Mailed fecal immunochemical test (FIT) and patient navigation for follow-up colonoscopy have been shown to increase colorectal cancer (CRC) screening rates, but implementation barriers exist in rural settings. Partnerships between clinics and Medicaid health plans may address these barriers. In a large pragmatic randomized trial, we conducted a qualitative analysis to identify ways in which practice facilitation supported the implementation of a partnered health plan-clinic intervention of mailed FIT and patient navigation to improve CRC screening among rural Medicaid enrollees. Respective characteristics and contributions of two cataract surgery delivery models to the eye health national action plan in Rwanda.

BACKGROUND: Cataract is the leading cause of avoidable blindness worldwide, and limited surgical access continues to affect elimination efforts, especially in low- and middle-income countries. In line with this burden, Rwanda implemented two complementary models for cataract surgery services: The Mobile Teams Outreach Cataract Surgery Model (MTO-CSM) and the Continuous Hospital-Based Cataract Surgery Model (CHB-CSM). This study compared the performance, quality adherence, efficiency, and cost-effectiveness of the two models. METHODS: A retrospective, cross-sectional study was conducted across

Medicaid Managed Care Prior Authorization For Buprenorphine Tied To State Partisanship And Health Plan Profit Status, 2018.

Buprenorphine is among the most effective drugs for treating opioid use disorder, yet only a quarter of Americans who need it receive it. Requiring prior authorization has been identified as an important barrier to buprenorphine access. However, the practice remains widespread in Medicaid-the largest insurer of Americans with opioid use disorder. In this study, we examined how prior authorization for buprenorphine is related to plan structure and state political environment, using data on all 266 comprehensive Medicaid managed care plans active in 2018. We found substantial variation in prior

Could Intern Health Care Students Control Their Emotions and Make a Career Plan During the COVID-19 Pandemic?

The negative effects of the pandemic caused students to make changes in their career plans and their ability to manage their emotional states. Not only health students in our country, but also students in other countries of the world experienced fear, anxiety, and unwillingness to participate in professional practices and the provision of care for patients with COVID-19 during the COVID-19 pandemic. The study was conducted to determine factors influencing intern healthcare students' career adaptability and their ability to manage emotions during the COVID-19 pandemic. The sample of this cross-

Opioid Dose Trajectories and Associations With Mortality, Opioid Use Disorder, Continued Opioid Therapy, and Health Plan Disenrollment.

IMPORTANCE: Uncertainty remains about the longer-term benefits and harms of different opioid management strategies, such as tapering and dose escalation. For instance, opioid tapering could help patients reduce opioid exposure to prevent opioid use disorder, but patients may also seek care elsewhere and engage in nonprescribed opioid use. OBJECTIVE: To evaluate the association between opioid dose trajectories observed in practice and patient outcomes. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study was conducted in 3 health systems in Colorado and Wisconsin. The study popu

The older prisoner health and social care assessment and plan (OHSCAP) versus treatment as usual: a randomised controlled trial.

BACKGROUND: Older people are the fastest-growing demographic group among prisoners in England and Wales and they have complex health and social care needs. Their care is frequently ad hoc and uncoordinated. No previous research has explored how to identify and appropriately address the needs of older adults in prison. We hypothesised that the Older prisoner Health and Social Care Assessment and Plan (OHSCAP) would significantly increase the proportion of met health and social care needs 3 months after prison entry, compared to treatment as usual (TAU). METHODS: The study was a parallel random
