High-risk genes involved in common septal defects of congenital heart disease.

The septation defect is one of the main categories of congenital heart disease (CHD). They can affect the septation of the atria leading to atrial septal defect (ASD), septation of ventricles leading to ventricular septal defect (VSD), and formation of the central part of the heart leading to atrioventricular septal defect (AVSD). Disruption of critical genetic factors involved in the proper development of the heart structure leads to CHD manifestation. Because of this, to identify the high-risk genes involved in common septal defects, a comprehensive search of the literature with the help of

[Restrictive ventricular septal defect in a dwarf rabbit].

A 9-week-old intact female dwarf rabbit was presented for evaluation of a heart murmur. Physical examination revealed a grade IV/VI systolic heart murmur with the maximal intensity over the right heart base. Evidence for a left-sided cardiomegaly was present on lateral and dorsoventral radiographs. An electrocardiogram was recorded in right lateral recumbency, which revealed a sinus tachycardia with a heart rate of 360 beats/minute. Echocardiography showed normal dimensions for the atria and ventricles. Two-dimensional echocardiography confirmed the presence of a perimembranous ventricular sep

MR imaging and CT of vascular anomalies and connections in patients with congenital heart disease: significance in surgical planning.

To plan effective management of congenital heart disease, one needs the clearest understanding of the anatomy. Although echocardiography and angiography are the dominant imaging modalities in patients with congenital heart disease, magnetic resonance (MR) imaging and computed tomography (CT) are valuable noninvasive adjuncts. MR imaging and CT are effective in demonstrating the complex cardiovascular morphology present in congenital heart disease, especially the extracardiac morphology. In patients with tetralogy of Fallot with complex pulmonary artery anatomy, MR imaging and CT are useful in

The roles of global longitudinal strain imaging in contemporary clinical cardiology.

Myocardial deformation imaging is now readily available during routine echocardiography and plays an important role in the advanced care of cardiovascular diseases. Its clinical value in detecting subtle myocardial dysfunction, by helping diagnose disease and allowing prediction of disease progression and earlier pharmacological intervention, has been demonstrated. Strain imaging has been the most studied and clinically used technique in the field of cardio-oncology. A relative percent reduction in left ventricular (LV) global longitudinal strain > 15% from baseline is considered a marker of e

Ductus venosus Doppler in the assessment of fetal cardiovascular health: an updated practical approach.

The ductus venosus has a central role in the distribution of highly oxygenated umbilical venous blood to the heart. Its waveform is related to the pressure-volume changes in the cardiac atria and it is therefore important in the monitoring of any fetal condition that may affect forward cardiac function. The cardiovascular parameters that can influence forward cardiac function include afterload, myocardial performance and preload. Decreased forward flow during atrial systole (a-wave) is the most sensitive and ubiquitous finding when any of these parameters is affected. In contrast, decreased fo

[Structural and electrophysiological characteristics of the heart function during paroxysmal atrial fibrillation].

AIM: To examine structural and electrophysiological predictors of the onset and establishment as chronic of cardiac fibrillation (CF). MATERIAL AND METHODS: Echocardiography, transesophageal electrophysiological examination of the heart were performed in 420 cases of paroxysmal cardiac fibrillation (PCF). They were diagnosed as having new-onset (n = 68), rare (n = 92) and frequent (n = 260) PCF. RESULTS: Compared to healthy subjects, PCE patients had larger size of the left atrium (LA), higher atrium/ventricle value, lower frequency threshold of arrhythmia induction, decreased left atrial ef

Differential diagnosis of pulmonic stenosis by means of intracardiac phonocardiography.

The purpose of the present paper is to describe the origin of the systolic murmur in pulmonic stenosis and to discuss the diagnostic possibilities of intracardiac phonocardiography. Right heart catheterization was carried out with the aid of a double-lumen A.E.L. phonocatheter on 48 pulmonic stenosis patients with or without associated heart lesions. The diagnosis was confirmed by heart catheterization and angiocardiography in all cases and in 38 of them, by surgical intervention. Simultaneous phonocardiograms were recorded with intracardiac pressure tracings. In valvular pulmonic stenosis, th

Estimation of PQ distance dispersion for atrial fibrillation detection.

BACKGROUND AND OBJECTIVE: Atrial fibrillation (AF) is the most common cardiac arrhythmia in the world. It is associated with significantly increased morbidity and mortality. Diagnosis of the disease can be based on the analysis of the electrical atrial activity, on quantification of the heart rate irregularity or on a mixture of the both approaches. Since the amplitude of the atrial waves is small, their analysis can lead to false results. On the other hand, the heart rate based analysis usually leads to many unnecessary warnings. Therefore, our goal is to develop a new method for effective AF

Minimally invasive resection of a giant left atrial myxoma: a case report.

Cardiac tumors are a rarity. Most diagnosed primary tumors of the heart are benign, with an incidence ranging between 0.001% and 0.03%. Cardiac myxoma is one of these benign entities. A 44-year-old Caucasian woman who presented with symptoms of a common cold was diagnosed with a massive obstructing myxoma of the left atrium. Despite its large size, the tumor was completely removed using minimally invasive access through right anterior thoracotomy. However, the myxoma was adherent to the left atrial septum and was excised in toto. Pathological examinations confirmed the diagnosis of a primary c

[Giant left atrium in a patient with decompensated heart failure].

A 71-year-old woman was admitted to our institution with symptoms and signs of acute congestive heart failure. The patient had a long history of rheumatic heart disease and, in 1974, she had undergone mitral valve replacement with a single tilting-disk mechanical valve. Transthoracic echocardiography showed a giant left atrium, measuring 18.5 cm longitudinally and 17 cm transversely from the apical four-chamber view with a volume of 1706 ml/m2 body surface area, associated with intense spontaneous echocontrast. Massive tricuspid regurgitation and severe pulmonary hypertension were diagnosed. I

[The method of atriotomy, ventriculotomy and their repairs].

In this series, we describe the method of atriotomy, ventriculotomy and their repairs. Right atriotomy is needed for expose tricuspid valve, atrial and ventricular septal defect. Left atriotomy is needed for expose mitral valve disease and technique for, maze or pulmonary vein isolation. Left ventriculotomy is needed for resection of scar and/of thrombus of ventricular aneurysm, repair of ventricular septal rupture and endoaneurysmorrhphy. Recently, this method is also used to repair for ischemic mitral valve regurgitation. These techniques have already been described in many textbooks. Regard

Electrophysiologic and anatomical relationships studied in primum atrioventricular septal defect.

Anatomy and pattern of electrical activation predict the function and contraction pattern of the heart. Patients with primum atrioventricular septal defect (primum AVSD) present with abnormality of both anatomical arrangements and electric activation and serve therefore as an interesting population for studying electro-anatomic relationships in the heart. Understanding the relationships between anatomic and electrophysiologic abnormality is appropriate not only for diagnosis, therapy, and prognosis in patients with primum AVSD but also for understanding the developmental relationship between t

[Winging effect in interatrial defect after transseptal mitral valvuloplasty: an anatomical study].

In the balloon catheters the redundancy of the deflated balloon produces 2 or 3 sort of thin wings. The presence of wings can reduce the catheter "pushability" or, in certain conditions, can determine tissue lesion (winging effect). Simulating a transeptal valvuloplasty of the mitral valve by technique of the 2 balloon in human fresh heart, we studied the winging effect over the interatrial septum in 12 hearts; in the first 6 the atrial septum was dilated with a 6 mm balloon (Group A), in the second 6 the atrial septum was dilated with a 10 mm balloon (Group B). The procedure was completed int
