Hemodynamic monitoring in cardiogenic shock.

PURPOSE OF REVIEW: Cardiogenic shock remains a major cause of mortality today. With recent advancements in invasive mechanical support strategies, reperfusion practice, and a new classification scheme is proposed for cardiogenic shock, an updated review of the latest hemodynamic monitoring techniques is important. RECENT FINDINGS: Multiple recent studies have emerged supporting the use of pulmonary artery catheters in the cardiogenic shock population. Data likewise continues to emerge on the use of echocardiography and biomarker measurement in the care of these patients. SUMMARY: The integra

The crashing patient: hemodynamic collapse.

PURPOSE OF REVIEW: Rapid restoration of tissue perfusion and oxygenation are the main goals in the resuscitation of a patient with circulatory collapse. This review will focus on providing an evidence based framework of the technological and conceptual advances in the evaluation and management of the patient with cardiovascular collapse. RECENT FINDINGS: The initial approach to the patient in cardiovascular collapse continues to be based on the Ventilate-Infuse-Pump rule. Point of care ultrasound is the preferred modality for the initial evaluation of undifferentiated shock, providing informa

Hemodynamic changes in iliofemoral disease.

BACKGROUND: Iliofemoral venous obstruction, caused by post-thrombotic disease, can be treated by percutaneous angioplasty and additional stenting with good results. However, no hemodynamic parameter determining the need for treatment has been defined. This article describes the preliminary results of a study investigating the pressure changes occurring in post-thrombotic deep venous obstruction. METHODOLOGY: Four patients with post-thrombotic deep venous obstruction of the iliofemoral tract were identified. Intravenous pressure was pre-operatively measured in the common femoral vein and in a

Hemodynamic modeling of the intrarenal circulation.

Three dimensional, time dependent numerical simulations of healthy and pathological conditions in a model kidney were performed. Blood flow in a kidney is not commonly investigated by computational approach, in contrast for example, to the flow in a heart. The flow in a kidney is characterized by relatively small Reynolds number (100 < Re < 0.01-laminar regime). The presented results give insight into the structure of such flow, which is hard to measure in vivo. The simulations have suggested that venous thrombosis is more likely than arterial thrombosis-higher shear rate observed. The obtaine

Common hemodynamic problems in the neonate.

Hemodynamic problems are common in neonatal intensive care. They occur in the context of incomplete myocardial and vascular development and in cardiovascular responses to interventions which are, as a result, limited and often uncertain and unpredictable. In this review, I outline the hemodynamic features of 4 neonatal conditions which often require intervention: (1) persistent pulmonary hypertension of the newborn, (2) cardiogenic shock (most commonly in the context of hypoxic ischemic injury), (3) sepsis and (4) low blood pressure in the transitional period of the extremely preterm infant. I

Hemodynamic support of the trauma patient.

PURPOSE OF REVIEW: The aim of this review is to address and summarize some key issues and recent insights into the hemodynamic support of the trauma patient related to fluid administration. RECENT FINDINGS: Colloids are not superior to crystalloids in treating hypovolemia in the trauma patient and show no survival benefit. Furthermore, several adverse effects (renal failure, bleeding complications and anaphylaxis) have been reported with the use of artificial colloids. Hypertonic saline is effective and well tolerated in the treatment of hypovolemic shock and traumatic brain injury. Potential

Hemodynamic monitoring during hemodialysis.

Intradialytic monitoring of hemodynamic parameters is an active area of research; future developments in this field will decrease intradialytic morbidity and the mortality of end-stage renal disease patients treated by hemodialysis. Recent investigations have been assisted by the development of devices that can continuously and noninvasively measure hematocrit and plasma protein concentration during the treatment. Intradialytic morbidity, fluid overload, and hypertension in chronic hemodialysis patients have been shown to be associated with either large or small intradialytic decreases in bloo

Hemodynamic changes of aortic regurgitation.

Hemodynamic changes associated with acute AR include a rapid increase in LV pressures during diastole, markedly elevated pressures at end diastole, and premature closure of the mitral valve. Systemic diastolic pressures may be low, but there is a minimal increase in pulse pressure. In very severe cases of acute AR, cardiac output may decrease, leading to hypotension. In chronic AR, the LV remodels to accommodate the regurgitant volume flow, and stroke volume increases to maintain effective forward blood flow. These adaptations lead to a dilated LV, a widened pulse pressure, and a low diastolic

[Hemodynamic effect of intrathecal clonidine].

Intrathecal administration of bupivacaine and clonidine results in a significant prolongation of both motor and sensitive blocks but side effects-hypotension and bradycardia-are observed. We compared two groups of patients allocated randomly. One group received intrathecal bupivacaine-morphine and the other one received clonidine-bupivacaine-morphine. In the group which received clonidine a blood pressure decrease was observed from 2nd to 8th hour, independently from the sympathetic block and easily corrected by i.v. ephedrine. Some possible explanations are discussed. No complications due to

[The application of hemodynamic examination in psychiatry].

The authors discuss the most modern non-invasive methods for the examination of the circulatory system and consider the opportunity of their application in psychiatry. They refer to the results of their earlier studies on the some selected parameters of the circulatory system by means of radiocardiography i.e. Total Peripheral Resistance Index (TPRI), Heart Rate (HR), Arterial Blood Pressure (AP) and the Stroke Index (SI). It follows from the results that such examinations are appropriate mainly in patients with endogenous depression and may contribute to an increased safety factor in the trea

Hemodynamic effects of antidysrhythmic drugs.

Antidysrhythmic medications, when administered in the clinical setting, demonstrate a variety of effects on cardiac electrophysiology, inotropy, and peripheral vascular smooth muscle. The resultant hemodynamic effects may be of profound importance to the clinical well-being of the patient being treated for dysrhythmias. The following review will discuss the hemodynamic effects of the Class I agent moricizine; Class IA agents: quinidine, procainamide hydrochloride, and tocainide hydrochloride; Class IC agents: flecainide acetate, encainide hydrochloride, and propafenone; Class II agents: propra

[Hemodynamic measurements by Doppler echocardiography].

This paper discusses the measurement of cardiac output and pulmonary artery pressures by Doppler echocardiography. Blood flow may be measured through the aortic valve, the left ventricle and the mitral and pulmonary valves. In each case certain conditions for the validity of calculations must be respected. These measurements contribute to the evaluation of valvular stenosis and regurgitation, intracardiac shunts and cardiac function. Pulmonary artery pressures may be estimated from Doppler signals of tricuspid and pulmonary regurgitation, from the morphology of systolic pulmonary blood flow an

Hemodynamic consequences of neonatal polycythemia.

The hemodynamic consequences of neonatal polycythemic hyperviscosity and the effects of partial exchange transfusion were evaluated in 13 infants. Mean (+/- SD) venous hematocrit was 72% +/- 2.5%. After partial exchange transfusion, whole blood viscosity at a shear rate of 11.5 sec-1 decreased from 16.2 to 8.4 centipoise. There were significant (P less than 0.05) increases in heart rate (127 +/- 7.5 to 139 +/- 7.8 beats/min), Doppler-derived cardiac index (200 +/- 35 to 263 +/- 48 ml/kg/min), left ventricular stroke volume index (1.56 +/- 0.23 to 1.89 +/- 0.33 ml/kg), systemic oxygen transport

Hemodynamic indicators of postthrombotic sequelae.

Single or repetitive episodes of lower-extremity venous thrombosis may result in venous obstruction, valvular incompetence, and the postthrombotic syndrome. Seventy-seven patients with ileofemoral deep venous thrombosis diagnosed with biplane phlebograms received routine anticoagulant therapy and follow-up with clinical and noninvasive vascular examinations (reflux photoplethysmography, Doppler ultrasonography, and venous outflow plethysmography). The results of the final visit (mean, 25 months; range, three to 50 months) revealed that 53 patients (69%) had evidence of edema and/or hyperpigmen

Hemodynamic assessment of venous problems.

Objective assessment of venous function based on venous refilling time recorded by photoplethysmography (PPG) was done in 89 patients with postphlebitic syndrome (47), primary varicose veins (29), and after an episode of acute deep vein thrombosis (13). Limbs with symptomatic postphlebitic changes had a shortened venous refilling time of 15.6 +/- 11 seconds (normal greater than 20 seconds). When venographic findings (ascending and descending) in 47 patients with postphlebitic syndrome were analyzed, it was noted that femoral valve incompetence demonstrated by descending venography was a common

[Hemodynamic reactions to intraventricular hemorrhage].

In a 13-year girl with head injury the hemodynamic parameters during 2 episodes of intraventricular hemorrhage were monitored. There were hyperdynamic values for heart rate, Cardiac output, mean arterial and pulmonary artery pressures, systemic vascular and pulmonary vascular resistances. It is likely that centrogenic, sympathico-adrenergic reactions are the cause of pulmonary failure after isolated head injuries.
