Addison's disease due to Histoplasma duboisii infection of the adrenal glands.

Histoplasmosis is a fungal infection caused by Histoplasma capsulatum. In the normal individual, both disseminated histoplasmosis and symptomatic adrenal histoplasmosis are rare. Herein, we describe the case of a 50-year-old gentleman residing in western Sudan who presented with a 7-month history of generalized body weakness, easy fatigue, and frequent attacks of vomiting and diarrhea. Physical examination and laboratory investigations confirmed the diagnosis of Addison's disease due to Histoplasma capsulatum var duboisii infection of the adrenal glands. He was treated with intravenous hydroco…

Reduction in false antigenemia in the second generation Histoplasma antigen assay.

We have previously reported false Histoplasma antigenemia in solid organ transplant patients who received rabbit antithymocyte globulin (ATG). False antigenemia was caused by human anti-rabbit antibody, produced in response to ATG administration. Those specimens were obtained through a research study to assess the development of anti-rabbit antibodies following ATG administration. Here we compare the likelihood of false antigenemia between the original and second generation assays in clinical specimens obtained specifically for the diagnosis of histoplasmosis. Of 12 serum specimens that were f…

Interaction of Histoplasma capsulatum with human macrophages, dendritic cells, and neutrophils.

Histoplasma capsulatum (Hc) is a dimorphic fungal pathogen indigenous to the Ohio and Mississippi River Valleys in the United States. Infection is initiated by inhalation of microconidia or small mycelial fragments into the terminal bronchioles of the lung. The conidia are taken up by alveolar macrophages (Mphi), in which they convert to the pathogenic yeast phase. The yeasts replicate in the alveolar Mphi and other Mphi recruited to the lung as part of the inflammatory response. Thus, the yeasts are able to disseminate from the lung to other organs, such as the liver and spleen. As a facultat…

Molecular cloning, characterization, and expression of the M antigen of Histoplasma capsulatum.

The major diagnostic antigens of Histoplasma capsulatum are the H and M antigens, pluripotent glycoproteins that elicit both humoral and T-cell-mediated immune responses. These antigens may play a role in the pathogenesis of histoplasmosis. M antigen is considered immunodominant because antibodies against it are the first precipitins to arise in acute histoplasmosis and are commonly present during all phases of infection. The biological activity of monomolecular M antigen and its ability to elicit a protective immune response to H. capsulatum are largely unknown. A molecular approach was used …

[Cerebral miliary granulomatosis with Histoplasma capsulatum in an HIV seronegative patient].

A 51 year old patient who worked in Africa for eight years, presented twelve years later a progressive ataxia associated with headaches. Neuroimaging studies done after a partial complex seizure demonstrated multiple supra and sub-tentorial cortical ring enhancing lesions. Histoplasma capsulatum histoplasmosis was found on histological examination of brain biopsy and confirmed by isolation of the fungus. Medical treatment with intravenous amphotericin B followed by oral itraconazole (400 mg per day) improved both clinical and radiological status. This observation of cerebral histoplasmosis is …

Molecular epidemiology, pathogenesis, and genetics of the dimorphic fungus Histoplasma capsulatum.

Histoplasma capsulatum, the causative agent of the most common systemic fungal infection, histoplasmosis, has become subject to increasing study in parallel with rising prevalence of human immunodeficiency. This review presents a summary of the advances made in the investigation of H. capsulatum genomics, molecular epidemiology, pathogenesis, and molecular genetics.

Duration of fungal culture incubation in an area endemic for Histoplasma capsulatum.

To determine the optimum incubation period for recovery of fungi from clinical specimens in an area where Histoplasma capsulatum is endemic, we reviewed the results of 4259 consecutive fungal cultures. Of the total 1306 fungi isolated, 92.6% were detected by day 7, and 97.9% were detected by day 14. For the 1027 yeast isolates, only 1.4% were detected after day 7, none of which was considered clinically significant. Of the 281 mould isolates, 91.1% were detected by day 14; and of those detected after day 14, only isolates of H. capsulatum were considered clinically significant. An incubation p…

Identification of Histoplasma capsulatum organisms in the pleural and peritoneal effusions of a dog.

Histoplasma capsulatum organisms were identified by cytologic evaluation in the thoracic and abdominal effusions of a 5-year-old sexually intact male Cocker Spaniel that was referred because of anorexia and lethargy. Treatment with amphotericin B and ketoconazole was instituted. The dog developed respiratory arrest, a complication of the disseminated disease, and died. Necropsy findings included pleural effusion, hepatomegaly, and enlarged tracheobronchial, hilar, mediastinal, and mesenteric lymph nodes. Granulomas containing periodic acid-Schiff (PAS)-positive yeast-like organisms identified …

Histoplasma variation and adaptive strategies for parasitism: new perspectives on histoplasmosis.

This review summarizes the biology of Histoplasma capsulatum in relation to a wide variety of corresponding pathologies in histoplasmosis. Features of these disease syndromes can be explained in part by natural variations within the fungal population and adaptations made by individual organisms to specific environments. H. capsulatum grows as mycelia and conidia in the soil; once inhaled, the organism undergoes a dramatic morphological and physiological conversion to a yeast form. The yeasts proliferate within the phagolysosomes of macrophages, using a variety of specific strategies for intrac…

Histoplasma capsulatum infections of the central nervous system. A clinical review.

Central nervous system manifestations occur in 10 to 20% of patients with disseminated histoplasmosis. Additionally, histoplasmosis may be the cause of cases of chronic meningitis in patients with no other evidence for dissemination. Histoplasmosis may also cause cerebral or spinal cord mass lesions resembling neoplasms or abscesses, and encephalitis. Diagnosis of chronic meningitis or mass lesions caused by H. capsulatum may be difficult and involves careful analysis of serologic tests for antibodies, cultures and tests for HPA in body fluids. Amphotericin B remains the treatment of choice, b

[Disseminated histoplasmosis caused by Histoplasma capsulatum and sarcoidosis. Apropos of a case].

Histoplasma capsulatum can, sometimes, simulate sarcoidosis, clinically and pathologically. A case is reported in which disseminated histoplasmosis is proved only by fungus granulomatous tissue. Another similar cases of the literature incite to propose the practice of special stains, above all Gomori-Grocott technical, before sarcoid-like lesions. Lesions pathogeny was discussed. In our case it's not possible to eliminate the role of primary sarcoidosis in disseminated histoplasmosis.

Irreversible block of the mycelial-to-yeast phase transition of Histoplasma capsulatum.

p-Chloromercuriphenylsulfonic acid (PCMS), a sulfhydryl inhibitor, prevented the mycelial-to-yeast transition of the dimorphic fungal pathogen, Histoplasma capsulatum. The effect of PCMS was specific for the mycelial-to-yeast transformation; it had no effect on growth of either the yeast or mycelial forms or on the yeast-to-mycelial transition. The failure of PCMS-treated mycelia to transform to yeast was permanent and irreversible. PCMS-treated mycelia could not infect mice but could stimulate resistance to infection by a pathogenic strain of Histoplasma capsulatum. These results suggest a ne…

Successful management of Histoplasma capsulatum infection of an abdominal aortic aneurysm.

A 65-year-old woman with disseminated histoplasmosis underwent resection of an atherosclerotic abdominal aortic aneurysm. Yeast forms of Histoplasma capsulatum were present in the aneurysm. Surgical resection and revascularization with a Dacron graft followed by systemic amphotericin B therapy and chronic ketoconazole suppressive therapy have resulted in a patient without symptoms 15 months postoperatively. It is important to be aware of the potential for artherosclerotic aortic aneurysm involvement by H. capsulatum.

[Disseminated Histoplasma duboisii histoplasmosis in a female inhabitant of the Ivory Coast].

The authors describe a disseminated multivisceral form of african histoplasmosis due to H. duboisii in a 45 years old black woman. Clinical examination shows characteristic lesions of skin, bones and lymph glands. Lymphography reveals lesions of deep lymph glands and laparascopy lesions of liver and spleen. First untimely stopped, treatment was started again with ketoconazole. 20 months later, the course of the disease is, as for as we can appreciate it, favourable.

Adoptive transfer of immunity to Histoplasma capsulatum in athymic nude mice.

Studies of adoptive transfer of resistance to Histoplasma capsulatum were performed with athymic nude (nu/nu) recipients and heterozygous (nu/+) donor mice. This model has the benefit of employing a recipient mouse (nu/nu) which cannot be immunized actively against H. capsulatum. Nu/nu mice were protected against H. capsulatum by whole spleen cells, fractioned T lymphocytes, and macrophages from immunized nu/+ donor mice. Similar cell populations from nonimmune nu/+ mice were not protective. Treatment with gold sodium thiomalate, an inhibitor of macrophage function, increased susceptibility to…

[A case of disseminated histoplasmosis due to Histoplasma capsulatum (author's transl)].

A case is reported of American histoplasmosis due to Histoplasma capsulatum presenting as a hepatic granulomatosis. The patient was a young adult from Haiti and investigations revealed the presence of an ulcerous enterocolitic suggesting tuberculosis or Crohn's disease. The condition became worse after antituberculous therapy and corticoids, and operation was necessary. Extemporaneous histological examination, of the colonic ulcerations showed Histoplasma capsultaum yeasts. The patient, who had a cellular immunity deficiency, died during antifungal treatment with amphotericin B of the dissemin…
