INTRODUCTION: Ophthalmoparesias is a frequent complication of ophthalmic herpes zoster. It occurs in 31% of all cases. However, the presence of Horner's syndrome during viral reactivation is a rarity which has only been previously described on two occasions, and never associated with cranial nerve involvement.
CLINICAL CASE: We describe a patient with the first case of Horner's syndrome secondary to ophthalmic herpes zoster, with simultaneous, homolateral lesions of the third and sixth cranial nerves. Clinical evaluation, the course of the disorder, negative magnetic resonance studies and tes

A 7-year-old boy developed a Horner's syndrome after falling on a stick that penetrated his peritonsillar soft palate. He did not suffer from any major vascular injury, and pharmacologic testing indicated a preganglionic lesion. We review previously reported cases of oculosympathetic paresis caused by surgical and nonsurgical intra-oral trauma. Because of the proximity between sympathetic and vascular structures in the lateral and parapharyngeal space, Horner's syndrome in the setting of intra-oral trauma should prompt evaluation of the internal carotid artery. Magnetic resonance imaging may b

The triad miosis, ptosis and hemifacial anhidrosis make up the well-known Horner's syndrome. This is caused by a lesion or a dysfunction in the "oculosympathetic pathway" running from the hypothalamus to the eye. Various disease entities may manifest themselves in this way, and it is important to be able to determine whether the dysfunction is localized to the central, preganglionic or postganglionic neurone. This review presents the various tests used for this purpose, describes our recent contributions in the field, and finally proposes a simple scheme that is suitable for topographical diag

Horner's syndrome is derived from an interruption of the sympathetic pathway at any point along its course between the hypothalamus and the orbit. This pathway is generally considered to consist of three neurons. It had been suggested that the pupillary response to various pharmacologic agents will aid in localizing the lesioned neuron in the sympathetic pathway. We had the opportunity to observe a patient who developed Horner's syndrome as a consequence of pulmonary apex adenocarcinoma. Clinical, radiologic and pharmacologic studies are presented. The methods of pharmacologic localization are

Two patients with congenital Horner's syndrome had unilateral facial flushing. Both showed pupillary supersensitivity to epinephrine as well as anhidrosis on the affected side of the face and neck. Facial skin temperature after exercise increased on the intact side, but decreased on the affected side. Thermal vasodilation in the major portions of the face is regulated by sympathetic vasodilator fibres, and less predominantly by adrenergic vasoconstrictor fibres. The asymmetry of facial flushing may have been caused by impaired sympathetic vasodilation and further intensified by active vasocons

Sympathetic fibers destined for the eye join the abducens nerve for a short distance within the cavernous sinus; thus, a unilateral sixth nerve palsy with an ipsilateral Horner's syndrome is of significant localizing value. We report two cases of cavernous sinus lesions producing an ipsilateral abducens palsy and Horner's syndrome: one case due to an intrinsic and the other to an extrinsic lesion of the sinus region.

Eight patients with Horner's syndrome (five with a 1st neuron lesion and three with a 2nd neuron lesion) were examined for their pupillary responses to pharmacologic stimulation with tyramine (2%) and with phenylephrine (1%) eye drops. The same patients were also evaluated for their forehead sweating pattern on stimulation with body heating and pilocarpine injection, using the Evaporimeter. Five patients had a brain stem (1st sympathetic neuron) lesion, while three patients had had a traumatic C8-Th1 root avulsion and hence had a preganglionic neuron lesion. The average response with the pheny

A preganglionic Horner's syndrome developed shortly after ipsilateral percutaneous insertion of a Swan-Ganz catheter in the internal jugular vein for management of pulmonary edema. There were no other neurologic findings nor evidence of mass lesions in the neck or pulmonary apex. This is believed to be the first reported case of Horner's syndrome complicating Swan-Ganz catheterization, which may occur more commonly than is generally appreciated.

In 14 patients with unilateral Horner's syndrome there was a significant increase in central corneal thickness on the affected side (mean difference 4.86 micron +/- SEM 1.71 micron; P less than 0.02). It is suggested that the sympathetic nerve supply of the eye is of importance for corneal hydration. Different possible mechanisms are discussed. Among the investigated patients no permanent ocular hypotonia or hyperaemia was found. On slit-lamp investigation there was no corneal change. The corneal sensitivity was equal on both sides.

Two cases of Horner's syndrome complicating lumbar epidural block in pregnant women in labour are described. The aetiology of the complication is discussed and it is suggested that epidural injections should not be made while the patient is in the lithotomy position.

Four cases of Horner's syndrome, two in dogs and two in cats, are described. Miosis, ptosis, and enophthalmos were present in three of the cases and in addition, protrusion of the nictitating membrane was present in the fourth case. In the two cases described in cats, there was also evidence of peripheral vasodilation in the skin of the face on the same side as the Horner's syndrome.
