Anemia management: development of a rapidaccess anemia and intravenous iron service.

This article describes the initiation and evolution of the Rapid-Access Anemia Clinic (RAAC) at Guy's and St Thomas' Hospitals, London, UK. This clinic was set up to provide diagnosis and treatment, and to coordinate investigative procedures, where necessary, into the underlying causes of anemia. Initially piloted with anemic preoperative orthopedic patients, the clinic now treats a wide range of conditions, deriving from both internal and external referrals. Treatment includes dietary advice, supplementation with iron, vitamin B12 and folate, and blood transfusion. Most patients at the RAAC n

Making sense of street chaos: an ethnographic exploration of homeless people's health service utilization.

BACKGROUND: Homeless people have poor health and mortality indices. Despite this they make poor usage of health services. This study sought to understand why they use health services differently from the domiciled population. METHODS: Ethnographic observations were conducted at several homeless services, in Dublin. This was supplemented with 47 semi-structured interviews with homeless people and two focus groups of homeless people and hospital doctors. A critical-realist approach was adopted for interpretation of the data. RESULTS: Homeless people tended to present late in their illness; def

End-of-life issues for American Indians/Alaska Natives: insights from one Indian Health Service area.

BACKGROUND: In the United States, the American Indian and Alaska Native (AI/AN) population is aging and the leading causes of death for those 55 and older are chronic diseases such as cancer, heart disease, and the complications of diabetes. Since 1955, the federally directed Indian Health Service, along with Tribal governments has been providing comprehensive health care to over 500 AI/AN communities. OBJECTIVE: This is the first study to examine end-of-life and palliative care issues broadly at selected Indian Health Service facilities in the Albuquerque area. DESIGN: Retrospective medical

State-level variation in access to long-acting injectable antiretroviral therapy for HIV in the United States.

Long-acting injectable antiretroviral therapy (LAI-ART) is expected to improve health outcomes among persons with HIV. Yet, uptake has been slow and data on potential barriers to access are sparse. We used medication formulary data from state Medicaid and AIDS Drug Assistance Programs (ADAPs) to examine state-level variation in access to LAI-ART among uninsured and low-income persons with HIV. We identified substantial coverage gaps: cabotegravir/rilpivirine was not covered without prior authorization by 26 state Medicaid programs and not covered at all by 15 state ADAPs; lenacapavir was not c

The Impact of Certified Community Behavioral Health Clinics on Service Access and Engagement.

The mental health system struggles to engage people with severe mental illnesses (SMI) and substance use disorders (SUD) in care. Certified community behavioral health clinics (CCBHCs) are designed to meet service gaps by providing person-centered, comprehensive, coordinated, accessible, and quality care. This study examines the impact of the CCBHC model on access and engagement of service users utilizing administrative data. There was a significant increase in existing and new service users (χ2 = 5.91; p < .05) after the CCBHC implementation and among existing service users with SMI (χ2 = 43.

Cohort profile: a data linkage cohort to examine health service profiles of people with intellectual disability in New South Wales, Australia.

PURPOSE: People with intellectual disability are a minority group who experience poorer physical and mental health than the general population and have difficulty accessing healthcare services. There is lack of knowledge about healthcare service needs and gaps experienced by people with intellectual disability. This study aims to interrogate a large linked administrative data set containing hospital admissions, presentations to emergency departments (ED) and mortality data to provide evidence to inform the development of improved health and mental health services for this population. Operation Enduring Freedom (OEF) and Operation Iraqi Freedom (OIF) military mental health issues. Information on the wars' signature wounds: posttraumatic stress disorder and traumatic brain injury.

This topic can be related to the nursing profession and the need to be better educated on military mental health. Since mood disorder, suicide, alcohol abuse, PTSD and TBI are evident in actively serving and returning service members, it is imperative to educate nurses and healthcare providers about these conditions, the available evidence-based treatments and referrals to programs for these signature OEF/OIF wounds. The authors encourage nurse educators to consider ways to include military mental health and other service-related health issues into nursing curricula and to use Veterans Adminis

Kenya: the development of private services and the role of the Kenya Veterinary Association.

Private veterinary practice has existed in Kenya for more than half a century. Between the early 1930s and the mid-1960s, provision of clinical and advisory services almost entirely involved servicing commercial ranches and dairy farms. The Department of Veterinary Services (VSD) was mainly responsible for providing regulatory services in these areas. Until the mid-1960s, public sector veterinary responsibilities were predominantly associated with the prevention of notifiable diseases outside the commercial farming areas. In a major agrarian reform programme initiated in 1954, Kenya initiated

[Sanitary and antiepidemic regulations of medical services of the state administration in Lodz at the turn of the XIXth and XXth c. (till 1914)].

At the turn of XIX and XX c., because of the spreading of acute infectious diseases in Lodz, the medical services of the state administration, responsible for the public health care, were obliged to publish and to enforce different sanitary and anti-epidemic regulations. They contained basic instructions e.g. the care of sanitary state of dwellings, hospitalization of infectious patients and isolation of persons suspected of contact with diseased, disinfection and anti-smallpox vaccination. Such prophylactic and therapeutic activity was neither systematic nor complementary due to the bureaucra

Substance abuse treatment as HIV prevention: more questions than answers.

This report examines associations between the availability of human immunodeficiency virus (HIV)-related health services in substance abuse treatment programs and characteristics of the programs and the patients they serve. In a cross-sectional, descriptive design and via a validated survey, program administrators within the National Drug Abuse Treatment Clinical Trials Network provided information on program characteristics, patient characteristics (rates of risky sexual and drug behaviors and HIV infection), and the availability of 31 different HIV-related health services. Of 319 programs, 8

HRSA's evidence-based tele-emergency network grant program: Multi-site prospective cohort analysis across six rural emergency department telemedicine networks.

BACKGROUND: The Health Resources and Services Administration (HRSA), Federal Office of Rural Health Policy (FORHP) funded the Evidence-Based Tele-Emergency Network Grant Program (EB TNGP) to serve the dual purpose of providing telehealth services in rural emergency departments (teleED) and systematically collecting data to inform the telehealth evidence base. This provided a unique opportunity to examine trends across multiple teleED networks and examine heterogeneity in processes and outcomes. METHOD AND FINDINGS: Six health systems received funding from HRSA under the EB TNGP to implement t

Creating a recovery-oriented system of behavioral health care: moving from concept to reality.

This article describes challenges and successes seen in the first four years of efforts the state of Connecticut has made to reorient its behavioral health system to promoting recovery. Beginning in 2000, the Connecticut initiative was conceptualized as a multi-year, systemic process that involved the following interrelated steps: a) developing core values and principles based on the input of people in recovery; b) establishing a conceptual and policy framework based on this vision; c) building workforce competencies and skills; d) changing programs and service structures; e) aligning fiscal a

Does age at diagnosis influence the use of health services for multiple sclerosis?

BACKGROUND: Clinical studies suggest that disease course of multiple sclerosis (MS) differs according to age of onset. However, most of these studies were cross-sectional and had modest sample sizes. Population-based administrative data provide an alternative long-term perspective on disease progression and further document the association between age at diagnosis and progression of MS. Our objective was to study the association between age at diagnosis and the use of health services for MS. METHODS: Data on 1426 subjects with MS were extracted from the Québec Birth Cohort on Immunity and Hea

Systems thinking in the prevention, control, and elimination of neglected tropical diseases (NTDs).

OBJECTIVES: It is estimated that more than one billion people are affected by neglected tropical diseases (NTDs). Whilst there are existing treatments to reduce morbidity, a major challenge is creating robust programs that are both country-led and leverage the existing health systems. Using a systems thinking approach is a critical component for integrating NTD services, as it takes into account all aspects of what might otherwise be considered fragmented systems to create a more efficient system. DESIGN: This article showcases three case studies where a systems lens was applied: (1) to exami

Health in Seychelles - an overview.

Health in Seychelles has improved significantly over the past three decades, largely as a result of investment not only in the health services, but also in other social sectors that have direct impact on child survival and the health of individuals. Through different stages of the evolution of the health care system there have been close links between health policy and strategy, the overall national development efforts and the wider social and economic environment. Awareness of these links are useful in the understanding of current health issues such as the lifestyle related patterns of morbid
