Twenty-Four Syndrome: An Untold Presentation of Pontine Hemorrhage.

Pontine hemorrhages are relatively uncommon. Various atypical manifestations of pontine stroke like eight-and-a-half syndrome, fifteen-and-a-half syndrome, and sixteen syndrome have been described in the past. We came across a case of pontine bleed that presented with bilateral facial palsy, bilateral horizontal gaze palsy, and contralateral sensorineural hearing loss accounting to the hitherto not described "twenty-four syndrome" with Horner's syndrome and left hemiparesis.

Mitochondrial defects and hearing loss.

The techniques of human molecular genetics have been rapidly applied to the study of hearing loss. These studies have implicated more than 60 loci as causes of nonsyndromic hearing loss. Mutations at more than a dozen nuclear genes have been demonstrated to cause hearing loss, and these have been covered in recent reviews. However, a perhaps unexpected feature of the molecular characterization of human hearing loss has been the occurrence of mutations in the mitochondrial DNA (mtDNA). The importance of mitochondrial function in hearing is emphasized by the recent discovery of mutations in a nu

Determinants of auditory performance after cochlear implantation in post-lingual adults: beyond the duration of hearing loss.

PURPOSE: Prolonged auditory deprivation progressively compromises central auditory pathway integrity, yet its prognostic impact in post-lingual adults undergoing cochlear implantation remains poorly defined. This study evaluates the influence of hearing loss duration - expressed in absolute terms, dichotomised by the conventional 10-year threshold, and as a proportion of lifetime with hearing loss - on auditory performance, and identifies the clinical determinants associated with the immediate activation effect and the post-implantation trajectory. METHODS: Single-centre, retrospective, obser

Effect of peripheral hearing loss on the masking level difference.

The effects of both symmetric and asymmetric peripheral hearing loss on the masking level difference (MLD) at 500 Hz were studied in 651 subjects with conductive or sensorineural hearing loss and 270 normal controls. The data supported the hypothesis that the modification in MLD due to peripheral hearing loss results from deterioration in threshold in the antiphasic condition. These results provide correction factors for the normal MLD range as functions of boundary frequency of sensorineural loss above 500 Hz and degree of both symmetric and asymmetric sensitivity loss at 500 Hz in both subje

Primary Neural Degeneration after Cochlear Implantation: Histological and Electrophysiological Evidence from a Mouse Model.

Patients undergoing hearing-preservation cochlear implantation (HPCI) often retain low-frequency thresholds initially, yet many develop progressive loss of residual hearing and variable speech performance. The biological basis of this delayed functional decline remains unclear. Here we show that HPCI at the basal turn induces primary neural degeneration at the cochlear apex-a region anatomically remote from the electrode array and functionally important for low-frequency hearing and speech discrimination. Using a mouse model of HPCI (all male), we systematically quantified longitudinal electro

The Functional Hearing Gain with an Active Transcutaneous Bone Conduction Implant Does Not Correlate with the Subjective Hearing Performance.

The functional hearing outcome with hearing implants does not always properly reflect the subjective benefit in everyday listening situations. In this study, the functional hearing gain and the impact on the subjective hearing ability and quality of life were assessed in patients with a Bonebridge. A chart review was performed on 45 patients with a Bonebridge who were provided with questionnaires regarding the hearing quality and health-related quality of life during their last clinical visit. The questionnaires consisted of the Speech, Spatial and Qualities (SSQ) and the Health Utility Index

A Retrospective Study of Hearing Loss in Patients Diagnosed with Peroxisome Biogenesis Disorders in the Zellweger Spectrum.

OBJECTIVES: Peroxisome Biogenesis Disorders in the Zellweger Spectrum (PBD-ZSD) are autosomal recessive disorders characterized by defects in peroxisome function, biosynthesis, and/or assembly. Despite its frequent documentation, hearing loss associated with PBD-ZSD has not been extensively characterized. The purpose of this retrospective natural history study was to better characterize the hearing loss associated with PBD-ZSD and to provide additional insight into the evaluation and management of PBD-ZSD patients with hearing loss. DESIGN: Audiological data from medical records of 42 patient

Direct reprogramming of fibroblasts into spiral ganglion neurons by defined transcription factors.

Degeneration of the cochlear spiral ganglion neurons (SGNs) is one of the major causes of sensorineural hearing loss and significantly impacts the outcomes of cochlear implantation. Functional regeneration of SGNs holds great promise for treating sensorineural hearing loss. In this study, we systematically screened 33 transcriptional regulators implicated in neuronal and SGN fate. Using gene expression array and principal component analyses, we identified a sequential combination of Ascl1, Pou4f1 and Myt1l (APM) in promoting functional reprogramming of SGNs. The neurons induced by APM expresse

CDC Grand Rounds: Promoting Hearing Health Across the Lifespan.

Globally, one in three adults has some level of measurable hearing loss, and 1.1 billion young persons are at risk for hearing loss attributable to noise exposure. Although noisy occupations such as construction, mining, and manufacturing are primary causes of hearing loss in adults, nonoccupational noise also can damage hearing. Loud noises can cause permanent hearing loss through metabolic exhaustion or mechanical destruction of the sensory cells within the cochlea. Some of the sounds of daily life, including those made by lawn mowers, recreational vehicles, power tools, and music, might pla

Cognitive and behavioral effects of hearing loss.

PURPOSE OF REVIEW: Hearing loss is one of the largest modifiable risk factors for developing dementia, accounting for up to 9% of the overall modifiable risk. The neuropsychologic and psychosocial impacts of hearing loss are becoming increasingly appreciated. The objective of this review is to explore the recent literature regarding the cognitive and behavioral effects of hearing loss and the role of hearing rehabilitation, particularly in older adults. RECENT FINDINGS: Cognitive decline may begin while patients have subclinical hearing loss, earlier than previously thought. Hearing rehabilit

Contralateral recovery of cochlear function after cochlear implantation.

Significant recovery of sensorineural hearing loss in either ear after cochlear implantation (CI) is rare. We present the case of a 57-year-old lady with medically treated depression and a background of chronic suppurative otitis media (CSOM) in the non-implanted ear, who clearly fulfilled audiological criteria for CI. Two years post-implantation her CSOM in the non-implanted ear was addressed with blind sac closure of the ear. Post-operatively the hearing thresholds in this ear had improved to the extent that a bone-anchored hearing aid became a viable option. The literature is carefully revi

PALM3 and hearing loss: a potential dual diagnosis interfering with novel gene discovery.

Hereditary hearing loss is highly genetically heterogeneous, with emerging overlap between genes implicated in early-onset and age-related hearing loss. We report a consanguineous family with autosomal recessive, non-syndromic hearing loss in which the proband harbors a homozygous splice-site variant in PALM3 NM_001145028.2:c.314+1G>A and a homozygous missense variant in OTOA NM_144672.4:c.1939G>C p.(Gly647Arg). A minigene assay for the PALM3 variant demonstrated aberrant splicing with exon skipping, resulting in either a frameshift or a large in-frame deletion, both consistent with loss of fu

Long-term hearing preservation after surgery for vestibular schwannoma.

OBJECT: Vestibular schwannomas (VSs) are now amenable to resection with excellent hearing preservation rates. It remains unclear whether immediately postoperative hearing is a durable result and will not diminish over time. The aim of this study was to determine the rate of long-term preservation of functional hearing following surgery for a VS and to examine factors influencing hearing preservation. METHODS: All patients eligible for hearing preservation (Gardner-Robertson Class I or II) who had undergone resection of a VS by a single surgeon were reviewed retrospectively. Follow-up audiogra

A review of the effectiveness of otoacoustic emissions for evaluating hearing status after newborn screening.

OBJECTIVES: The purpose of this study was to give a brief review of the effectiveness of otoacoustic emissions for getting frequency-specific information about a hearing-loss problem in newborns after hearing screening. Especially, the advantages of distortion-product otoacoustic emissions (DPOAE) over transiently evoked otoacoustic emissions (TEOAEs) are described. DATA SOURCES: Approximately 186 ears of 104 children aged between 76 days and 15 years and 436 ears of adults with normal hearing and sensory hearing loss. METHODS: Extrapolated DPOAE I/O-functions at frequencies between 1.5 and

Hearing vulnerability after noise exposure in a mouse model of reactive oxygen species overproduction.

Previous studies have convincingly argued that reactive oxygen species (ROS) contribute to the development of several major types of sensorineural hearing loss, such as noise-induced hearing loss (NIHL), drug-induced hearing loss, and age-related hearing loss. However, the underlying molecular mechanisms induced by ROS in these pathologies remain unclear. To resolve this issue, we established an in vivo model of ROS overproduction by generating a transgenic (TG) mouse line expressing the human NADPH oxidase 4 (NOX4, NOX4-TG mice), which is a constitutively active ROS-producing enzyme that does

Cognitive, physical, and mental profiles of older adults with misplaced self-evaluation of hearing loss.

BACKGROUND: Although inconsistency between objective and subjective hearing loss among older adults has been suggested, a systematic examination of the cognitive and physical functioning among such older adults is lacking. Our objective was to assess the cognitive, physical, and mental profiles associated with the discrepancy. METHODS: The auditory acuity of 696 community-dwelling older adults was evaluated using a pure-tone average of hearing thresholds at 1.0 and 4.0 kHz in the better-hearing ear. Participants were then stratified as follows: normal hearing ≤ 25 dB, mild loss >25 dB and ≤40

Connexins in Acquired Hearing Loss: Expanding Research Perspectives.

Connexins, as key players in intercellular communication in the inner ear, are vital for maintaining normal hearing function. While numerous studies have explored their role in congenital hereditary hearing loss, the underlying mechanisms and therapeutic potential of connexins in acquired hearing loss remain to be fully elucidated. This review summarizes recent advances in connexin research in the context of acquired hearing loss, with a focus on presbycusis, noise-induced, and drug-induced hearing loss, and delves into their pathophysiological roles. Through the analysis and organization of t

Drug Discovery Targeting Nicotinic Acetylcholine Receptors for Hearing Loss.

Hearing loss is detrimental to human health, and currently, more than 1.5 billion people are affected by hearing loss. Active military personnel and construction workers are examples of individuals in the workplace who are exposed to loud noise and are at serious risk of hearing loss. While there is currently no therapy for hearing loss, evidence supports investigating the enhancement of the Medial Olivocochlear (MOC) system, an efferent pathway for hearing that serves as a gain-control for hearing loss protection. Selectively modulating the α9α10 nicotinic acetylcholine receptor (nAChRs) foun
